Jennifer Bradshaw Garrett accompanied me on today's visit. D. Coston, Family Advocate, and M. Armond, Lead Teacher assisted me with the visit. Your program currently operates with a five-star license, issued March 6, 2026, with daytime care only and meeting Head Start Early Head Start ratios. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-two percent (92%) as of April 15, 2026. The NC Secretary of State website was reviewed on April 15, 2026, and Economic Improvement Council, Inc. was listed as current- active. The new license was not posted. D. Banks, Early Head Start/Head Start Director was contacted via telephone to discuss receipt of the license and she stated the license was enroute to the center. Ms. Banks submitted via email an updated legal designee form indicating lead teachers as the legal designee for the facility. A walk-through of the facility was completed today; all indoor and outdoor areas were monitored. Children throughout the facility were participating in group time, free play in activity areas, transitions and personal care routines. In space # 1 children were observed eating breakfast, participating in group time where they identified colors and numbers, and brushing their teeth. In Space # 2 the children were observed in group time. They identified their name on a file card, placed it in their class mailbox, and then chose their center. One teacher sat on the floor and joined the children in block play. The second teacher assisted the children with sponge brush painting. The children chose what color they painted with. No new staff have been hired since the last visit. The sanitation inspection was completed March 3, 2026, with a “Superior” classification. The last fire inspection was conducted November 25, 2025, and your facility was approved for daytime care only. Breakfast and lunch was observed. Breakfast consisted of blueberry muffin, grape juice, and milk. Lunch consisted of Beefaroni, garlic bread, peaches, eggs, and milk. The following violation was documented. Violation Number Comment Rule 805 Fire drills were not practiced monthly and/or the drill record was incomplete. Documentation was not on file verifying a fire inspection was completed for March 2026. .0604(t); .0302(d)(5) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before April 30, 2026, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware any information submitted by you is legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted. A fire drill was not documented for the month of March 2026. You said a fire drill was completed for the month of March; however, the designated person who completes the documentation may have forgot to record it. Monthly fire drills must be documented on the emergency drill log. The new license was issued on March 6, 2026, but was not posted. It must be posted immediately. The Classroom Staff to Child Ration for Accreditation or Head Start form must be posted in each classroom. An employee arrived during the visit and posted the updated license and staff/child ratio charts. SURVEY REQUEST: Elizabeth City State University (ECSU) is exploring the possibility of offering their Early Childhood Education program in an online format, potentially as early as this fall. A survey was released today to gather interest and feedback from individuals who may want to take courses. The survey should be completed by this Friday. S. Lee, Teacher, assisted me with the visit. Your program currently operates with a two-star license, issued April 7, 2020, earning 3 points in the education component, 1 point in the program standards component (meeting enhanced space and enhanced ratios) and 1 quality point for having reduced group size by one in each group from the seven-point level as required in Rule .2818(c). The last annual compliance visit was conducted on January 13, 2025. The sanitation inspection was completed May 20, 2025, with a “Superior” classification. The last fire inspection was conducted November 25, 2025, and your facility was approved for daytime care only. I received a copy of the fire inspection report during the visit. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-five percent as of December 12, 2025. The NC Secretary of State website was reviewed on December 12, 2025, and Economic Improvement Council, Inc. was listed as current- active. I visited each indoor space and the outdoor space with me. Children enrolled in space #1 were eating breakfast, brushing teeth, engaged in morning activities, completing toileting routines, and participating in outdoor play. Children in Space #2 were completing circle time, and a coloring activity. Both groups were observed interacting with teachers during morning routines. Proper hand washing techniques were completed. Breakfast was observed and consisted of cereal, orange juice, and milk. The following violations were documented: Violation Number Comment Rule 528 Food substitution was not of comparable food value or recorded on the menu prior to the meal or snack being served. Orange juice was not recorded as a substitute for bananas on the menu prior to the meal or snack being served. 10A NCAC 09 .0901(b) 618 Diaper changing surfaces were not kept free of storage. A clip board was observed on the diaper changing table in Space #1. Also, a staff member placed number cut-outs on the diaper changing table prior to posting on the wall in Space #1. STAFF RECORDS: An employment application was not on file for one employee hire don 9/2/2025. You stated the application was on file in the administrative office and could be sent over via email. You provided a copy of the application prior to the end of the visit for verification and then placed in the staff file. Be reminded staff files should be complete with all documentation and onsite to ensure they are available for review and compliance is maintained. CPR/FIRST AID Documentation verifying completion of CPR/FIRST Aid was not on file for one employee hired on 9/2/2025. I reminded you that any staff providing direct care to children must complete and maintain a record of completing CPR/First Aid training from an approved agency within 90 day form the date of employment. The training should be scheduled immediately and training cards/certificates placed in the file for verification. It was suggested designating an qualified person to conduct periodic file reviews for new staff as well as current employees to ensure all required documentation is available to be reviewed during monitoring visits. This will help ensure compliance is maintained for recordkeeping. The First Aid/CPR training certificate was emailed over and placed in the file prior to the end of the visit. SANITATION & HEALTH: In Space #1, a teacher was observed placing number cut-outs on the diaper changing table to add to the wall. When asked, the teacher stated the diaper changing table was used sometimes for changing. I informed the teacher the cut-outs could not be placed on the diaper changing table and they were immediately removed. The clip board was moved also during my observation. A blanket and cot sheet were observed on the diaper changing table in Space #2. The teacher in that space indicated that the diaper changing table is not used. Be reminded the diaper changing surface must remain free of storage except those used during the diaper changing procedures. NUTRITION: Cereal, orange juice and milk were served for breakfast; however, the menu listed bananas as a component instead of orange juice. Based on my review of all menus posted, orange juice was not recorded as a substitute for the bananas on the menu prior to the meal or snack being served. I informed the cook the item must be recorded as a change prior to providing the component to the children. I also reviewed the substitution sheets for each space and there was no documentation for meals served today. This item was corrected as the cook made the necessary changes and added to the classroom substitution sheets. QRIS CONVERSATION: A meeting was conducted with all Head Start administrative staff on September 22, 2025, to review and discuss the QRIS transition plan and pathways. During the meeting information was shared about the three pathways and requirements. Applications requesting completion for the QRIS Pathway to the Stars have been emailed to the Head Start director. The Director was encouraged to complete the application and submit it when ready to begin the transition process. These academies enhance educator competencies, provide comprehensive training, and offer ongoing support at no cost to participants. Each academy delivers a comprehensive curriculum that meets or exceeds DCDEE training standards for new child care professionals and includes information related to Quality Rating Improvement System (QRIS) program standards. Training includes CPR/First Aid, Health and Safety, Infant/Toddler Safe Sleep and Sudden Infant Death Syndrome (ITS-SIDS), Playground Safety, Recognizing and Responding to Suspicions of Child Maltreatment, and instruction on the Infant/Toddler Environment Rating Scale, Third Edition (ITERS-3) and Early Childhood Environment Rating Scale, Third Edition (ECERS-3). Participants also receive an overview of North Carolina Foundations for Early Learning and Development and support in completing DHHS Criminal Qualifying Background Checks. The academies offer training in multiple formats across various counties to ensure statewide accessibility, incorporating both virtual and in-person learning opportunities. This initiative aligns with North Carolina's Preschool Development Grant Birth through 5 (PDG B-5) goals of expanding access to high-quality early learning programs and supporting workforce development, contributing to long-term workforce stabilization and strengthening the economic infrastructure by enabling parents to remain in the workforce. New Sanitation Rules - DCDEE and the Department of Public Health have received feedback from child care programs that additional guidance would be helpful in implementing the child care sanitation rules that the Public Health Commission readopted effective July 1, 2023. Please know demerits will be taken for new requirements of these rules starting January 2024. Emergency Preparedness and Response (EPR) Requirements, Emergency Medical Care Plan (EMCP) and Ready-to-Go File – Your Emergency Preparedness and Response (EPR) Plan needs to be reviewed and updated annually. *Log into the Emergency Management Portal on the DCDEE website -> Provider tab -> Emergency Preparedness and Response -> Emergency Management Portal -> Log in using NCID used to create or update EPR Plan. Review EPR Plan and update as needed. Document review date on page 28. If changes are needed, print the plan up to page 29. If no changes are needed, print page 28 with the review date reflected. Review annually with staff and during orientation with new staff. Staff Education and Works - Formal education (NCECC, college coursework, Principal and Teaching Licenses, BSAC training certificates, CDA Certificates, and Early Educator Certificates) are submitted to DCDEE through WORKS, an on-line portal where you can register, request to be approved for various positions, and access your education information once evaluated and posted. You can now request to have your transcripts sent electronically from colleges attended. *Staff education verifying qualification for the position hired, must be submitted to WORKS within six (6) months of hire or change of position. *Staff not meeting formal education requirements for specific positions must enroll in required coursework within six (6) months of hire or change of position and complete the NCECC (lead teacher and program coordinator) within 18 months after enrollment or NCECAC (administrator) within 24 months after enrollment. *When education is posted, staff need to log back into Works and print a copy of their Education Status Letter for their file. On-going Training Hours – Review Row 18 on the Staff Worksheets to determine how many on-going training hours returning staff will need by 2/20/2024 (anniversary of this Annual Compliance Visit). New staff will complete Health & Safety in their first year and then will complete on-going training annually based upon their level of education posted in DCDEE Works. *ECE college coursework or coursework leading to an ECE degree counts towards on-going training hours. Each semester credit counts as 16 hours so a 3-credit classes also earns 48 hours of on-going training.
