The visit was conducted with Wendi Bennett, Administrator/Owner Representative. The program currently operates with a Four-Star License issued on June 9, 2021. The last annual compliance visit was conducted on August 7, 2024. The last sanitation inspection was completed June 2, 2025, with a “Superior” classification and four demerits. The last fire inspection was conducted on July 25, 2025, and the facility was approved for daytime care only. The center's compliance history was reviewed with the operator. The program’s compliance history was 96% as of today. The NC Secretary of State website was reviewed today and the owner, Child Enrichment Centers LLC, was listed as current- active. The Clean Water for Us Kids shows that lead water testing was completed for the facility on 12/18/2023. The website also shows that lead-based paint and asbestos testing is completed with no actions required. In order to remain in compliance with child care rule and sanitation rules, lead water testing must be completed every three years. A review of the DCDEE ABCMS staff roster for the facility was reviewed today and there are 38 staff members listed. All indoor and outdoor spaces approved for child care were monitored during today’s visit. Staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Children in care were observed to be engaged in free choice activities, personal care routines, leaving for a field trip, outdoor play and lunch. Lunch was observed and consisted of fish sticks, corn, peaches and milk. Children under twelve months old were observed to receive care according to individual needs including diapering and feeding. Program records including fire drills, emergency drills, and playground inspection, required postings, nutrition, storage of hazardous items and storage of and administration of medication was monitored today. Staff and children’s files were monitored by DCDEE procedures. The facility conducts field trips for school-age and older preschool children during the summer months and provides school pick-up and transportation to the center during the school year. The vehicles used to transport children are two (2) 2015 Ford Transits and one 2024 Ford Transit Van were monitored today with no violations documented. The school age children were on-site today upon arrival and left for a field trip to Bean Shoals Park. The schedule for field trips was posted for the month of July 2025. Permission slips for transportation to each field trip was on file, attendance with name to face recognition is used during the field trips and an attendance of who is on the trip remains at the center. Each vehicle contained binders with children’s emergency and identifying information including a photograph, and an attendance sheets for pick-up/leaving for destination, arriving at destination, and arrival back to the center. The following violation(s) were documented: Violation Number Comment Rule 721 All equipment and furnishings were not in good repair. A fabric covered chair in Space 6B2 was stained. G.S. 110-91(6); .0601(b) 9995 A violation was found for which there is no item number. A designated emergency medication for an enrolled child was taken with the child on a field trip, but staff did not return the medication to the classroom and properly store the medication, a violation of Sanitation Rule 15 NCAC 18A .2820(d)Medications including prescription and non-prescription items shall be stored in a locked cabinet or other locked container and shall not be stored above food. Designated emergency medications shall be kept inaccessible to children, but are not required to be in locked storage. Non-prescription diaper creams and sunscreen shall be kept inaccessible to children when not in use, but are not required to be in locked storage. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before August 13, 2025, I must receive compliance documentation that describes accurately and in detail how and when the violations were corrected. The documentation must also include the facility ID number, date the documentation is being submitted, title of the person signing the documentation, and each violation item number. If sufficient information is not received by the due date, a follow-up visit will be conducted. Email the information to tammy.childress@dhhs.nc.gov. Technical Assistance provided during today’s visit in response to violations documented: Ask staff to clean cushions of furniture as soon as they observe them to be dirty. You may consider creating a checklist for staff who open the classrooms in the morning and include such items as is equipment/furnishings clean and in good repair, AND, are emergency medications on-site. Consultation Provided During Today’s Visit: You have received email communications from the Division of Child Development and Early Education regarding Quality Rating Improvement System (QRIS) Modernization. To earn a Two to Five Star License there are now three pathways you may choose from. The Division is working on a gradual transition for this new process, and you will be provided with more detailed information, training and resources in the upcoming months. I encourage you now to review the QRIS Modernization information that is available on the DCDEE website at https://ncchildcare.ncdhhs.gov, by selecting “QRIS Modernization” from the options below the “What’s New” tab on the home page. If you have not already, I also encourage you to view the information for the new Environment Rating Scale Assessments, Third Edition, on the same page on the DCDEE website for QRIS Modernization, and you can also visit the North Carolina Rated License Assessment website at ncrlap.org. This visit was conducted by Audrey Davis and Emily Lamquaye, Childcare Consultants with Wendi Bennett, center owner/director. Your program currently operates with a four-star license. Restrictions include 1st shift care, meets enhanced ratios, children < age 2 in rooms with direct exits, and capacity for building 1 is 66 and building 2 118. The license was observed, and the restrictions were found in compliance. The NC Secretary of State website was reviewed on 8/6/24 and Child Enrichment Centers, LLC. was listed as current and active. The program’s compliance history was 94% prior to today’s visit. All programs are required to maintain at least 75% compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. We observed positive interactions today as children engaged in indoor and outdoor play, lunch, and rest. There were developmentally appropriate materials and activities available to the children in the classrooms and outside. Lunch included pizza crunchers, pears, green peas, and milk. The last annual compliance visit was conducted on 8/24/23. A sanitation inspection was conducted 6/12/24 with four (4 ) demerits and a Superior classification. An approved, satisfactory fire inspection was conducted 7/2/24. You remain approved for day time care. You provided us with a copy of the fire inspection during this visit. Program records and required postings were monitored. A fire drill was conducted 7/3/24 and a lockdown emergency drill was documented on 7/5/24. An outdoor inspection log was maintained with the last inspection documented on 7/8/24. Staff files and children’s records were monitored per DCDEE procedures. Storage of hazardous items was monitored today and found in compliance. Cleaning supplies are stored in a locked hallway closet and in the kitchen which is kept locked. Medication: Topical medications were stored properly with authorizations as required. The following violations were documented today and reviewed with you as follows: Violation Number Comment Rule 1320 Children's records that include an application for enrollment, medical and immunization records, and permission to seek emergency medical care was not on file for each child. I observed developmentally appropriate materials and activities were available to the children in the classrooms and outside. The last annual compliance visit was conducted on 9/27/22. A sanitation inspection was completed 3/7/23 with six (6) demerits and a Superior classification. An approved, satisfactory fire inspection was conducted 8/7/23 by fire marshal David A. Jones. You remain approved for day time care. Program records and required postings were monitored. A fire drill was conducted on 7/18/23 and a lockdown emergency drill was documented on 7/17/23. An outdoor inspection log was maintained with the last inspection documented on 7/17/23. Staff files and children’s records were monitored per DCDEE procedures. Storage of hazardous items was monitored today and found in compliance. Cleaning supplies are stored in a locked hallway closet and in the kitchen which is kept locked. I observed diaper creams properly stored with authorizations that contained the required information. Transportation: You transport school age children from four area schools to your school age program in the afternoons. You also transport school age children in the summer for off-premises activities. You have three 15-passenger vans which I monitored during today’s visit. The following violations were documented today and reviewed with you as follows: Violation Number Comment Rule 612 Each child's bed, cot, or 2" mat was not individually assigned and identified. Each infant's sleep space was not labeled with the child's name. 15A NCAC 18A .2821(b) & (c) 1115 First aid kit and/or fire extinguisher was not located in vehicle; kit and/or extinguisher was not mounted or secured if in passenger area. A first aid kit was not located in one of three vehicles used for routine transport of school age children to the center from school in the afternoons. 10A NCAC 09 .1003(c) 1125 Before children were transported, written permission from a parent was not obtained that included when and where the child was to be transported, expected time of departure and arrival, and the transportation provider. Parent's permission to tranport children to the center from school did not include when and where the child was to be transported. .1003(i)(j) Child Care programs are expected to maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. Technical assistance provided during today’s visit: • You must notify your consultant 30 days prior to changing the ownership status of your facility such as deciding to become incorporated or dissolve your corporation. • Please maintain documentation to show how school age children participate in the planning and implementation of their activities. * During today's visit, we discussed that there is not enough space in the room for infants for cribs for the nine children enrolled. You discussed today your plan to remove items in the room that are not necessary to create more space. There are five cribs and you have sleep mats if needed for the four oldest infants, ages 8 months, 7 months, and 6 months. Ms. Hauser is researching best practice for your situation. We are available to assist you in working this out. Cohort 1 – Your facility is in Cohort 1 to Resume the Rated License following the Hold Harmless during the COVID pandemic. This means you are currently in your Preparation Year through June 30, 2024. During this preparation year, make sure all staff have up-to-date WORKS accounts in order to receive credit for all of their college coursework when the education component is assessed. Also, you may have new staff who have never participated in the North Carolina Environment Rating Scales – ITERS-R, ECERS-R, and SACERS-U. I recommend that you take advantage of all available resources to help acquaint staff with the Scales and refresh those staff with previous experience. Some resources include the ncrlap.org, the local Child Care Resource Center, Smart Start, and DCDEE. You can also access a Quality Point form on the DCDEE website – ncchildcare.dhhs.nc.gov. Select the Provider tab, then Provider Documents, and scroll down to Quality Point Form to see a list of options you may choose to earn a quality point. You said you participated in the NCRLAP assessment project's mock assessments during the summer. You said you are using the results to help prepare for your upcoming assessment. Keep in mind facilities have the option to request the ERS during your preparation year. You said you do plan to have the assessment as part of your upcoming rated license and will let me know if you decide to get assessed early. This visit was conducted by Audrey Davis, Childcare Consultant with Wendi Bennett, center owner/director. Your program currently operates with a four-star license. Restrictions include 1st shift care, meets enhanced ratios, children < 2 ½ in rooms with direct exits, and capacity for building 1 is 66 and building 2 118. The license was observed, and the restrictions were found in compliance. The NC Secretary of State website was reviewed on 8/22/23 and Child Enrichment Centers, LLC. was listed as current and active. The program’s compliance history was 98% prior to today’s visit. All programs are required to maintain at least 75% compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed positive interactions today. I observed children having indoor and outdoor play, lunch, and rest. I observed developmentally appropriate materials and activities were available to the children in the classrooms and outside. The last annual compliance visit was conducted on 9/27/22. A sanitation inspection was completed 3/7/23 with six (6) demerits and a Superior classification. An approved, satisfactory fire inspection was conducted 8/7/23 by fire marshal David A. Jones. You remain approved for day time care. Program records and required postings were monitored. A fire drill was conducted on 7/18/23 and a lockdown emergency drill was documented on 7/17/23. An outdoor inspection log was maintained with the last inspection documented on 7/17/23. Staff files and children’s records were monitored per DCDEE procedures. Storage of hazardous items was monitored today and found in compliance. Cleaning supplies are stored in a locked hallway closet and in the kitchen which is kept locked. I observed diaper creams properly stored with authorizations that contained the required information. Transportation: You transport school age children from four area schools to your school age program in the afternoons. You also transport school age children in the summer for off-premises activities. You have three 15-passenger vans which I monitored during today’s visit. The following violations were documented today and reviewed with you as follows: Violation Number Comment Rule 612 Each child's bed, cot, or 2" mat was not individually assigned and identified. Each infant's sleep space was not labeled with the child's name. 15A NCAC 18A .2821(b) & (c) 1115 First aid kit and/or fire extinguisher was not located in vehicle; kit and/or extinguisher was not mounted or secured if in passenger area. A first aid kit was not located in one of three vehicles used for routine transport of school age children to the center from school in the afternoons. 10A NCAC 09 .1003(c) 1125 Before children were transported, written permission from a parent was not obtained that included when and where the child was to be transported, expected time of departure and arrival, and the transportation provider. Parent's permission to tranport children to the center from school did not include when and where the child was to be transported. .1003(i)(j) Child Care programs are expected to maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. You must correct the violations cited during today's visit immediately and I must receive written documentation from you verifying compliance on or before September 7, 2023. The following information must be included in your signed and dated compliance letter: • the name of your facility • facility ID number • date • title of the person who signs the letter • each violation number(s) • an accurate and detailed description of how and when you corrected each violation You may email the letter to Audrey.Davis@dhhs.nc.gov or mail two copies of the letter to: Audrey Davis PO Box 17273 Winston-Salem, NC 27116 If you state in your letter that corrections or changes have been made when they have not yet been made, it will be considered falsification of information and could lead to administrative action taken by the DCDEE. Repeated violations, continued non-compliance, or failure to submit the compliance letter on or prior to the date given in this visit summary can also lead to administrative action taken by the DCDEE. If there are violations that cannot be corrected by the due date due to circumstances beyond your control, you must submit a written request for an extension prior to the due date. In this case, I must still receive your letter for all other item numbers by the original due date. Additional monitoring visits may be conducted during the year to monitor compliance with applicable childcare requirements. Technical assistance provided during today’s visit: • You must notify your consultant 30 days prior to changing the ownership status of your facility such as deciding to become incorporated or dissolve your corporation. • Please maintain documentation to show how school age children participate in the planning and implementation of their activities. * During today's visit, we discussed that there is not enough space in the room for infants for cribs for the nine children enrolled. You discussed today your plan to remove items in the room that are not necessary to create more space. There are five cribs and you have sleep mats if needed for the four oldest infants, ages 8 months, 7 months, and 6 months. Ms. Hauser is researching best practice for your situation. We are available to assist you in working this out. Cohort 1 – Your facility is in Cohort 1 to Resume the Rated License following the Hold Harmless during the COVID pandemic. This means you are currently in your Preparation Year through June 30, 2024. During this preparation year, make sure all staff have up-to-date WORKS accounts in order to receive credit for all of their college coursework when the education component is assessed. Also, you may have new staff who have never participated in the North Carolina Environment Rating Scales – ITERS-R, ECERS-R, and SACERS-U. I recommend that you take advantage of all available resources to help acquaint staff with the Scales and refresh those staff with previous experience. Some resources include the ncrlap.org, the local Child Care Resource Center, Smart Start, and DCDEE. You can also access a Quality Point form on the DCDEE website – ncchildcare.dhhs.nc.gov. Select the Provider tab, then Provider Documents, and scroll down to Quality Point Form to see a list of options you may choose to earn a quality point. You said you participated in the NCRLAP assessment project's mock assessments during the summer. You said you are using the results to help prepare for your upcoming assessment. Keep in mind facilities have the option to request the ERS during your preparation year. You said you do plan to have the assessment as part of your upcoming rated license and will let me know if you decide to get assessed early.
