This visit was conducted by Melinda Lescar, Childcare Consultant with DeAnna Barnes, Director. Your program currently operates with a Four-Star license. Restrictions include 1st shift, meets enhanced ratios, children under age 2 ½ years old in rooms with direct exits. in rooms with direct exits. A Written Warning Administrative Action was issued on December 23, 2025. The Written Warning Administrative Action, Cover Letter, and Corrective Action Plan were posted. The program’s compliance history was 90% prior to today’s visit. All programs are required to maintain at least 75% compliance. The NC Secretary of State website was reviewed on 1/29/2026 and ACWLC, IV Inc. was listed as current and active. Today, the following items were monitored: • Supervision • Staff / Child Ratio • CPR / First Aid • Special Training • CBC Qualification • ITS – SIDS • Storage of Hazardous Products • Storage of Medication • Discipline • New Staff Files (if applicable) • Adequate / Approved Space • Program Records • License Posted • Permit Restrictions • Group size • Licensed capacity Upon arrival, staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. One (1) new staff files were monitored per DCDEE procedures. Program records and required postings were monitored. A fire drill was conducted on 12/24/2025. A shelter in place drill was documented on 9/24/2025. An outdoor inspection was documented on 12/25/2025. Storage of hazardous items was monitored today. No medications were observed nor reported. The Corrective Action Plan progress was monitored during today’s visit. Item #1: The child care operator shall maintain compliance at all times with all applicable child care requirements including, but not limited to, Child Care Rule 10A NCAC 09.1801(a) (1-5) regarding supervision. -All spaces were in compliance regarding ratios and supervision during the visit. One violation was cited for an emergency drill that was not conducted every three (3) months. The last shelter-in-place drill was conducted and documented on 9/24/2025. Ms. Barnes stated it was overlooked and she would make sure this would not occur again by putting a reminder on her calendar to conduct the emergency drills every three(3) months to ensure compliance. Item #2: Within one (1) week after this notice is received, DeAnna Barnes, administrator, shall contact Melinda Lescar, Child Care Consultant, PO Box 25174, Winston-Salem, NC 27114, telephone number (336) 987-0960, email melinda.lescar@ dhhs.nc.gov, to arrange for a rules review that will address supervision. No children shall be in care during this training. All staff members, including administrative, full-time, part-time, auxiliary, substitute, and volunteer staff, shall participate in the mandatory training. Documentation of the training shall be maintained in the facility file for review by representatives of the Division of Child Development and Early Education upon request. -The notice was received on 1/6/2026 and reviewed on 1/7/2026. Ms. Barnes contacted Ms. Lescar on 1/9/2026 to schedule the rules review. The rules review was scheduled for 1/29/2026. Due to weather conditions the rules review was rescheduled for 2/5/2026. Item #3: Within two (2) weeks after the required rules review is completed, Ms. Barnes shall review and revise the facility’s supervision policy and procedures to incorporate strategies learned in the rules review. The policy and procedures should describe, in detail, the steps the facility will take to ensure compliance with supervision. The policy and procedures shall include, but not limited to, the following: • Child Care Rule 10A NCAC 09.1801(a) (1-5) regarding supervision • Implementation of a name-to-face supervision record • When children are arriving and departing from the facility each day • When more than one group of children is combined • When children are transitioned from one space to another • When children are transitioned from the inside the classroom to the outdoor playground, and then from the outdoor playground to inside the classroom • Procedures for supervisory staff members to visit each classroom and monitor staff members’ implementation of the supervision plan • Procedures for periodic review of the supervision plan with all staff members, including the review of the plan in the orientation of new staff members before they assume child care responsibilities The written procedures shall be submitted to Ms. Lescar, for review. Ms. Lescar shall notify Ms. Barnes, orally and in writing, as to whether the written policy and procedures meet the requirements of this stipulation or if modifications are needed. One notification of stipulation being met is received, the policy and procedures shall be immediately implemented and incorporated into operating procedures. A copy of the policy and procedures shall be maintained in facility files for review by representatives of the Division of Child Development and Early Education upon request. Item #4: Within two (2) weeks after the notification from the Division that the stipulation has been met for the revision of procedures related to supervision, Ms. Barnes shall conduct a staff meeting with all staff members to discuss the procedures. Documentation of the staff meeting shall include an attendance roster with the printed name and signature of each staff member in attendance, the date, time and length of the meeting, and minutes documenting the information discussed during the meeting. All documentation shall be maintained in facility files for review by representatives of the Division of Child Development and Early Education upon request. The following violations were cited during today’s visit: Violation Number Comment Rule 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. You must correct the violations cited during today's visit immediately and send me documentation verifying compliance on or before February 13, 2026. During today’s visit, we conducted a walk-through of the facility unaccompanied. During the visit, I discussed the information with DeAnna Barnes, administrator, and Meredith Burnette, assistant director. Limited monitoring of child care requirements occurred during today’s visit. Staff/child ratios in space5, supervision, adequate approved space, license posted and permit restrictions were monitored. Based on information obtained, the following was determined. On Monday, 11/17/25 at 3:32 pm, a four-year-old child asked the lead teacher in the classroom where four- and five -year-old children are enrolled, space 5, that he needed to go the bathroom. The bathroom is not in the classroom, so the children have to go down a little hallway. Neither the lead teacher nor teacher went with child to the bathroom. The child left the classroom at the front door facing the school age classroom turned left and walked to the bathroom located in a small hallway with the bathroom on the right. The child never returned to the classroom. The child was unsupervised from 3:33 until 4:23pm when the parent arrived. At 4:23PM the child’s mother arrived to pick up her child. When the parent arrived in the classroom, she discovered that her child was not in the classroom. The parent located her child in the bathroom, in the hallway. Ms. Barnes discussed the incident with the parent and the child was not injured or upset and is still enrolled in care. The two staff members have been terminated. Both the director and assistant director were interviewed, and video was observed of the incident. Based on the above information the incident was confirmed; therefore, the allegation was substantiated. The following violation was confirmed during today’s visit. Violation Number Comment Rule 303 Children were not adequately supervised at all times. One (1) child is space 5, left the classroom to go to the bathroom in the hallway and was unsupervised for approximately 50 minutes. .1801(a)(1-5) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. You must correct the violation cited during today's visit immediately and send me documentation verifying compliance on or before 12/16/25. The caregiver was unaware the child had not returned to their assigned classroom for 50 minutes. Child Care Rule 10A NCAC 09 .1801(a) requires children be adequately supervised at all times in child care centers. Due to a violation being cited regarding inadequate supervision documented on two consecutive visits, an administrative action may be recommended, and you will be notified in writing of any action taken. In addition, a follow-up visit will be conducted. Ms. Barnes reviewed supervision with staff on November 6, 2025, and after the incident she reviewed this information once again. Since the incident, the facility has removed the bathroom door in the hallway, moved game cart outside of door to allow for a clear view of hallway, the facility plans to install cameras in the hallway outside the bathrooms, and mirrors. In addition, a new support staff has been hired, and they are training the new support staff in regard to supervision. The administration has decided to require staff to be more diligent regarding the transition policy and will provide additional orientation with new employees. Ms. Barnes stated Thursday, December 4, 2025, a staff meeting is scheduled. The agenda will include supervision, safe handling of children, and transition policy. Active Supervision According to the Early Childhood Learning and Knowledge Center (ECLKC), active supervision requires focused attention and intentional observation of children at all times. Staff position themselves so that they can observe all of the children: watching, counting, and listening at all times. Staff use their knowledge of each child’s development and abilities to anticipate what they will do, then get involved and redirect them when necessary. This constant vigilance helps children learn safely. You may consider a dry erase board inside the classroom at door to continuously monitor the children present and when children and enter the space upon arrival and transition times. Additional Information/Consultation: Today upon arrival the director and assistant director were not present for approximately 30 minutes as the local public school was on a 2-hour delay, and they were transporting children to school. Upon arrival, Ms. Barnes, director, asked who let us into the building and we shared with her a parent, and she said a staff member told her we were doing a walk through. I shared with Ms. Barnes we only went into space 3 and space 7. I asked Ms. Barnes if she would prefer us not to complete a walk without her present. Ms. Barnes requested a walk-through not be completed if the assistant director or herself were not present. At the completion of the visit, this visit summary was reviewed with and provided to you. In addition, a follow-up visit will be conducted. Ms. Barnes reviewed supervision with staff on November 6, 2025, and after the incident she reviewed this information once again. Since the incident, the facility has removed the bathroom door in the hallway, moved game cart outside of door to allow for a clear view of hallway, the facility plans to install cameras in the hallway outside the bathrooms, and mirrors. In addition, a new support staff has been hired, and they are training the new support staff in regard to supervision. The administration has decided to require staff to be more diligent regarding the transition policy and will provide additional orientation with new employees. Ms. Barnes stated Thursday, December 4, 2025, a staff meeting is scheduled. The agenda will include supervision, safe handling of children, and transition policy. Active Supervision According to the Early Childhood Learning and Knowledge Center (ECLKC), active supervision requires focused attention and intentional observation of children at all times. Staff position themselves so that they can observe all of the children: watching, counting, and listening at all times. Staff use their knowledge of each child’s development and abilities to anticipate what they will do, then get involved and redirect them when necessary. This constant vigilance helps children learn safely. You may consider a dry erase board inside the classroom at door to continuously monitor the children present and when children and enter the space upon arrival and transition times. Additional Information/Consultation: Today upon arrival the director and assistant director were not present for approximately 30 minutes as the local public school was on a 2-hour delay, and they were transporting children to school. During today’s visit, Deanna Barnes, administrator, accompanied me during a walk-through of the facility. During the visit, I discussed the information with Deanna Barnes, administrator, and an additional staff member. On Monday, November 3, 2025, at approximately 5:00pm, the lead teacher in the classroom for two-year-old children, A.J., was told by the assistant director M. Burnette to combine and take children remaining in her classroom to the younger two-year-old classroom. A.J. had the head count and name to face sheet with her when she left the classroom with four (4) of the five (5) children. A. J. did not turn around as she shut the door behind her. Ms. Barnes stated the child was left less than a minute. Ms. Barnes stated that C.M., the lead teacher in the classroom for the younger two-year-old children, was returning the vacuum cleaner to the classroom for the older two-year-old children and seen the child at the door looking at the window. Ms. Barnes stated C.M. took her back to the younger classroom with the other children. I asked Ms. Barnes when the teachers transitioned their children to another room are the children signed onto the new transition sheet, and she stated no as this would be too confusing. The mother of the child was made aware of the situation that had occurred on Tuesday, November 4, 2025. During the visit the video from the classroom for older two-year-old children was observed. The child was left alone in the classroom from approximately 4:54pm to 4:56 when C.M. came into the classroom. The child was walking near the door and looking at the window. In the video the child did not appear to be crying or in distress. We observed in the two classrooms for 2- and 3-year-old children. Supervision was adequate. I interviewed one teacher during today’s visit. C.M. the lead teacher in the classroom for younger two-year-old children was interviewed. She stated she was vacuuming in the classroom when A. J. came into the room with four (4) children. She stated she had her back to the door when they walked in and as she turned around A.J. was standing there. A.J. asked if she could take the vacuum cleaner to her room when she was finished. C.M. stated she walked to the room and the child was waving at her through the window. As C.M. walked into the room she asked the child are you by yourself and child said no and C.M. looked around the room and no one was there. She took the child back to the front to the assistant director and explained what had just occurred. C.M. then took the child to her room and the assistant director accompanied CM to the room. I asked Ms. Barnes what steps have been taken since the incident occurred. Ms. Barnes stated a staff meeting occurred on November 5th and reviewed the staff transition policy and all twenty-two (22) staff signed statements that the staff transition policy had been reviewed with them. The staff transition policy and all signed statements were observed during the visit. Based on information obtained, the following was determined. A two-year-old child was left alone approximately 2 minutes in the classroom before a staff member found the child. Limited monitoring of child care requirements occurred during today’s visit. Effective supervision includes anticipating potential risks, monitoring behavior and interactions, and positioning oneself strategically to see all children and be near those who might need extra support. It is crucial to consider the individual needs, ages, and activities of the children, as well as environmental hazards, to provide appropriate and continuous supervision. Caregivers must always be able to see and hear all children, even during transitions and must continuously scan the environment and frequently count children, especially when moving from one location to another. Transitions are acritical time. Procedures like "face-to-name" checks are essential to ensure no child is left behind. Caregivers should be able to render immediate assistance is critical. This might change depending on the activity and the child's age and abilities Staff should strategically place themselves to see all children and be close to areas of high risk or activity, such as play structures or craft tables and attentively listening can help identify potential dangers or concerns expressed by children. Engaging with children helps prevent misbehavior and accidents and shows them that you are paying attention and caregivers must be ready to intervene and provide guidance when children's behavior becomes unsafe or when they need assistance. Ms. Barnes addressed this concern. The staff member was terminated November 4, 2025. This visit was conducted with Ms. Deanna Barnes, Director. This program currently operates with a Four Star license. Restrictions include daytime care only, meets enhanced ratios, children under 2 ½ years old in rooms with direct exits. The license was posted and the restrictions were being followed. The NC Secretary of State website was reviewed on 09-11-2024. The program’s compliance history was 96% prior to today’s visit. Programs are required to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. I have observed indoor and outdoor areas today. I monitored staff/child ratios and space capacity today, as is indicated on the attached worksheet. I observed proper supervision of children. During the visit I observed children playing indoors and outdoors and taking a ride in the stroller. The last sanitation inspection was conducted on 05-28-2024. The last fire inspection was conducted on 05-24-2024. A fire drill was documented on 08-16-2024. A shelter in place drill was documented on 06-25-2024. An outdoor inspection was documented on 08-23-2024. I observed program records and posted items today. I observed staff and children’s records today according to DCDEE procedures. I observed the storage of hazardous items and cleaning supplies. I observed both the storage of medications and the administration of medication paperwork. This program provides transportation. During today’s visit, I monitored one vehicle(s) for the following: • Emergency information • Permission to transport • Transportation logs • First aid kits and fire extinguishers • Condition of the vehicle(s) • Paperwork concerning vehicle compliance with NC Law I documented the following violations during today’s visit: Violation Number Comment Rule 811 Potentially hazardous items including but not limited to power tools, nails, chemicals, propane stoves, lawn mowers, gasoline, or kerosene were not stored in locked areas, removed from the premises, or made inaccessible to children. One container of hand sanitizer was on a low shelf in a cupboard in space 4. .0604(a) 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. One outlet was not covered in space 5. 10A NCAC 09 .0604(c) Additional monitoring visits may be conducted during the year to monitor compliance with applicable childcare requirements. Consultation: I suggested that the staff look at the room arrangement in space 3 and possible consider getting wooden blocks. I have reviewed this visit summary with Ms. Barnes at the conclusion of the visit and a copy was given to her. The visit was conducted with DeAnna Barnes, Director, by Tammy Childress, Lead Child Care Consultant. Currently this facility operates with a Temporary License, issued on July 20, 2023, to operate first and provide care for children ages zero to twelve years old in seven classrooms. Space 1 has been divided into two classrooms for infants and Space 2 has been divided into 2 classrooms for toddlers. Restrictions are daytime care only and children under 2 ½ years old in rooms with direct exits only. The Secretary of State website was checked today, and the owner, ACWLC IV, INC., remains current on all required documents and active on the NC Business Registry. If any changes to the business type need to be made or you decide to sale your business, then you must notify your consultant at least thirty days prior to any changes occurring. The license cannot be bought, sold, subleased, transferred to another person or location or inherited. All designated indoor and outdoor spaces for child care were monitored, as well as all staff files and a percentage of enrolled children’s files. All program records and documents required to be posted were monitored. Four playgrounds separated for infants, toddlers, preschoolers, and school age children were monitored. All transportation requirements and the vehicle(s) used to transport children were monitored during the visit. Children present were observed to be engaged in outdoor play, free choice activities, personal care routines, lunch, and nap. Staff use the Learn Every Day: Preschool Curriculum. The following violations were cited today: Violation Number Comment Rule 1123 All vehicles used to transport children were not free of hazards. Foam was exposed on the arm rest of the passenger seat of the vehicle used to transport children. 10A NCAC 09 .1002(a) All violations must be corrected immediately. You must submit documentation on how each violation was corrected to Ms. Childress by November 28, 2023. The documentation should be submitted by email to Ms. Childress at tammy.childress@dhhs.nc.gov. During today’s visit we again discussed the Rated License Assessment Process and the Environment Rating Scale Assessment. We reviewed the program and education components for a star rated license, how points are earned, and the Environment Rating Scale Assessment. The application for a Star Rated License Assessment and Request for an Environment Rating Scale Assessment were received on October 15, 2023. Ms. Barnes previously requested a delay in requesting the ERS Assessments until a technical assistance visit could be conducted by the local smart start agency and stated she was waiting for additional materials to arrive. An Environment Rating Scale Assessment has now been scheduled with the North Carolina Rated License Assessment Project. You should continue to prepare for the ERS Assessment by reviewing the Early Childhood Environment Rating Scale (revised edition) booklets, visit the North Carolina Rated License Assessment Project website at www.ncrlap.org and review resource guides/additional notes/ informational videos and participate in the free online training. Your Temporary License will expire on January 20, 2024, and in order to continue to operate you must be issued a star rated license. In order to continue to participate in the NC Subsidized Child Care Program, you must be issued a license with Three Stars or higher. The Rated License Assessment will include a review of the following to determine points in each component: Program Standards- TO BE DETERMINED FOLLOWING ENVIORNMENT RATING SCALE AND STAR RATED LICENSE ASSESSMENT. Based upon the ITERS-R, ECERS-R and SACERS-U Assessment scores, you will need to determine if you will meet the enhanced space and/or ratio requirements. Education- 4 Points Current Education Points are based upon WORKS Accounts and statements from Ms. Barnes that certain staff will complete additional semester hours in December 2023. Quality Point- 1 Point The Quality Point options were reviewed with Ms. Barnes during today’s visit and Ms. Barnes chose to meet the following which was verified in the center policies and website: Staff Benefits Package (4): Paid Leave for Professional Development; Paid Planning Time; Vacation: Health Insurance Infrastructure of Parent Involvement (2): Quarterly Newsletter, Periodic Conferences for All Children Technical Assistance: During the pre-license consultation visits, transportation policies were approved on 7/17/2023 and on 7/19/2023 a 2008 Chevrolet Private/Small Bus was approved for use in transporting children. Since that time a 2003 Ford 15 Passenger Van has been issued to the center for use for school age field trip transportation. Tag# RYA-2055 with valid registration and insurance. This vehicle was monitored today, and foam was exposed on the arm of the passenger front seat and on the arm of one bench seat. The velcro material covering the bench seat arm was able to be readjusted and foam was no longer accessible. The velcro on the arm on the passenger seat will need to be repaired. A small wooden ladder to assist those children who cannot step onto the van alone was stored under the first bench seat and was attached to the seat anchors with bungie cords. One bungie cord was frayed and not working. This cord was removed. Ms. Barnes stated that the van will not be used until the repair to the armrest is done. At this time, the vehicles are only used for field trips. All other transportation requirements were observed to be in compliance including a binder located on the vehicle with children’s identifying and emergency information and a photo. Ms. Barnes stated that she would submit compliance documentation stating the van would not be used until the arm rest was repaired. I would also encourage you to implement a form for drivers to check to condition of the interior and exterior of the vehicles before using to note any needed repairs. 10A NCAC 09 .1002 SAFE VEHICLES (a) Vehicles used to transport children enrolled in child care centers shall be free of hazards such as, but not limited to, torn upholstery that allows children to remove the interior padding, broken windows, holes in the floor or roof, or tire treads of less than 2/32 of an inch. Consultation: Emergency Preparedness and Response Plan Emergency Preparedness and Response in Child Care training must be completed by at least one staff member (usually the administrator) within one year of the effective date of the initial license. Maintain documentation of this training. Once the EPR training is completed, you must complete an EPR Plan on a template at the following website https://rmp.nc.gov/portal/portal.aspx within four (4) months. After you complete the Emergency Preparedness and Response in Child Care training, you must conduct either a shelter-in-place or a lockdown drill every three months. Use the Emergency Drill Log to document these drills as well as the fire drills. The form is located under the Provider Documents tab on the DCDEE website at www.ncchildcare.ncdhhs.gov. Review the guidance document Ms. Childress emailed to you on October 17, 2023, regarding materials for activity centers as you prepare for upcoming Environment Rating Scale Assessments, especially in the areas regarding blocks, free art expression and displays of diversity. In addition, review “number talk” and encouraging reasoning with staff during their conversations with children as a whole group and individually. Ensure all materials, including music players (CD players if in the classroom) are accessible to children and can be used without teacher assistance. A summary of today’s visit was reviewed with and provided to Ms. Barnes. Ms. Barnes was given the opportunity to ask any questions.
