Your program currently operates with a three-star license, issued November 7, 2019, earning 6 points in the education component, 1 point in the program standards component and 1 quality point for limiting infant capacity to no more than two children under one year of age. The last annual compliance visit was conducted June 5, 2025. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-nine percent as of June 1, 2026. You visited each indoor and outdoor space with me. During today’s visit, I observed children in the indoor learning environments. Children were participating in general personal care routines and free play activities. The operator was seen engaging with children during play. I found supervision, capacity, and interactions to be in compliance. The following were also inspected during today’s visit: program requirements, equipment, furnishings, capacities, supervision, medication, medication administration, outdoor learning environments, and program records including staff records and children records. The following was cited during today’s inspection. Violation Number Comment Rule 910 Copy of each child's health assessment, including the operator's own preschool child(ren), was not on file within 30 days of enrollment. One child did not have a copy of the health assessment on file within 30 day of enrollment. GS 110-91(1); 10A NCAC 09.1721(a)(1) Child Care programs are expected to always achieve and maintain compliance and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before June 15, 2026, I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected. Your program currently operates with a three-star license, issued November 17, 2019, earning 6 points in the education component, 1 point in the program standards component meeting minimum requirements and 1 quality point for limiting infant capacity to no more than two children under one year of age. The last annual compliance visit was conducted July 19, 2022. The program’s compliance history was ninety-seven percent as of July 13, 2023. Children enrolled were participating in free choice activities. The operator was talking and engaging with the children in care. Fire drills were conducted each month and shelter in place and lock down drills were conducted quarterly. All program documentation was reviewed. The following items were out of compliance. The well water test was last completed October 9, 2020. This test must be done every two years. The operator’s health and safety training was last completed April 1, 2017. All health and safety trainings must be completed every five years. Today's lunch consists of hotdogs, beans, applesauce, bread and milk. The following violations were cited during today’s visit. Violation Number Comment Rule 801 Bacteriological analysis indicating the water is free of bacteria had not been performed on a sample of water from each well used for the children's water supply every two years. Results were not kept on file in the home. Your program currently operates with a three-star license, issued November 17, 2019, earning 6 points in the education component, 1 point in the program standards component meeting minimum requirements and 1 quality point for limiting infant capacity to no more than two children under one year of age. The last annual compliance visit was conducted July 19, 2022. The program’s compliance history was ninety-seven percent as of July 13, 2023. Children enrolled were participating in free choice activities. The operator was talking and engaging with the children in care. Fire drills were conducted each month and shelter in place and lock down drills were conducted quarterly. All program documentation was reviewed. The following items were out of compliance. The well water test was last completed October 9, 2020. This test must be done every two years. The operator’s health and safety training was last completed April 1, 2017. All health and safety trainings must be completed every five years. Today's lunch consists of hotdogs, beans, applesauce, bread and milk. The following violations were cited during today’s visit. Violation Number Comment Rule 801 Bacteriological analysis indicating the water is free of bacteria had not been performed on a sample of water from each well used for the children's water supply every two years. Results were not kept on file in the home. The last well water bacteriological analysis was last conducted October 9, 2023. 10A NCAC 09 .1725(a)(1) 2023 Operator and/or staff who work with children, did not complete health and safety training as part of on-going training so that every five years, all the topic areas were covered. Health and safety training was last completed April 1, 2017. .1703(d)(2) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before July 27, 2023, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware any information submitted by you is legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted.
