Upon arrival, You, T. Holden, NC PreK Teacher, was present and available to assist with today’s visit. You notified R. Johnson, PreK Coordinator, of my visit. Your program currently operates with a five-star license issued on May 8, 2019, earning 7 points in the education component, 7 points in the program standards component and 1 quality point for having a staff benefits package and having an infrastructure of parent involvement. Your program was also monitored for compliance with implementing an approved curriculum, the curriculum you are implementing is Creative Curriculum 6th edition as required for all four- and five-star licensed facilities where four-year-old children are enrolled. The last annual compliance visit was conducted on December 4, 2024. The last sanitation inspection was completed on June 2, 2025, with a “Superior” classification “6” demerits. The last fire inspection was conducted on August 28, 2025, and your facility was approved for daytime care only. The license was posted, and the restrictions were in compliance. Thirty-four (34) children from ages four (4) to five (5) were present during the visit. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in the indoor learning environments and found supervision and staff/child ratios to be in compliance. When I arrived, the children were participating in group time and music and movement activities. The caregivers were interacting and meeting the developmental needs for each of the children. Children were observed eating the following for breakfast: chicken, biscuit, 100% fruit juice, and milk. The NC Pre-K requirements in section .3000 of the child care rules were monitored for compliance. The NC Pre-K Program Site Monitoring Tool was not reviewed today. The monitoring tool was off-site with the Down East Partnership for children. The monitoring tool will be reviewed during the next visit. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified in compliance. The current NC Pre-K staff are teacher M. Walston and assistant teacher, J. Bridgers in space #704, teacher K. Higgs and assistant teacher S. Hewlin in space #703, teacher T. Holden and assistant teacher T. Albritton in space #701. M. Williams, substitute teacher, was in space #701 for the day. A selection of children’s files were monitored for completed health assessments and developmental screenings. You stated you use the Ages and Stages for your developmental screenings. The center uses the Teaching Strategies Gold instrument to document evidence of children's ongoing progress. The checkpoint assessments are conducted three times per year: Fall, Winter, Spring, Summer. The first day of school was September 2, 2025. The classroom operates from 7:30 am-3:15pm. Parent conferences are held at each of the checkpoints and when needed throughout the year. You stated that you use the Class DoJo and Thrill Share app to communicate with parents. Any violations of the child care requirements observed today were discussed with you and documented in the visit summary report and reviewed with you at the conclusion of the visit. You have completed the Moodle training and have created the portal for your facility; however, your staff are not connected to the portal. G.S. 110-90.2 & .2703(r) Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before December 4, 2025, I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected. Upon arrival, T. Holden, Lead Teacher, was present and available to assist with today’s visit. You notified R. Johnson, Pre-K Coordinator, of my visit. Your program currently operates with a five-star license issued on May 8, 2019, earning 7 points in the education component, 7 points in the program standards component and 1 quality point for having a staff benefits package and having an infrastructure of parent involvement. The last annual compliance visit was conducted on December 4, 2024. The last sanitation inspection was completed on September 11, 2024, with a “Superior” classification “6” demerits. The last fire inspection was conducted on August 29, 2024, and your facility was approved for daytime care only. The license was posted, and the restrictions were in compliance. Forty-four (44) children from ages four (4) to five (5) years old were present during the visit. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in the indoor learning environment and found supervision and staff/child ratios to be in compliance. When I arrived, the children were participating in group-time, free play in centers and transitions. The caregivers were interacting and meeting the developmental needs for each of the children. The following were monitored: Supervision, Staff Child Ratio, First Aid, CPR, Special Training, CBC Qualification, Emergency Medical Care Plan, Storage of Hazardous Substances, Administration of Medication, Adequate/Approved Space, License Posted, Permit Restrictions, and Program Records. Any violations of the child care requirements observed today were discussed with you and documented in the visit summary report and reviewed with you at the conclusion of the visit. Children were observed eating the following for breakfast: pancakes, orange juice, and milk. The NC Pre-K requirements in section .3000 of the child care rules were monitored for compliance. The NC Pre-K Program Site Monitoring Tool was not reviewed today. The monitoring tool was off-site with the Down East Partnership for children. The monitoring tool will be reviewed during the next visit. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified in compliance. The current NC Pre-K staff are teacher M. Walston and assistant teacher, J. Bridgers in space #704, teacher K. Higgs and assistant teacher S. Hewlin in space #703, teacher T. Holden and assistant teacher T. Albritton in space #701. P. Battle substitute teacher was in space #703 for the day. A selection of children’s files were monitored for completed health assessments and developmental screenings. You stated you use the Ages and Stages for your developmental screenings. The center uses the Teaching Strategies Gold instrument to document evidence of children's ongoing progress. The checkpoint assessments are conducted three times per year: Fall, Winter, Spring, Summer. The first day of school was September 4, 2024. The classroom operates from 7:30 am-3:15pm. Parent conferences are held at each of the checkpoints and when needed throughout the year. You stated that you use the Class DoJo and Thrill Share app to communicate with parents. Any violations of the child care requirements observed today were discussed with you and documented in the visit summary report and reviewed with you at the conclusion of the visit. Upon arrival, Taya Walston, Lead Teacher, called you Rene Johnson, NC Preschool Coordinator to notify you of my visit. You arrived shortly after the visit began and assisted with today’s visit. Your program currently operates with a five-star license issued on May 8, 2019, earning 7 points in the education component, 7 points in the program standards component and 1 quality point for having a staff benefits package and having an infrastructure of parent involvement. Your program was also monitored for compliance with implementing an approved curriculum, the curriculum you are implementing is Creative Curriculum 6th edition as required for all four- and five-star licensed facilities where four-year-old children are enrolled. The last annual compliance visit was conducted on January 26, 2023. The last sanitation inspection was completed on September 29, 2023, with a “Superior” classification “4” demerits. The last fire inspection was conducted on August 28, 2023, and your facility was approved for daytime care only. The license was posted, and the restrictions were in compliance. Thirty-six (36) children from ages four (4) to five (5) were present during the visit. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in both the indoor learning environments and found supervision and staff/child ratios to be in compliance. When I arrived, the children were participating in free play in centers and transitions. The caregivers were interacting and meeting the developmental needs for each of the children. The NC Pre-K requirements in section .3000 of the child care rules were monitored for compliance. The NC Pre-K Program Site Monitoring Tool was not reviewed today. The monitoring tool was off-site with the Down East Partnership for children. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified in compliance. Your program was also monitored for compliance with implementing an approved curriculum, the curriculum you are implementing is Creative Curriculum 6th edition as required for all four- and five-star licensed facilities where four-year-old children are enrolled. The last annual compliance visit was conducted on January 26, 2023. The last sanitation inspection was completed on September 29, 2023, with a “Superior” classification “4” demerits. The last fire inspection was conducted on August 28, 2023, and your facility was approved for daytime care only. The license was posted, and the restrictions were in compliance. Thirty-six (36) children from ages four (4) to five (5) were present during the visit. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in both the indoor learning environments and found supervision and staff/child ratios to be in compliance. When I arrived, the children were participating in free play in centers and transitions. The caregivers were interacting and meeting the developmental needs for each of the children. The NC Pre-K requirements in section .3000 of the child care rules were monitored for compliance. The NC Pre-K Program Site Monitoring Tool was not reviewed today. The monitoring tool was off-site with the Down East Partnership for children. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified in compliance. EARLY CHILDHOOD EQUIVALENCY EXAM NOW ON DCDEE MOODLE: The North Carolina Early Childhood Equivalency Exam is a new pathway option comparable to a NC Early Childhood Credential. Applicants who achieve a score of 80% or higher on the NC Early Childhood Equivalency Exam earn the new North Carolina Early Childhood Equivalency Certificate. This certificate is equivalent to a North Carolina Early Childhood Credential and will qualify the individual to be a lead teacher in an early childhood classroom. Upon arrival, Taya Walston, Lead Teacher, called you Rene Johnson, NC Preschool Coordinator to notify you of my visit. You arrived shortly after the visit began and assisted with today’s visit. Your program currently operates with a five-star license issued on May 8, 2019, earning 7 points in the education component, 7 points in the program standards component and 1 quality point for having a staff benefits package and having an infrastructure of parent involvement. Your program was also monitored for compliance with implementing an approved curriculum, the curriculum you are implementing is Creative Curriculum 6th edition as required for all four- and five-star licensed facilities where four-year-old children are enrolled. The last annual compliance visit was conducted on January 26, 2023. The last sanitation inspection was completed on September 29, 2023, with a “Superior” classification “4” demerits. The last fire inspection was conducted on August 28, 2023, and your facility was approved for daytime care only. The license was posted, and the restrictions were in compliance. Thirty-six (36) children from ages four (4) to five (5) were present during the visit. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in both the indoor learning environments and found supervision and staff/child ratios to be in compliance. When I arrived, the children were participating in free play in centers and transitions. The caregivers were interacting and meeting the developmental needs for each of the children. The NC Pre-K requirements in section .3000 of the child care rules were monitored for compliance. The NC Pre-K Program Site Monitoring Tool was not reviewed today. The monitoring tool was off-site with the Down East Partnership for children. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified in compliance. The current NC Pre-K staff are teacher, Taya Walston and assistant teacher, Julianne Bridgers in space #704, teacher Melannie Newcombe and assistant teacher Stella Worsley in space #703, teacher Taryl Holden and assistant teacher Sheena Hewlin in space #701. Patricia Davis, substitute teacher, was in space #701 for the day; Ms. Holden was out with medical issues. A selection of files was monitored for completed health assessments and developmental screenings. You stated you use the Ages and Stages for your developmental screenings. The center uses the Teaching Strategies Gold instrument to document evidence of children's ongoing progress. The checkpoint assessments are conducted three times per year: Fall, Winter, Spring, Summer. The first day of school was September 5, 2023. The classroom operates from 7:30 am-3:00 pm. Parent conferences are held at each of the checkpoints and when needed throughout the year. You stated that you use the Class DoJo app to communicate with parents. Any violations of the child care requirements observed today were discussed with you and documented in the visit summary report and reviewed with you at the conclusion of the visit. The following violations of the child care requirements were documented today: Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. In space #701 there was an outlet uncovered underneath the white board and to the right of the white board there was a power strip with three electrical outlets not covered. The teacher immediately covered the outlets with protective plugs. 10A NCAC 09 .0604(c) 842 A drug or medication was administered without written authorization and/or instructions from a child's parent or authorized health professional. In space #701 there was an Epi Pen and Benadryl that did not have a permission form available for review. 10A NCAC 09 .0803(1)(a & b) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment.
