Upon my arrival, I was greeted by L. Hare. A checklist was used to note the requirements I monitored today. A. Rowlands, Executive Director, arrived a short time later. I was accompanied by Ms. Rowlands as I completed a general walk-through of the indoor and outdoor areas, which consisted of four (4) classrooms, the kitchen, and the outdoor play environment. The children were observed during outdoor play and completing routine care tasks. Today’s meal was observed and in compliance with the Meal Patterns. **Current License Status Currently this center operates with a five-star rated license issued January 14, 2020. ** Annual Inspections: The last sanitation inspection was completed May 4, 2026, and received a ‘Superior’ classification with 0 demerits. The last fire inspection was completed on May 27, 2025. The last documented fire drill was conducted on May 8, 2026, the last documented lockdown drill was completed on March 4, 2026, and the last documented playground inspection was completed on May 8, 2026. Prior to today’s visit I reviewed the NC Secretary of State’s website and observed that the owner of this facility is listed as current/active. During today’s visit, a full assessment of Child Care Requirements was conducted. I observed all required postings, attendance, and safe sleep logs. I reviewed a sample of children’s records, staff files and all applicable program records. The following violations were observed, documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 862 The EMC plan was not reviewed with all staff annually and whenever the plan was revised. Emergency Medical Care plan was not reviewed annually by staff. 10A NCAC 09 .0802(a) 1232 Each employee's personnel file did not contain an annual staff evaluation and a staff development plan. All staff evaluations were not completed annually. 10A NCAC 09 .0514(f) 1825 All staff did not review the center's EPR Plan during orientation and/or on an annual basis with the trained staff. Documentation of the review was not maintained on file. EPR was not reviewed annually with staff. .0607(f) To comply with the NC Laws and Rules, any violations cited today must be corrected immediately. A compliance letter must be sent to me by June 3, 2026. The letter must address each violation, explain how it has been corrected and how it will be maintained in the future. If the compliance letter is not received by the date listed a return visit will be made and the violations cited again. During today’s visit, I observed that the 2015 Ford Van plate DMV7051 had an expired sticker. Ms. Hare stated that the van had not been driven in over a year. The permit, sanitation, evacuation, safe arrival and departure procedures, first aid chart, menus, activity plan, daily schedule, staff/child ratios, emergency medical care plan, emergency telephone numbers and a summary of NC Child Care Law were all posted at the time of the visit. The storage of medication and hazardous products was also monitored for compliance. Files for new staff hired since the 11-25-24 routine unannounced visit and a sample of existing staff files were reviewed. A sample of children's records were reviewed and the worksheet was completed. The last fire drill was documented as conducted on 5-28-25. The last outdoor inspection was documented as conducted on 4-21-25. A lock down drill was conducted on 5-28-25. An approved fire inspection was completed on 5-27-25. A sanitation inspection was last completed on 3-21-25 with two demerits and a superior classification. During the visit, the children were observed participating in a circle time activity and playing outside. For lunch the children had sloppy joes, peas, oranges and milk. A five star child care center with seven points in education, six points in program standards and one quality point was issued on 1-14-20. Based on information provided and observations made during the visit you continue to meet the requirements previously met to obtain such a license. Violation Number Comment Rule 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance).
