The last annual compliance visit was conducted on July 24, 2025. Your program currently operates with a 2-star license, effective August 19, 2022, earning 1 point in Staff Education, 2 points in Program Standards and 1 point in Quality component for restricting infant capacity to no more than two children under one year of age. License restrictions include daytime care only, maximum of 5 preschool children at any time, serves no more than 2 infants under age 1 year, fireplace/woodstove not used during operating hours. Tessa Richarde, home operator, was present during today’s visit. I observed a one year old child napping during today's visit. I found interactions and supervision to be in compliance. During today’s visit I monitored the following: program space, program records, interactions, supervision, equipment and furnishings, medication administration, indoor and outdoor learning environments. You stated you do not provide transportation. Seven violations of child care requirements were observed and documented during today's visit. Each were thoroughly reviewed with you upon the conclusion of today's visit. This was corrected during today's visit when the operator completed the health questionnaire. .1703(a)(1) 1301 Operator did not complete the required number of on-going training hours as specified in rule. The operator completed three (3) hours of on-going training for the 2025-2026 training year. The operator is missing seven (7) hours to complete the training year. GS 110-91(11); 10A NCAC 09 .1705(b)(5) 1306 Operator has not completed ITS-SIDS training every three years from the completion date of the previous ITS-SIDS training. The last documented ITS-SIDS training was on 11-7-2020 making it expire in November 2023. .1703(a)(4) 2019 Operator did not complete Recognizing and Responding to Suspicions of Child Maltreatment training within the required time frame. Tessa Richarde, operator, assisted me with the visit. Your program currently operates with a two star license, issued August 23, 2022, earning one point in the education component, 2 points in the program standards component and one quality point. Restrictions include first shift, maximum of five preschoolers each shift, fireplace not used during operating hours, no more than 2 infants under one each shift. The last annual compliance visit was conducted August 10, 2022. The program’s compliance history was ninety five percent as of August 2, 2023. You visited each indoor and outdoor space with me. One child present was observed having freeplay, diaper change and drinking a bottle in the provider’s arms. There was one violation documented and discussed with the provider. There was one violation documented and discussed with Ms. Richarde. Violation Number Comment Rule 910 Copy of each child's health assessment, including the operator's own preschool child(ren), was not on file within 30 days of enrollment. Two children did not have the health assessment on file within 30 days of enrollment. GS 110-91(1); 10A NCAC 09.1721(a)(1) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before August 17, 2023, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware any information submitted by you is legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted.
