Your program currently operates with a five-star license, issued August 2, 2025, earning 7 points in the education component, 5 points in the program standards component meeting enhanced space and enhanced ratios and 1 quality point offering a staff benefits package and an infrastructure of parent involvement. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. The sanitation inspection was completed on July 21, 2025, with a “Superior” classification. The last fire inspection was conducted on June 3, 2025, and your facility was approved for daytime care only. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-one percent as of January 12, 2026. The NC Secretary of State website was reviewed on January 12, 2026, and Seeds of Wonder 2 LLC was listed as current- active. You visited each indoor and outdoor space with me. During today’s visit, I observed children in the indoor learning environments. Children were participating in general personal care routines and free play activities. Teachers were seen walking around the room engaging with children during play. I found supervision, ratios, and interactions to be in compliance. The following were also inspected during today’s visit: program requirements, equipment, furnishings, staff/child ratios, supervision, medication, medication administration, outdoor learning environments, and program records including staff records and children records. The following was cited during today’s inspection. Violation Number Comment Rule 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. Teachers YM and JM did not have certification in First Aid on file. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. Teachers YM and JM did not certification in CPR training on file. .1102(d) 1874 The Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy was not reviewed with new staff prior to providing care with children and/or a signed acknowledgement with all the required information was not maintained in the staff person's file. Teacher EM did not have acknowledgement of the Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy on file. .0608(d)(1-4) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. Teachers JM, YM and AI did not have verification of completing Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. .1102(g) Child Care programs are expected to always achieve and maintain compliance and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before January 26, 2026, I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected. Please be aware any information submitted by you is legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. This program currently operates with a temporary license effective February 1, 2025, to August 1, 2025. Upon my arrival you, Ingrid Laprea, Administrator, was not present. You arrived at 10am. A walkthrough of the approved indoor and outdoor spaces for child care was conducted. During the visit, children were observed engaging with their assigned staff during free play and routine activities. Handwashing practices were appropriately implemented and followed; I observed a teacher and child wash their hands after diaper changing. Interactions between staff and children were positive and developmentally appropriate. Children were adequately supervised at all times throughout the observation period. Classroom Six (6) was temporarily closed due to a staff shortage. Seven (7) children enrolled in Classroom Six (6) were combined with Classroom Five (5). Staff/child ratios were maintained in accordance with regulatory requirements. In addition to classroom and playground observations, the following components were monitored: Staff/child ratios Current CPR/FA certification Proper storage of hazardous products Availability of adequate and approved space Posted child care license and any applicable permit restrictions All center program records were reviewed. It is the center’s responsibility to ensure that all files are complete, accurate, and maintained in accordance with regulatory requirements. According to the menu for lunch the children will be served tacos with ground turkey, sweet peas, corn, and milk. Overall attendance and enrollment were collected today. The attached space, capacity and enrollment worksheet reflects the enrollment and attendance information along with the staff that were present and caring for children today. At this time, I am unable to process your Star Rated License application until I receive your ERS assessment scores. Once I receive your assessment scores, I will be able to proceed with processing your Rated License. Please be reminded that your Temporary License is set to expire on August 1, 2025. There are no extensions granted for Temporary Licenses. Therefore, if I do not receive your assessment scores prior to the expiration date, I will have to process your rated license based on the information available at that time. In order to maintain subsidy payments at the end of the temporary time period, the facility must transition to a three to five star rated license. Technical Assistance: During the visit I conducted on May 15, 2025, I provided technical assistance on how to access the ABCMS Portal. According to you, you have tried to access the portal on several occasions but you get an error message. This was not cited as a violation during today's visit because you were not able to access ABCMS portal due to technical issues with the website. This must be corrected immediately. You have until July 8, 2025, to notify me that it has been completed. If it is not corrected by this date a violation could be cited. The following technical assistance was provided: I discussed with you that a certificate will be issued upon successful completion of the test. During my visit you were in compliance with SIDS training for staff working in the classroom for infants; however, one (1) of the two (2) staff present in the classroom had not completed SIDS training. They started working in the classroom on June 23, 2025 so, they have two (2) months from that date to complete SIDS training. I discussed with you that the staff could not be left alone with the infants at anytime. This includes lunch and bathroom breaks. At least one (1) staff that has completed SIDS training must be in the infant classroom at all times. This program currently operates with a temporary license effective February 1, 2025 to August 1, 2025. You, Mariell Ramirez, Owner was present. Ingrid Laprea, Administrator/Owner was not present because she was on leave. I conducted a walkthrough of the indoor and outdoor spaces approved for child care. The children observed today were interacting with assigned staff while they were playing during free play activities and eating morning snack. Children and parents were observed washing their hands as they entered the classroom. Staff and children were observed washing their hands before and after eating. Appropriate interactions were observed between the children and staff. Children were adequately supervised. For morning snack, the children were served a bagel with cream cheese, and milk. During the visit five (5) children’s files were randomly selected and reviewed. Three (3) new staff files were reviewed. All center program records were also monitored. It is the center’s responsibility to review all files for completeness. During the visit I received your building inspection dated May 8, 2025. You will also need to submit your fire inspection and your zoning approval to me no later than July 7, 2025, to ensure that all required information has been submitted by August 1, 2025. Keep in mind that your temporary license cannot be extended. Overall attendance and enrollment were collected today. According to you, you have contacted the Chatham County Partnership to provide TA to help you prepare for the ERS assessment. You can also visit: www.ncrlap.org for additional information and resources. Technical Assistance: One (1) violation was cited in regard to storage of hazardous cleaning supplies. Hazardous Items Storage for Child Care was reviewed with you, and you were also given a copy. I recommend that you also post it where it is easily seen by all staff. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This program currently operates with a temporary license effective February 1, 2025 to August 1, 2025, because of a Change of Ownership. During the visit, a modified rules review was conducted with you, Mariell Ramirez, Administrator using The Pre-Licensing Checklist for Child Care Centers. You were also given a copy. Upon my arrival you were not present so, Gabriela Chapellin, Administrative Staff accompanied me as I conducted a walkthrough of the indoor and outdoor spaces used for child care. During the walk through I observed children interacting with assigned staff during free play activities, eating morning snack, and napping. Children were adequately supervised. For morning snack children were served pancakes with syrup and milk. Eight (8) staff files (including the administrator’s file) and three (3) children’s files were monitored during the visit. In addition, the required center program records were also monitored. It is the center’s responsibility to review all files to ensure they are complete and accurate. The building is constructed for child care. Six (6) classrooms were approved for child care use. Classroom one (1) has two (2) handwash sinks and one (1) bathroom with one (1) toilet. Classroom two (2) has two (2) handwash sinks and one (1) bathroom with one (1) toilet. Classroom three (3) has two (2) handwash sinks. Classroom four (4) has two (2) handwash sinks. I measured the above spaces less than a year ago (July 25, 2024) and there have been no structural changes since then so, I didn’t remeasure them during the visit. During the visit, a modified rules review was conducted with you, Mariell Ramirez, Administrator using The Pre-Licensing Checklist for Child Care Centers. You were also given a copy. Upon my arrival you were not present so, Gabriela Chapellin, Administrative Staff accompanied me as I conducted a walkthrough of the indoor and outdoor spaces used for child care. During the walk through I observed children interacting with assigned staff during free play activities, eating morning snack, and napping. Children were adequately supervised. For morning snack children were served pancakes with syrup and milk. Eight (8) staff files (including the administrator’s file) and three (3) children’s files were monitored during the visit. In addition, the required center program records were also monitored. It is the center’s responsibility to review all files to ensure they are complete and accurate.
