Renee Wicker, administrator, was on the premises and I reviewed the attached visit summary documentation with her at the end of today’s visit. This program currently operates with a five-star license, issued March 15, 2021. Permit restrictions are 1st and 2nd shift, meets enhanced ratios, and meets enhanced space. Staff/child ratio, group size, supervision, use of licensed space, space capacity, permit restrictions, and the indoor and outdoor environment were monitored. The permit and emergency care plan were posted. Fire drills, medication administration, and storage of hazardous products were also monitored. The Staff and Training Worksheet, Staff/Child Ratio Worksheet, children’s records worksheet, and the ACV checklist. The last annual compliance visit was conducted on May 14, 2026. The last sanitation inspection was conducted on December 31, 2025, earning a superior rating. The last fire inspection was conducted on October 20, 2025. The program’s compliance history was ninety-nine percent (99%) as of today. I visited each indoor and outdoor space with me. Children were observed during morning activities. All the children were being attended to in a nurturing and appropriate manner and in keeping with their developmental level. Lunch was fish nuggets, mac and cheese, tater tots, applesauce and milk. The following violations were observed during today’s visit: Violation Number Comment Rule 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. Nine staff documented on the Staff and Training Worksheet did not have current certification in First Aid. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. Nine staff documented on the Staff and Training Worksheet did not have current certification in CPR. .1102(d) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4)(d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. Please email me a letter describing how and when all nine staff achieved compliance with current CPR and First Aid Certifications by May 20, 2026. QRIS Discussion: Today we reviewed the new QRIS Pathways, and you plan on pursuing the Classroom and Instructional Pathway. We reviewed the child care center requirements applicable to pathway 2. We reviewed the educational requirements for a center. You plan on updating all staff in WORKS. Make sure that the staff have been assessed for the position they are working in. Please submit your application for your star rated license and an updated staff and training worksheet no later than September 15, 2026. An unannounced annual compliance visit with a rated license visit will be completed before your probationary license expires. The last annual compliance visit was conducted May 21, 2024. The sanitation inspection was completed December 23, 2024, with a “Superior” classification. The last fire inspection was conducted October 15, 2024 and your facility was approved for daytime and overnight care. The program’s compliance history was ninety nine percent as of May 13, 2025. The NC Secretary of State website was reviewed on May 13, 2025 and First Health of the Carolinas was listed as current- active. Children were observed playing outside, having story time, preparing for and eating lunch, having tummy time with staff, toileting and napping. Lunch was observed and consisted of cheese pizza, strawberries, broccoli, and milk. The following one violation(s) was documented - Violation Number Comment Rule 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). Name of Operation: FIRST HEALTH CHILD DEVELOPMENT CENTER Facility ID: 6355079 Consultant: DEBORAH DAVIS Operation Type: Center Case Number: 0424-337L Visit Date: 4/30/2024 Number Present: 114 Completed Date: 4/30/2024 Age: From 0 To 5 Total Minutes: 115 Time In: 10:20 AM Time Out: 12:15 PM Time In: Time Out: List to Use: Center Type Of Visit: Complaint Visit Announced/Unannounced: Unannounced The purpose of today's unannounced visit was to investigate allegations received April Reporter stated There is a concern that on two occasions in the past month, an infant was given the bottle of another child. I interviewed Renee Wicker, administrator, and Lisa Hickman, Assistant Administrator, concerning the allegations. Both stated the staff person immediately informed them that on January 16, 2024 and March 21, 2024 she gave a child the wrong bottle. The staff person received disciplinary action on both occasions. Staff training was provided to all infant and float staff by Crystal Roberts, Child Care Health Nurse, on April 23, 2024 on Safe Handling and Storage-Bottle Feeding. Mrs. Wicker provided me with an agenda, sign in sheet and certificates from the training. Mrs. Hickman stated she has observed the staff person on several occasions since the incidents and the staff person used the protocol "Right baby, right bottle" which staff repeat three times while checking name on the bottle and viewing baby's face. Mrs. Wicker stated Ms. Roberts will conduct a follow up visit in the future to monitor bottle feeding procedures. I also interviewed the staff person and she admitted to giving the baby the wrong bottle on two occasions. She stated she is making sure to follow protocol regarding bottle feeding. Based on information gathered this complaint is substantiated. One violation was documented and has been corrected. Violation Number Comment Rule 1887 Each infant was not served only bottles labeled with their individual name. A staff person gave a milk bottle to the wrong child on two separate occasions. .0902(d) Comments Section – Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent.
