This visit was conducted with Melinda Lescar, Child Care Consultant and Kari Jessup, Teacher. Your program currently operates with a Five-Star license effective 2/5/2020. Restrictions include first shift, meets all enhanced requirements and reduced ratios minus one and preschool children may not use school age playground equipment. The NC Secretary of State website was reviewed on 5/8/2026, and Surry County Schools were listed as current and active. The program’s compliance history was 94 % prior to today’s visit. All programs are required to maintain at least 75% compliance. Today, the following items were monitored: • Supervision • Staff / Child Ratio • CPR / First Aid • Special Training • CBC Qualification • ITS – SIDS • Emergency Medical Care Plan • Administration of Medication • Storage of Hazardous Products • Storage of Medication • General Safety • Discipline • Adequate / Approved Space • Program Records • License Posted • Permit Restrictions The license was observed, and the restrictions were found in compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed staff/child interactions today. I observed children in free play activities and rest time. There are not any new staff reported at this program. Storage and administration of medication were monitored. Medication authorization was monitored. Storage hazardous items were monitored today. A playground inspection was recorded on 4/13/2026. A fire drill was recorded on 4/30/2026. A shelter in place drill for the facility was recorded on 3/3/2026. The most recent fire inspection was on 3/24/2026. I observed a sanitation inspection that was last conducted on 5/4/2026, earning a Superior classification. You stated that you do transportation in the morning for three (3) children and in the afternoon for one (1) child. During today’s visit, I monitored emergency information, permission to transport forms for all children being transported. All DPI transportation forms were on file and signed by the appropriate individual. All bus drivers had their criminal background checks on file. The following violations were cited during today’s visit: Violation Number Comment Rule 843 A drug or medicine was administered after its expiration date. One inhaler observed expired on January 2026. 10A NCAC 09 .0803(1)(d) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. I observed staff/child interactions today. I observed children large group time, free play, and lunch. I observed materials and activities that were available to the children in the classrooms. The last annual compliance visit was conducted on February 19, 2025. A sanitation inspection was completed September 8, 2025, with a Superior classification. The last fire inspection was conducted on September 11, 2025. Program records and required postings were monitored. A fire drill was conducted on December 19, 2025. A locked down drill was documented on September 30, 2025. An outdoor inspection was documented on December 3, 2025. Staff files and children’s records were monitored per DCDEE procedures. Nutrition requirements were monitored per DCDEE procedures. Storage of hazardous items was monitored today. No medications were observed nor reported. You stated that you provide transportation in the morning and afternoon for five (5) children. During today’s visit, I monitored emergency information, permission to transport forms for all children being transported. All the required DPI paperwork for transportation was available onsite. The NC Pre-K requirements in section .3000 of the childcare rules were monitored for compliance. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified in compliance. The current NC Pre-K staff are Lead Teacher, Tracy Lewellyn and Assistant Teacher, Kari Jessup. A selection of files was monitored for completed health assessments and developmental screenings. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. Your facility uses Creative Curriculum. You stated you were in the process of completing the Dial-4 screenings. The center uses the Teaching Strategies Gold instrument to document evidence of children's ongoing progress and documentation was viewed today. Checkpoint assessments are conducted three times per year: at the beginning, middle and end of the school year. The first day of school was August 27, 2025. The classroom operates from 7:30a.m. to 3:00 p.m. Parent conferences are held two times a year. You stated that you use Class Dojo and daily interactions to communicate with parents. In addition, a planner is sent home to the parents which includes any information about the child’s day and any reminders. You verified that you incorporate family engagement by sending projects home for families to complete together. Parents are invited to have lunch and attend special events in the classroom. The environment rating scale score received on January 24, 2020, was 5.76. The license was observed, and the restrictions were found in compliance. This program is owned and operated by Surry County Schools. The program’s compliance history was 86% prior to today’s visit. All programs are required to maintain at least 75% compliance. I conducted a walk-through of the facility observing indoor and outdoor areas. Upon arrival, staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision of children was adequate during my observations. I observed positive interactions today. I observed children having indoor and outdoor free play, group time, lunch, and rest. Lunch included pepperoni pizza, potato wedges, frozen juice cup, and unflavored milk. I observed developmentally appropriate materials and activities were available to the children in the classroom and outside. The last annual compliance visit was conducted 3/20/23. A sanitation inspection was last conducted 9/7/23 with a Superior classification. The last fire inspection was conducted 10/9/23 which you provided me with a copy following your routine unannounced visit in September. Program records and required postings were monitored. A fire drill was last documented on 2/19/24 and a lockdown emergency drill on 2/5/24. A playground inspection checklist was last completed on 2/2/24. Staff files and children’s records were monitored per DCDEE procedures. Storage of hazardous products was monitored and are kept locked. No medications were observed or reported. Transportation: I monitored transportation requirements for bus riders. A transportation form was completed and signed by a school administrator verifying that all vehicles are in good condition and free of hazards; have current registration and inspection; all drivers have proper license and have completed orientation on transporting preschool children; appropriate child restraint devices are used, if required by NC Child Restraint Laws; and all DCDEE procedures are being followed. This Pre-K classroom operates from 8am until 3pm Monday through Friday, 10 months per year. Staff-child ratios and maximum group sizes required for NC Pre-K classrooms were verified in compliance. Ms. Lewellen, lead teacher has a NC B-K licenses and the assistant teacher has an AAS in Early Childhood Education. The principal is a Level III NCECAC. A selection of children’s files was monitored for completed health assessments and developmental screenings. Your program was monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. Your facility uses The Creative Curriculum. Tracey Lewellyn, lead teacher and Kari Jessup, assistant teacher assisted me with the visit. Your program currently operates one classroom with a five star license effective 2/5/20. Restrictions include daytime care, meets enhanced space, enhanced ratios and enhanced ratios minus one; and school-age playground equipment off-limits to preschool children. The license was observed, and the restrictions were found in compliance. The program’s compliance history was 83% prior to today’s visit. All programs are required to maintain at least 75% compliance. A playground inspection form was last completed September 26, 2023. A fire drill was documented 9/28/23 and a shelter-in-place on 9/20/23. The last documented fire inspection was 9/8/22. You stated an inspection was conducted in September 2023 and you agreed to email the report to me tomorrow, 10/5/23. I observed a sanitation inspection was last conducted on 9/7/23 earning a Superior classification with four (4) demerits. Indoor and outdoor areas were observed. Upon arrival, staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed positive interactions today. I observed a teacher redirect inappropriate behavior in a manner that did not appear to be disciplinary but instead just allowed the child to change activities. The classroom was well furnished with developmentally appropriate materials and equipment. During this visit, I observed indoor and outdoor free play, group time, lunch, and rest. Lunch included cheese pizza, potato wedges, pears, and 1% unflavored milk. No medications were observed or reported. Storage of hazardous items was monitored today and are stored in a locked classroom closet. No new staff were reported at this program. Limited monitoring of continuing staff files was conducted for the purpose of reviewing training regarding CPR/First Aid, EPR, Playground Safety, Special Training including Health and Safety Training and Criminal Background Checks. Transportation: Several children enrolled at this site ride the public school bus to and from school. Each bus driver has been provided with an emergency and identifying information sheet with photo for each child transported. I observed transportation documentation verifying vehicle maintenance and compliance with DCDEE transportation requirements. I observed written permission to transport forms completed although some of the forms did not contain arrival and departure times and where the child is to be transported. Your program was monitored today for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. Your facility uses The Creative Curriculum. This is an NC Pre-K classroom although there are two three year old children enrolled. You stated instructional hours are 7:30 to 3:30. You conducted home visits at the beginning of the year and will do one parent conference with each family. You said you communicate with parents daily through email, daily notes, and a weekly newsletter. You offer families the opportunity to participate in PTO meetings, movie night, fall festival, skate night activity, etc. You stated you do three checkpoint assessments each year. Your program is in Cohort 2 to resume the NC Rated License Assessment which means your preparation year will begin July 1, 2024. The following violations were observed during today’s visit and documented as follows: Violation Number Comment Rule 620 All walls and ceilings including doors and windows were not kept clean and in good repair. There is a hole in the wall in the classroom bathroom used by the children. 15A NCAC 18A .2825(a) 859 Monthly playground inspections were not completed and/or they were not completed by an individual trained in playground safety requirements. Monthly playground inspections were not completed for the months of March, April, and May 2023. .0605(q) 9995 A violation was found for which there is no item number. There was a water bottle stored in a child's cubby containing an orange beverage. The bottle was not labeled with the child's name. Staff was not aware of what the beverage was - whether juice, gator aid, punch, etc. and whether it required refrigeration. Child Care programs are expected to be in compliance at all times and are required by NC GS 110-90(4) (d) to maintain an eighteen-month compliance history score of at least seventy-five percent. Also, at the beginning of the day, you are required to review with the sub the Prevent Shaken Baby Policy and the EPR plan and its location. • You stated one 4-year-old child is having severe separation anxiety in the mornings at drop off. I recommend that you read developmentally appropriate stories to the children about separating from parents. Also you could consider doing a family wall where children bring in photos of their family to display in the classroom. Another thought is to use the child’s particular interests to come up with an incentive to reward the child when the child walks into the classroom versus having to be carried in by the teachers.
