I conducted a walk-through of the facility unaccompanied. During the visit, I discussed the allegations with Deborah Evans, Owner, Queen Smith, Administrator, and one additional staff member. Limited monitoring of child care requirements occurred during today’s visit. The following violations were observed and/or confirmed during today’s visit. Violation Number Comment Rule 1043 All staff records, except financial records, were not made available for review. The owner did not have a file available for review. G.S. 110-91( 9) 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). A staff member's qualification letter expired October 2025, has not received a new qualifying letter. Staff member must have a valid qualifying letter by December 4, 2025. G.S. 110-90.2(b) & .2703(n)&(o) 1757 A valid qualification letter was not on file and available to review at the facility. A therapist working alone with a child while in care does not have a valid qualifying letter. G.S. 110-90.2(b) & (d) & .2703(e) Violations must be corrected immediately. Queen Smith, administrator, assisted me with the visit. Your program currently operates with a four-star license, issued March 31st, 2019, earning seven (7) points in the education component, two (2) points in the program standards component (meeting enhanced space and 1 quality point. Restrictions include daytime and overnight and meets enhanced space. The NC Secretary of State website was reviewed on September 15, 2025, and Little Angels ChildCare and Developmental Center Incorporated was listed as current- active. The last annual compliance visit was conducted on September 30, 2024. The sanitation inspection was completed on March 26, 2025, with a “Superior” classification. The last fire inspection was conducted on June 27, 2025, and your facility was approved for daytime and night care. The center's compliance history is ninety-seven percent (97%) and it was reviewed with the operator. The Staff and Training Worksheet, The Children's Records Worksheet and the Annual Compliance Monitoring Checklist were completed during the visit. I visited each indoor and outdoor space with me. Children were observed in free play activities, singing with their teacher and making a dog shaker in the PreK classroom. Bean and cheese burritos, mixed vegetables, mandarin oranges, and milk. The following violations were observed: Violation Number Comment Rule 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. J. Oxendine began providing breaks in the classrooms on 5/22/25.She did not successfully complete certification in First Aid appropriate to the age of children in care. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. J. Oxendine began providing classroom breaks on May 22, 2025 and she did not successfully complete certification in CPR training appropriate to the age of the children in care. .1102(d) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. M.
