    Name of Operation: PLAYMATES CHILD CARE AND DEVELOPMENT CENTER OF JOH Facility ID: 51000853 Consultant: PAULA DAVIS Operation Type: Center Case Number: Visit Date: 1/15/2025 Number Present: 193 Completed Date: 1/15/2025 Age: From 0 To 5 Total Minutes: 165 Time In: 09:45 AM Time Out: 12:30 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today’s visit was to monitor compliance during your annual compliance visit. A complaint visit was also conducted in conjunction with today’s visit. The administrator assisted me with today’s visit. Child Care Consultant, Leigh McKenzie-Lee accompanied me on today’s visit. I monitored the indoor and outdoor environments for compliance with child care requirements. Staff/child ratios, supervision, use of space and capacity were monitored for compliance. The permit, sanitation, evacuation, safe arrival and departure procedures, first aid chart, menus, activity plan, daily schedule, staff/child ratios, emergency medical care plan, emergency telephone numbers and a summary of NC Child Care Law were all posted at the time of the visit. The storage of medication and hazardous products was also monitored for compliance. No new staff have been hired since the 9-19-24 routine unannounced visit. A percentage of existing staff files were reviewed. A sample of children's records were reviewed, and the worksheet was completed. The last fire drill was documented as conducted on 1-10-25 at 9:15 am with an evacuation time of one minute and forty seven seconds. The last outdoor inspection was documented as conducted on 1-10-25. A lockdown drill was documented as conducted on 12-14-24 at 2:15 pm. An approved fire inspection was last completed on 8-30-24. You stated that the fire inspector came to the facility on 9-17-24; A sanitation inspection was last completed on 11-19-24 with three demerits and a superior classification. The corporation status of Playmates Child Care and Development Center of Johnston County, Inc. was verified as current and active during today's visit using the Secretary of State's website. During the visit, the children were observed during free play and teacher directed group activities. I observed lunch being plated. Lunch consisted of cheese pizza, corn, applesauce and milk. A three star child care center license with four points in education, two points in program standards and one quality point was issued to you on 10-23-18. Based on information provided and observations made during the visit you continue to meet the requirements previously met to obtain such a license. Violation Number Comment Rule 526 Menus for all meals and snacks were not current or posted where easily seen by parents and cook. The menu in space 120 was not current. 10A NCAC 09 .0901(b) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. Ice packs were not in locked storage in space 117 and 122. .2820(b) 841 Medications including prescription and non-prescription items were not stored in a locked cabinet or other locked container. Gas drops, Nystatin, and Mupirocin was not in locked storage in space 106. 15A NCAC 18A .2820(d) 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). A qualification letter had expired for one employee with a hire date of 10-28-19. G.S. 110-90.2(b) & .2703(n)&(o) 1757 A valid qualification letter was not on file and available to review at the facility. A valid qualification letter was not on file for one staff member with a hire date of 10-28-19. Therefore, it is important for you to be knowledgeable of all of the licensing requirements that apply to your type of program. The best way for you to make sure that you are meeting all requirements is to periodically review the child care law and rules.**** Please send me a written statement describing how and when the above violations were corrected. The written statement must be received no later than 1-29-25. Please note that if your letter is mailed, a hand written signature is required. The permit, sanitation, evacuation, safe arrival and departure procedures, first aid chart, menus, activity plans, daily schedules, staff/child ratios, emergency medical care plan, emergency telephone numbers and a summary of child care law were posted. No new staff members were hired since the 2-13-24 annual compliance visit. Therefore, no staff files were reviewed. The storage of medication and hazardous products was also monitored for compliance. The children were observed painting, coloring, drawing, participating in circle time, story time and playing in activity areas. A three star license with four points in education, two points in program and one quality point was issued to you on 10-23-18. Based on observations made and information provided, you continue to meet the requirements previously met to obtain such a license.
