This visit was conducted with Melinda Lescar, Child Care Consultant and Melissa King, Lead Teacher. Your program currently operates with a Five Star license. Restrictions include 1st shift, meets all enhanced requirements and reduced ratios minus one, and school age equipment cannot be used by preschoolers. The license was observed, and the restrictions were found in compliance. The NC Secretary of State website was reviewed on January 12, 2025, and Surry County Schools was listed as current and active. The program’s compliance history was 97% prior to today’s visit. All programs are required to maintain at least 75% compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed staff/child interactions today. I observed children in large group time, free play, and lunch. I observed materials and activities that were available to the children in the classrooms. The last annual compliance visit was conducted on February 7, 2025. A sanitation inspection was completed September 9, 2025, with a Superior classification. The last fire inspection was conducted on March 4, 2025. Program records and required postings were monitored. A fire drill was conducted on December 11, 2025. A locked down drill was documented on September 17, 2025. An outdoor inspection was documented on December 16, 2025. Staff files and children’s records were monitored per DCDEE procedures. Nutrition requirements were monitored per DCDEE procedures. Storage of hazardous items was monitored today. No medications were observed nor reported. You stated that you provide transportation in the morning and afternoon for seven (7) children. During today’s visit, I monitored emergency information, permission to transport forms for all children being transported. All the required DPI paperwork for transportation was available onsite. The NC Pre-K requirements in section .3000 of the childcare rules were monitored for compliance. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified in compliance. The current NC Pre-K staff are lead teacher, Melissa King and teacher, Renee Bullington. A selection of files was monitored for completed health assessments and developmental screenings. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. Your facility uses Creative Curriculum. You stated you were in the process of completing the Dial-4 screenings. The center uses the Teaching Strategies Gold instrument to document evidence of children's ongoing progress and documentation was viewed today. Checkpoint assessments are conducted three times per year: at the beginning, middle and end of the school year. The first day of school was August 27, 2025. The classroom operates from 7:30a.m. to 3:00 p.m. Parent conferences are held two times a year. You stated that you use Class Dojo and newsletters to communicate with parents. In addition, a planner is sent home to the parents which includes any information about the child’s day and any reminders. You verified that you incorporate family engagement by holding Literacy Night and a Fall Carnival. The environment rating scale score received on December 12, 2019, was 5.84. The following violations were cited during today’s visit: Violation Number Comment Rule 1805 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. The facility roster did not have all the current staff and administrator linked. G.S. 110-90.2 & .2703(r) 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. The last lock down drill was not documented on the log. Melissa King, lead teacher assisted me with the visit. Derek Sheets, principal, was on site and available to assist as needed. Your program currently operates one classroom with a five star license effective 1/7/20. Restrictions include daytime care, meets enhanced space, enhanced ratios, enhanced ratios minus one, and school-age play equipment cannot be used by preschool children. The license was observed, and the restrictions were found in compliance. NC Pre-K requirements were monitored during your April 2023 annual compliance visit. The program’s compliance history was 95% prior to today’s visit. All programs are required to maintain at least 75% compliance. A playground inspection was conducted 11/16/23. You agreed to do a December inspection before you leave for winter break. A fire drill was last documented 12/15/23 and a lockdown drill on 11/1/23. You provided me with a copy of your most recent fire inspection conducted 8/20/23. I observed a sanitation inspection last conducted on 8/28/23 earning a Superior classification with 4 demerits. I observed positive interactions today. During this visit, I observed lunch, indoor free play, and children having the opportunity to attend a quarterly reward popcorn and movie experience in the gym for Pre-K through 2nd grade . The movie lasts 30 to 45 minutes and you have agreed to log this into your screen time log. I observed emergency medication stored 5 feet up in the classroom. We reviewed today that emergency medications are not required to be locked as they should be easily accessible when needed with no potential barriers to accessing them. Storage of hazardous items was monitored today and are stored in a locked classroom cabinet. No new staff were reported at this program since your annual compliance visit on April 11, 2023. Limited monitoring of continuing staff files was conducted for the purpose of reviewing training regarding CPR/First Aid, EPR, Playground Safety, Special Training including Health and Safety Training and Criminal Background Checks. Four children enrolled at this site ride the public school bus to and from school. I observed the required information on file for transportation. You stated each bus driver has been provided with an emergency and identifying information sheet with photo for each child transported. I observed a copy of parents signed permission to transport including when and where the child is to be transported, approximate times of arrival and departure, and signed authorization that buses are insured, inspected, and in good, safe condition. Pre-K Requirements: Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified in compliance.
