J. Bryan, Administrator, assisted me with the visit. Your program currently operates with a three-star license, issued 4/7/20, earning 5 points in the education component, 2 points in the program standards component (meeting enhanced space) and 1 quality point for center has a combined turnover rate of 20% or less for the administrator, program coordinator, lead teacher, teacher and group leader over the last 12 months. The last annual compliance visit was conducted 4/9/25. The sanitation inspection was completed on 12/12/24 with a “Superior” classification. The last fire inspection was conducted on 9/6/24. The center's compliance history was reviewed with the operator. The program’s compliance history was eighty nine percent as of 5/23/25. The NC Secretary of State website was reviewed on 5/23/25 and Emonnie’s Little Angels Inc. was listed as current- active. I visited each indoor and outdoor space. Children were engaged in free play. The children transitioned to washing their hands before breakfast was served. After breakfast the children washed their hands and transitioned to free play. The outdoor space included stationary equipment, ride on toys, balls, and storage for extra materials. Proper handwashing was observed. Breakfast was observed and consisted of shole grain toast, oatmeal, juice, and milk. The following violation was documented. Violation Number Comment Rule 303 Children were not adequately supervised at all times. Upon arrival, a staff member opened the door to let me in. There were three (3) children in space #3 alone while the staff member opened the door to let me in. .1801(a)(1-5) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before 6/12/25, I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected. Please be aware any information submitted by you is legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. Parents/guardians have a contract with caregivers to supervise their children. Upon arrival, a staff member opened the door to let me in. There were three (3) children in space #3 alone while the staff member opened the door to let me in. The staff member stated another staff member was on their way. When only 1 staff member is present, they are to take all the children with them when they need to open the front door. A violation was documented for failure to maintain compliance with supervision requirements. Violations of this nature directly impact the safety of the children while in your care. In addition, these types of violations have the greatest negative impact on your compliance history. During the visit, we discussed that repeated violations of this nature during consecutive visits may lead to an administrative action against your license. A follow-up visit may be conducted in the near future to monitor compliance with child care requirements. Additional Comments: The new QRIS has been approved by the Child Care Commission. The Division is working on implementation and trainings for the new system. We discussed the QRIS Modernization. Children throughout the center were engaged in free play in centers, group time, and personal care routines. In space #3, a staff member read the children the book If You Give a Mouse a Cookie. School aged children took turns reading a book aloud with a staff member. The outdoor space was observed to have a merry go round, climbing dome, basketball goal, riding toys, and balls for the children to engage in gross motor activities. Lunch was observed and consisted of beef meatballs, rice, corn, peaches, and milk. The following violation(s) were documented. Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. The landscape fabric beneath the mulch under the merry go round and the climbing dome was exposed causing a tripping hazard. The mulch beneath the stationary climbing dome was one inch in depth rather than the required six (6) inches. Background screenings are needed to provide a safe program environment, support children’s health and safety, and are important for preventing child maltreatment in early care and education programs. A CBC must be completed prior to employee hire and every five years thereafter. An expiration date is now listed on each Qualification Letter. Submissions can also be made six months before the expiration date listed on the current Qualification Letter. R. Cahoon, qualification letter expired on February 3, 2025, and the five-year renewal qualification letter was issued on March 4, 2025. You stated that it was the first time going through the new background check process with the portal and the fingerprints were not sent as child care prints to the SBI. We discussed using you staff and training worksheet as a tool and to add expiration dates in a digital calendar that will send reminders. Regularly reviewing your files will help keep you informed about upcoming expiring dates. Medication/Child Files: Medicines can be crucial to the health and wellness of children. They can also be very dangerous if the wrong type or wrong amount is given to the wrong person or at the wrong time. A medical action plan is required for children that need emergency medication, the form needs to be completed by the parent or a health care professional. In space #3, a child enrolled did not have a medical action plan on file for asthma. A child enrolled in space #3 had a medical action plan on file for asthma that was dated 3/30/23. We discussed the importance of understanding the symptoms the child shows and the instructions to administer the inhalers that are on site for the child. You stated that you were unaware that the medical action plans had to be updated annually and that you had requested a medical action plan for the other child but had not received it. In space #3, an albuterol inhaler was on file without the prescription labeled box. You stated that you have requested that the parent bring in the box. Two (2) out of the four (4) children’s files reviewed did not have a medical report on file. You stated that you have request medical reports within 15 days of enrollment. When a child enrolls, review the children’s file checklist to ensure that all documents are received and review the medication to ensure that it is in the prescription labeled box. CPR/First Aid: Knowing what to do in an emergency medical situation can mean the difference between life or death. Being trained in CPR and first aid can be invaluable when someone is in serious medical distress. Therefore, it is so important that all staff working directly with children have current CPR and First Aid Certification. J. Moore had certification of CPR and First Aid that expired on 2/24 and a renewal certificate that dated 1/29/25. You stated that the staff member was out on medical leave around the time that the certification expired, and you realized that it was needed when you were completing the staff and training worksheet. We discussed that if a staff member is out for 6 months or longer, they should be considered a new hire and receive all required documents. Ensure that staff member have all required certifications on file when they return from being on leave. Outdoor Environment: The outdoor learning environment offers a sense of freedom for children. Children are able to play freely with peers, expand their imagination beyond restraints of indoor activities, release energy and explore their sense of touch, smell, taste and their sense of motion. It is important that the outdoor area is checked daily and proper upkeep is conducted to ensure the environment is safe for use. The landscape fabric beneath the mulch under the merry go round and the climbing dome was exposed causing a tripping hazard. The mulch beneath the stationary climbing dome was one inch in depth rather than the required six (6) inches. The stationary climbing dome had exposed rust on much of the climber. You stated that you paint the climber and add mulch every spring and have the materials. I observed large bags of mulch. Check the outdoor spaces daily before the children arrive to ensure that hazards are removed and inspect equipment thoroughly each month during the playground inspections for hazards. Sleep charts for 11/6/2023-11/9/2023 were not available for review for one child enrolled in classroom space #1. Check sleep charts weekly to ensure they are completed daily for infants in care. Electrical Outlets-Approximately 2,400 children are injured annually by inserting objects into the slots of electrical outlets. All electrical outlets not in use that are in areas accessible to children must be protected with an outlet cover. This includes unused outlets on power strips. In space two(2) an outlet located in the library area did not have a safety cover. Daily safety checks in your classrooms should be done to ensure that safety covers are replaced.
