Your program currently operates with a five-star license, issued November 5, 2019, earning 7 points in the education component, 7 points in the program standards component meeting enhanced requirements and reduced ratios and 1 quality point for providing an infrastructure of parent involvement and offering a staff benefits package. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. The last annual compliance visit was conducted January 18, 2024. The sanitation inspection was completed May 30, 2024, with a “Superior” classification. The last fire inspection was conducted November 14, 2024, and your facility was approved for daytime care only. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety six percent as of December 5, 2024. Children in Space 1 were participating in a planned holiday craft making large paper gingerbread people. Teachers were engaged and walking around the room. I found supervision, interactions and ratios to be in compliance. Children in Space 2 were participating in center activities. Activities included, science, library, blocks, music, crafts, etc. Teachers were walking throughout the room and meeting the needs of children. During today’s inspection I inspected the following: indoor learning environment, outdoor learning environment, supervision, ratios, interactions, children records, staff records, emergency drills, medication administration, and all child care requirements. Lunch consisted of cheeseburger, potato roasters, peach cup, and milk. The following was cited during today’s inspection. Violation Number Comment Rule 844 Prescribed medicine was not in original labeled container or accompanied by signed and dated written instructions from prescribing physician or health care professional. A child's medication was not in the original labeled container or accompanied by a signed and dated letter from the health care professional. .0803(2)(a) 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. A child's medication expired as of November 30, 2024 and was not returned to the parent within 72 hours. .0803(12) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before December 19, 2024, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware any information submitted by you is legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted. We discussed returning the medication to the parents at pick up and not accepting the new medication until it has the original labeled box. As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS in an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. The compliance of this rule will be monitored during your next visit. Please note, the hard copy of the Change of Information form will no longer be needed or accepted. Lemondre Watson, administrator, accompanied me during a walk-through of the facility. During the visit, I discussed the allegations with Mr. Watson and two additional staff members. Limited monitoring of child care requirements occurred during today’s visit. The following violations were observed and/or confirmed during today’s visit. Violation Number Comment Rule 853 Incident logs were not completed and maintained as required. A staff member failed to log an incident report on the correct form with all fields appropriately completed as required by the rule. .0802(g)(1-6) 902 Each child was not attended to in a nurturing and appropriate manner, or in keeping with the child's developmental needs. A staff member held a four-year-old child's arms to manage their behavior and move them to a different area which resulted in marks left on the child’s left arm. G.S. 110-91(10) Violations must be corrected immediately. Within one week, 5/27/24, you must submit documentation of the corrections you made and your plan to maintain compliance with the identified child care requirements to me at tiffany.d.moore@dhhs.nc.gov.
