BENSON Operation Type: Family CC Home Case Number: Visit Date: 1/30/2024 Number Present: 3 Completed Date: 1/30/2024 Age: From 1 To 1 Total Minutes: 90 Time In: 08:50 AM Time Out: 10:20 AM Time In: Time Out: List to Use: Family CC Home Type Of Visit: Temp Time Period Announced/Unannounced: Unannounced The purpose of today's visit was to monitor compliance with applicable child care requirements pertinent to the Second Temporary Time Period Visit. This program currently operates with a temporary license effective November 7, 2023, to March 18, 2024. Applicable childcare requirements were discussed with Mrs. Felicia Fleming, Operator. All rooms approved for child care were monitored, including the outdoor space. Three (3) children all one year of age were present. Children were observed playing in the designated space for child care. I observed appropriate interactions between Mrs. Fleming and the children. Children were adequately supervised. Program records, children files, and the operator’s file were monitored during the visit. The children were served pancakes, bananas, and milk for morning snack. The operator is scheduled to take the EPR training on February 6, 2024. At the completion of the visit, the Visit Summary was reviewed with Mrs. Fleming. Violation Number Comment Rule 1103 For each child being transported, the operator or other transportation provider did not have identifying information, including the child's name, photograph, emergency contact information, and a copy of the emergency medical care information form required by Rule .1721(a )(3) in the vehicle whenever children were transported. She did not have the required identifying information for each child when she transported the children. .1723(13) 1892 Application was not on file for each enrolled child, including his/her own preschool child(ren) who are not school-age. There was no file for two school age children that are related to the operator. .1721(a)(3) 1996 Vehicle used for transportation did not have a first aid kit. The provider did not have a FA kit in the vehicle she used for transportation. .1723(11) 1997 Vehicle used for transportation did not have a fire extinguisher. A fire extinguisher was not in the vehicle used for transportation. .1723(11) The violation/s documented above must be corrected immediately. A signed and dated letter of compliance must be mailed to me by February 13, 2024, describing how and when the violation/s were corrected. You should email the compliance letter to leisa.benson@dhhs.nc.gov . If you state in the compliance letter that corrections have been made when they have not, this will be considered falsification of information. If you cannot meet the requirements by February 13, 2024, you must contact me with a purposed timeline of the corrections. In some cases, this timeline may be extended. You are required to always maintain compliance with all applicable child care rules and regulations. NC GS 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past eighteen (18) months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. Violations cited during today’s visit, may negatively affect your compliance score. As of today’s, visit your compliance history is 97%. Your rated license application cannot be processed until the violations listed above have been corrected. Technical Assistance: Because you have an AAS in ECE you are required to receive eight (8) hours of in-service trainings annually. All in-service training hours should be logged on the On-Going Training Documentation Form and copies of certificates should be kept in each caregiver’s file and available for review by a representative of DCDEE. Health and Safety training topics must be completed within the first year of employment and then within every five (5) years after. Documentation of training must be kept on file for review. Complete the health and safety training record as found on the DCDEE website. Health and Safety trainings may be found on the DCDEE website using the Moodle platform. You can access information for the Health and Safety Trainings by visiting: https://ncchildcare.ncdhhs.gov/Provider/Training-and-Professional-Development/Health-and-Safety-Trainings Thanks to additional grant funding, testing for lead in your drinking and cooking water is now available for free through the Clean Water for Carolina Kids program on a first-come, first-serve basis. Follow these steps to lead-free drinking water: 1. Enroll at www.cleanwaterforcarolinakids.org using your access code, then your test kit will arrive at your address in about one week. 2. Collect and ship your water samples according to the training webinar. 3. Get your results and, if needed, follow-up support and recommendations to get the lead out! You are encouraged to take part in this free testing to help ensure the safety of children across North Carolina. Please note that participation in this free testing is not required at this time. I discussed with Mrs. Fleming that she cannot be approved for transportation until I receive her compliance letter. Transportation requirements were reviewed with her. I monitored the two (2) car seats and one (1) booster seat that she will use for transportation. They have not expired. I discussed with her that she can check the expiration dates for car seats/booster seats by looking for the label or printed area that lists the car seat date of manufacture. In general, car seats expire between six (6) and ten (10) years from the date of manufacture. They expire for a number of reasons, including wear and tear, changing regulations, recalls, and the limits of manufacturer testing. I discussed with her that she is required to have identification information for all children that are transported. She can make copies of each child’s application with the most recent picture of the child attached to it and keep in a notebook in the vehicle she will use for transportation. Mrs. Fleming has two (2) school age children that are related to her living in the home. I discussed with her that because she is responsible for them when the parent is not present they are counted in her capacity and should have an individual file.
