S. Ridenour, Lead Teacher, assisted me with today’s visit. Your program currently operates with a four-star license, issued February 20, 2025, earning 7 points in the education component, 2 points in the program standards component and 1 quality point for seventy-five percent of lead teachers and teachers having at least ten years documented early childhood work experience that can be verified by the DCDEE. Your program was also monitored for compliance with implementing an approved curriculum (Creative Curriculum) as required for all four- and five-star licensed facilities where four-year-old children are enrolled. This is the program’s first annual compliance. The sanitation inspection was completed February 14, 2025, with a “Superior” classification. The last fire inspection was conducted January 7, 2025, and your facility was approved for daytime care only. I visited each space indoors and outdoor play area. Children engaged in morning routines, completing toileting and hand washing routines, free choice play in various activity areas, and interacting with teachers. Proper hand washing techniques and diaper changing procedures were observed. Breakfast and lunch were observed during today’s visit. Breakfast consisted of chicken biscuits, craisins and milk and lunch consisted of Teriyaki beef tips, mixed rice, broccoli, pears, and milk. The NC Pre-K requirements in section .3000 of the child care rules were monitored for compliance. The NC Pre-K Program Site Monitoring Tool was not reviewed today. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified in compliance. The current NC Pre-K staff are lead teachers, S. Ridenour, T. Sykes, and K. Speller and teachers, S. Smalls, Benton, C. Jones, D. Ambrose. A selection of files were monitored for completed health assessments and developmental screenings. Brigance assessments were completed in the six files reviewed today. Health assessments included the required screenings. The center uses the Teaching Strategies Gold instrument to document evidence of children's ongoing progress. Checkpoint assessments are conducted three times per year: at the beginning, middle and end of the school year. The first day of school was 9-2-25. The classroom operates from 8:25 am to 3:45 pm. Parent conferences are held three times per year. You stated that the program uses daily communication folders, school all-calls and the reminder app to communicate with parents. The program has not completed the environment rating scale assessment. The program is licensed for four NC Pre-K classrooms; however, one classroom is currently not being used. You reported the space is being utilized by kindergarten. COMPLIANCE HISTORY SCORE: The center's compliance history was reviewed with the operator. The program’s compliance history was eighty-four percent as of December 10, 2025. FACILITY PROFILE INFORMATION: You verified the email address and mailing address listed on the facility profile are correct. You stated the telephone number needs to be updated. I explained written documentation must be submitted formally requesting the change. The written documentation should include the new telephone number for your program. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS on an ongoing basis as staff members are hired and when their employment is terminated. I informed you that qualification letters must be on file and available for review at the program site. You stated the documents are kept at the central office and are provided as needed. I advised you to retrieve copies of the qualification letters from staff or the central office, so they are available on-site during monitoring visit. During the visit, you retrieved a copy of the qualification letter for the substitute. We discussed the option of substitute having mobile file to bring when they work at the facility. This will ensure all required documentation is onsite and available for review. I provided you with a copy of the substitute file checklist during the visit. ABCMS TRAINING: As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS on an ongoing basis as staff members are hired and when their employment is terminated. All licensed child care centers must register to receive e-mail notification when a registered sex offender moves within a one-mile radius of the center. S. Ridenour, Lead Teacher, assisted me with today’s visit. Your program currently operates with a four-star license, issued 2/20/25, earning 7 points in the education component, 2 points in the program standards component and 1 quality point for 75% of teachers who have an AAS or higher in ECE/CD. The last annual compliance visit was conducted 2/17/25. The sanitation inspection was completed on 1/6/25 with a “Superior” classification. The last fire inspection was conducted on 1/7/25. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety one percent as of 5/9/25. This facility is owned by Washington County Schools. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. Children throughout the facility were participating in group time, free play in activity areas, transitions, and personal care routines. Children in space #1 were creating butterfly crafts with stickers and stamps. Children in space #2 and space #3 were observed in the outdoor space riding bikes and climbing on the stationary structure. All children transitioned to the cafeteria to eat lunch. The outdoor space was observed to have a stationary climber, picnic table, riding toys, and balls for the children to engage in gross motor activities. Lunch was observed and consisted of hotdogs on buns, sweet potato fries, baked beans, pears, and milk. The following violations were documented. A child enrolled in space #2 left the outdoor space to go to the bathroom located in the classroom while a staff member stayed in the outdoor space with the door open. Staff must remain with the children assigned to them at all times. Leaving a child or a group of children alone is unacceptable. Younger children need closer supervision to remain safe. Active supervision requires focused attention and intentional observation of children at all times. Caregivers/teachers position themselves so that they can observe all of the children: watching, counting, and listening at all times. Supervision is directly tied to safety and the prevention of injury and maintaining quality child care for infants, toddlers, preschoolers, and school-aged children. A child enrolled in space #2 left the outdoor space to go to the bathroom located in the classroom while a staff member stayed in the outdoor space with the door open. The full door to the bathroom was closed so the child was not able to be seen or heard. We discussed when children need to use the restroom, children must accompany the teacher so that the staff members remain in ratio in the outdoor and indoor space, or another teacher needs to be called to come stand in the outdoor space or accompany the child. We discussed that staff members are inside or outside and cannot straddle the door to maintain supervision and ratio. You stated that there is usually a floater that can be used for assistance. Ensure that staff members know to contact someone for assistance when needed and that they must maintain ratios and supervision while children come inside to complete personal care routines. Medication: Medicines can be crucial to the health and wellness of children. They can also be very dangerous if the wrong type or wrong amount is given to the wrong person or at the wrong time. A medical action plan is required for children that need emergency medication, the form needs to be completed by the parent or a health care professional. All medications for children present at the center must have a current, completed medication permission form even if there is a Medical Action Plan. Medication permission forms are valid for a defined length of time and must be updated, if needed. A child enrolled in space #2 had a Ventolin HFA inhaler on file with written permission from the parent that expired 3/24/25. You stated that you had given a permission form for the inhaler but did not receive it from the parent. We discussed adding a reminder in a digital calendar to will remind you in advance of the permission form expiration date and a second reminder when you give the parent to form to ensure that you receive it. Both indoor and outdoor learning environments, staff and a sampling of children’s files, and other required program records were monitored. Four groups of children were present during today’s visit. Children throughout the center were engaged in free play. Developmentally appropriate toys were available on low-lying open shelves. Children transitioned to personal care routines to prepare for lunch. Lunch was served and consisted of hotdogs, baked beans, applesauce, and milk. Fire Inspection: 1/7/2025 Sanitation Inspection: 1/6/2025 This facility owner Washington County Schools and is registered as current/active on the Secretary of State’s website. The following violations were documented. Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. Outlets in the unlocked storage barns located on the playground did not have safety covers. 10A NCAC 09 .0604(c) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. In space three(3) an aerosol can of Equate shaving cream was stored in the unlocked cabinet above the sink. .2820(b) Child Care Licensing Requirements are established to ensure a safe, healthy, and developmentally appropriate environment for all child care facilities. Therefore, it is important for you to be knowledgeable of all licensing requirements that apply to your facility. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. It is your responsibility to maintain compliance with the applicable laws and rules; always whether they have been monitored or specifically discussed with you in the past. The best way for you to ensure that you are meeting all requirements is to periodically review the childcare laws and rules on the Division of Child Development and Early Education (DCDEE) website. Technical Assistance: Approximately 2,400 children are injured annually by inserting objects into the slots of electrical outlets. Outlets in the unlocked storage barns located on the playground did not have safety covers. All electrical outlets not in use that are in areas accessible to children must be protected with an outlet cover. This includes unused outlets on power strips. This violation was corrected during the visit. Storage of Hazardous Materials – Following the enhanced cleaning and sanitizing requirements is extremely important during this cold and flu season, however, having these items available for quick and easy use can lead to improper storage. Ensure that staff are keeping items that are not required to be locked, a minimum of 5 feet above the floor and out of reach of children. Aerosol cans must be under locked storage In space three(3) an aerosol can of Equate shaving cream was stored in the unlocked cabinet above the sink. Take a fresh look at where and how these items are stored to make them quick and easy to access when needed but still maintain the safety of the children in care. Refer to the Center for Disease Control (CDC) website for more information on best practice. This violation was corrected during the visit. Reminders: Linking Criminal Background Checks – Staff need to log into the Criminal Background Check Portal on the DCDEE website and link to the facility license number #94000113. It is important to do this so staff at DCDEE can contact the center if a random criminal history check uncovers a staff member with a criminal charge or conviction working in a child care facility. It is also a way to for staff at the CBC Unit to keep you apprised of the status of out-of-state checks if a new employee has a Provisional Qualification. Instructions on how to link to the facility was provided today. In the future, new staff need to link within five days of hire or transfer to your center.
