Candace Stevens, Administrator, accompanied me during a walk-through of the facility. During the visit, I discussed the allegations with Ms. Stevens and two additional staff members. Limited monitoring of child care requirements occurred during today’s visit. The following violations were observed and/or confirmed during today's visit: Violation Number Comment Rule 303 Children were not adequately supervised at all times. On April 8, 2025, staff members failed to provide adequate supervision when a two-year-old child exited the fenced area, unbeknownst to staff. .1801(a)(1-5) 807 A safe indoor and outdoor environment was not provided for the children. On April 8, 2025, staff members failed to provide a safe environment when, due to their lack of supervision, a two-year-old child exited the facility's fenced area. The staff member's failure to provide adequate supervision placed the child at risk of injury and/or harm. 10A NCAC 09 .0601(a) 852 Incident reports were not completed each time a child was injured, it did not include all the information required in rule, it was not signed by the parent and/or it was not maintained in the child's file. On March 19, 2025 a three-year-old child received medical care for an incident that occurred at the facility and staff failed to complete an incident report. .0802 (e) 1810 There was a substantiation of child maltreatment. Pursuant to its investigation, the Division has confirmed sufficient information to determine child maltreatment. GS 110-105.6(a) 1911 An incident report was not completed and mailed to a Division representative within seven days after the incident when medical treatment was required. On March 19, 2025 an incident occurred and, as a result, a three-year-old child received medical care. An incident report was not sent to the Division as required. .0802(f) All violations documented above must be corrected immediately. Candance Stevens, Owner/Administrator, accompanied me during a walk-through of the facility. During the visit, I discussed the allegations with Ms. Stevens and additional staff members. Limited monitoring of child care requirements occurred during today’s visit. The following violation was confirmed during today’s visit. Violation Number Comment Rule 303 Children were not adequately supervised at all times. In March 2024 or April 2024, a staff member failed to provide adequate supervision on at least two occasions which resulted in one or more three-year-old children completely disrobing in the classroom and on the playground. .1801(a)(1-5) 902 Each child was not attended to in a nurturing and appropriate manner, or in keeping with the child's developmental needs. On April 10, 2023, a staff member yelled at a three-year-old child while forcefully placing them in a chair and pushing the chair to the table. In December 2023, a different staff member used a harsh tone with a three-year-old child and roughly picked up and placed the child in a chair. The staff member left the child sitting without a developmentally appropriate activity. G.S. 110-91(10) 904 Child was handled roughly. On or about February 28, 2024, a staff member forcefully grabbed a three-year-old child off the lap of another staff member and carried the child by their biceps approximately four feet to a cot and put the child down roughly. .1803(a)(1) Violations must be corrected immediately. A checklist was used to note the requirements I monitored today. This facility currently operates with a Four Star Child Care Center License that was issued May 19, 2023, earning 7 points in the Education Component, 2 points in the Program Component and 1 Quality point for having a staff benefits package and an infrastructure of parent involvement. The sanitation inspection was completed on March 9, 2023, with a “Superior” classification. The last fire inspection was completed on December 6, 2023, and the facility was approved for day time care only. You, C. Stevens, Administrator, were not available upon arrival; however, you were called and arrived shortly after the visit began. You were notified of the purpose of the visit and was available for consultation at the conclusion of the visit. Prior to today’s visit, the program’s compliance history was 100% over the current 18- month period. A copy of your compliance history report was left with you. The facility is owned by the corporation C And L Child Development Center LLC. The corporation was researched and found to be current and active. The license was posted, and the restrictions were in compliance. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in both the indoor and outdoor learning environments and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in group time, free play in activity areas with blocks and manipulatives, outside on swings and climbing structures, and personal care routines. Infants were engaged in tummy time, napping, exploring the environment, and feedings. Sleep checks were completed, recorded, and maintained as required. The caregivers were interacting and meeting the developmental needs for each of the children. Activity plans were current, posted, and reflected NC Early Childhood Foundations. Rated license requirements were reviewed to ensure compliance was maintained with current program and education components as well as the quality point. Fire and sanitation inspections remain current. I used the Annual Compliance Monitoring Checklist for Child Care and the Child Care Center Item Number Listing (DCD 0357) as a basic monitoring tool to assess compliance with all applicable child care requirements. Violations documented were reviewed with you at the conclusion of the visit and a copy of the visit summary was provided to you. The following violations were documented: Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. On the playground for school age children and preschool age children, steps on the large play structure was rusting and flaking with sharp edges. On the playground for school age children, sharp thorny vegetation was growing through the fence, accessible to children. 10A NCAC 09 .0601(a) 843 A drug or medicine was administered after its expiration date. In space 3, Aquaphor diapering ointment was given to a child after the expiration date of 5/2023. SAFE ENVIRONMENT: While completing a walk-through of the outdoor environment, I observed the steps to the large play structures on the playground for preschool age children and school age children. The steps were rusting and flaking with sharp edges. We discussed you contacting the manufacturer to inquire about a warranty for the steps. We discussed options to make this area a safe area for the children to access. On the playground for school age children, there is sharp thorny vegetation growing through the fence, accessible to children. We discussed that this must be cut back or completely removed to make this inaccessible to children. MEDICATION: In space 3, there was medication that was expired. The Aquaphor diapering ointment expired 5/2023. When asked, staff member, L. Favors stated that the last time the medication was given was around October of 2023. We discussed having a staff member to periodically check all medication to ensure the medications have not reached the expiration date. This violation was corrected by having it removed from the classroom. CHALLENGING BEHAVIOR ASSISTANCE: Did you know assistance can be requested for help with handling challenging behaviors.
