Ophelia Howard, site director assisted me with the visit. The program operated with a four-star license, issued January 27, 2018, earning seven points in education standards, two points in program standards, and one quality point by choosing to meet an education option. License restrictions include first shift; school-age children, enhanced space, and playground area does not meet child care safety standards. The last annual compliance visit was conducted February 08, 2024. The center's compliance history was reviewed with the operator. The program’s compliance history was 99 % as of today. Your last sanitation inspection is dated December 11, 2024, with a “Superior” classification. The last fire inspection was conducted on January 10, 2024, and your facility was approved for daytime care only. You visited each indoor and outdoor space with me. Children were observed during teacher lead and free play activities. Teachers interacted with children on their level. Age-appropriate materials and equipment were accessible to children. Children were adequately supervised. BE ADVISED Othella Locklear's qualification letter expired 11/8/24. Joan Scott's qualification letter expired 10/31/24. Both teachers must have a valid qualification letter on file on or before 2/19/25. If a valid qualification letter is not on file on or before the 14th day, Ms. Locklear and/or Ms. Scott will not be allowed on the premises. Snack consisted of donuts and milk. The following violations were cited during today’s visit: Violation Number Comment Rule 526 Menus for all meals and snacks were not current or posted where easily seen by parents and cook. The menu posted was dated 1/31/25. 10A NCAC 09 .0901(b) 1035 Child care providers, including the director, uncompensated providers, substitute providers, and volunteers did not have the required Emergency Information Form on file on or before the first day of work, which included all the required information and/or the information on the form was not updated as changes occur and at least annually. Documentation was not updated at least annually for three staff members. .0701(a) 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). Two staff member's qualification letters are expired.
