The allegations are as follow: There are concerns of violations of child care requirements. Janet Edwards, Licensing Supervisor, accompanied today’s visit. Staff/child ratio, group size, supervision, use of licensed space, space capacity, license restrictions, and the outdoor environment were monitored. The license and emergency care plan were posted. Fire drills, medication administration, and storage of hazardous products were also monitored. No new files for new staff were reviewed. We observed the indoor and outdoor space used by the children. Today’s menu consisted of spaghetti, corn, bread, pineapples, and milk. Based on information provided, on March 25, 2025, there are concerns of violations of child care requirements. I interviewed Teresa Jacobs, Administrator and one teachers regarding allegations during today’s visit. Ms. Jacobs stated there are times when teachers raise their voice to correct or redirect children. Both Ms. Jacobs and one teacher confirmed a child was tapped as a form of discipline. Additionally at the end of the day children are moved into one classroom and watch an iPhone or tablet to watch movies. Based on the interviews and observations, the allegations of child care requirements were substantiated. Violation Number Comment Rule 524 When children 3 years and older were in care, screen time was not used to stimulate a developmental domain; was not limited to 30 minutes a day and no more than a total of two and a half hours per week, per child; and/or was not documented on a cumulative log or the activity plan that is available for review. For purposes of this Rule, "staff" shall mean any regular or substitute caregiver, any volunteer, and any auxiliary personnel, including cooks, secretaries, janitors, maids, or vehicle drivers. The following shall apply at all child care centers: (1) no child shall be handled roughly in any way, including shaking, pushing, shoving, pinching, slapping, biting, kicking, or spanking; (2) no child shall be placed in a locked room, closet, or box or be left alone in a room separated from staff; (3) no discipline shall be delegated to another child; (4) no food shall be withheld as punishment or given as a means of reward; (5) no child shall be disciplined for toileting accidents; (6) no child shall be disciplined for not sleeping during rest period; (7) no child shall be disciplined by assigning chores that require contact with or use of hazardous materials, such as cleaning bathrooms, floors, or emptying diaper pails; (8) physical activity, such as running laps and doing push-ups, shall not be withheld as punishment or required as punishment; (9) no child shall be yelled at, shamed, humiliated, frightened, threatened, or bullied; and (10) no child shall be restrained as a form of discipline unless the child's safety or the safety of others is at risk. The allegation are as follows: There are concerns that: A child was not treated in a nurturing and caring manner. A child was not adequately supervised. An assigned activity was not developmentally appropriate for a child. Children were not allowed outdoor time, although the weather was permitting. Staff/child ratio, group size, supervision, use of licensed space, space capacity, license restrictions, and the outdoor environment were monitored. The license and emergency care plan were posted. Fire drills, medication administration, and storage of hazardous products were also monitored. No files were reviewed due to no new employees. Additionally, I observed the indoor and outdoor space used by the children. I observed children during free play activities and teachers interacted with children as they played. Based on information provided on February 17, 2025, a three-year old child was sent to the bathroom unsupervised on the opposite end of the hallway. There is an exit door a few steps away from the bathroom and the child had access to walk out the door without supervision. I interviewed two lead teachers regarding supervision, and both confirmed that a child was sent to the bathroom across the hall without supervision. Based on the interviews and observed the allegation of inadequate supervision was substantiated. A parent arrived to pick up a three-year-old who was at a table with a pencil in his hand crying. According to the parent teacher, the child to write his name several times by copying dots. The parent observed the child crying and seating at the table while the other children played in the classroom. The parent stated the teacher did not help the child and just allowed him to cry while they both sat at the table. The parent enter classroom and asked the child what was wrong, and the child stated, “I can’t write my name”. The parent stated to the teacher that child was three years of age in a classroom for four-year-old children. The teacher stated to the parent as long as the child was in their class the child was going to learn to write their name. I interviewed the two lead teachers regarding allegations of a child was not treated in a nurturing and caring manner and assigned activity was not developmentally appropriate for a child both teachers responded that the teacher was seeing what skills the child has because they were preparing him to transition to the class for four-year-old children. The teacher of the classroom stated they did not just allow the child to cry but assisted the child in writing and when she got up to get another paper that is when the child started crying. Also, at the same time the child’s parent arrived to pick up the child for the day and the teacher started to explain she was seeing what skills the child had before the child transition to the classroom for four-year-old-children. Before she could finish talking the parent walked out and spoke with the administrator. Based on the interviews and observed the allegation of nurturing and caring manner was unsubstantiated. The allegation that an assigned activity was not developmentally appropriate for a child was unsubstantiated. Seven teachers addressed the allegation that children are not allowed to go outside because the teacher thought it was cold. All seven teachers stated they go outside unless there is a weather advisory. There are times when they adjust the daily schedule if they need to go out after the sun comes out or even rainy times. However, there are outdoor spaces with covers that allows all children to go outside even on wet and hot days. The allegation children were not allowed outdoor time, although the weather was permitting was unsubstantiated. The following violation was observed during today’s visit. Violation Number Comment Rule 303 Children were not adequately supervised at all times. Teresa Jacobs, Administrator, assisted me with the visit. The program operated with a five-star license, issued September 9, 2020, earning six points for education, six points for program standards, and one quality point for complying with an education option. License restrictions include daytime care only, children in care on ground level only, children under 2 ½ years old in rooms with direct exits only, meets enhanced ratios and meets enhanced space. The program’s compliance history was ninety-two percent as of December 17, 2024. The license was posted, and the restrictions were in compliance. The NC Secretary of State website was reviewed on December 17, 2024, and Island Grove Baptist Church was listed as current- active. Lunch consisted of smoked sausage, rice, tomatoes, bread, apple sauce, and peaches Violation Number Comment Rule 1032 Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. One staff did not have a medical report on file prior to employment. 10A NCAC 09 .0701(a) 1033 On or before the first day of work, all staff, including the director and individuals who volunteer more than once per week did not provide results indicating that they were free of active TB and/or TB test or screening was older than 12 months. License restrictions include daytime care only, children in care on ground level only, children under 2 ½ years old in rooms with direct exits only, meets enhanced ratios and meets enhanced space. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in both the indoor and outdoor learning environments and found supervision and staff/child ratios to be in compliance. Children were participating in free play in activity areas, transitions, and personal care routines. Teachers were interacting and meeting the developmental needs for each of the children. Age-appropriate materials and equipment were accessible to the children. Children were adequately supervised. One new staff files were reviewed due to no new employees. The following violations observed during today’s visit: Violation Number Comment Rule 544 Screen time was offered to children under three years of age. Space #4, three one year old toddlers were observed watching a phone screen displaying children cartoons. Language aids in a nonverbal child’s cognitive development. Conversations using descriptive and reasoning words help build a child’s vocabulary and language understanding. There are plenty of opportunities outside of a planned group time for conversations and language building activities. Children learn through play and interaction. Staff can use opportunities such as diaper changes or meals and snacks in addition to free play to talk to the children. Reminders: Qualifying Letters (background checks) The Division of Child Development and Early Education (DCDEE) is mandated to ensure all children who attend licensed /regulated child care facilities are protected and cared for in a nurturing and safe environment. The agency accomplishes this by assuring all caregivers working in licensed child care programs have a valid qualification letter on file with the facility prior to employment. Normie Bullard, directed assisted me with the visit. Your program currently operates with a Five- star license issued September 9, 2020. Your program is currently earning (6) points in the education component, (6) points in the program standards component (the program met enhanced ratios and enhanced space, an ERS score = 5.0 average and no classroom lower than 4.0), and 1 quality point for the child care administrator having at least 10 years of documented early childhood work experience that can be verified by the Division. Your program was also monitored for compliance with implementing an approved curriculum as required for all four and five star licensed facilities where four-year-old children are enrolled. The center's compliance history was reviewed with the operator. The program’s compliance history was eighty nine percent as of today. The NC Secretary of State website was reviewed today, and ISLAND GROVE BAPTIST CHURCH, was listed as current and active. The license was posted, and the restrictions were in compliance. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in both the indoor and outdoor learning environments and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in free play in activity areas, transitions, and personal care routines. The caregivers were interacting and meeting the developmental needs for each of the children. Teachers interacted with the children on their level. The infant engaged in carpet time. Teachers responded to the infants’ needs. Rated license requirements were reviewed to ensure compliance was maintained with current program and education components as well as the quality point. Lunch consisted of bologna, bread, string beans, apple sauce and milk. Two new staff member’s file was monitored and found in compliance during today’s visit. The infant room can hold its own unique set of challenges. At times all the babies may seem to need care all at once. During those moments when all the babies need your attention all at once, it is important to prioritize. Lay a tired baby down in his or her crib and turn on soft music. Make sure to follow sanitary guidelines and fill out the safe sleep chart. Place feeding babies in highchairs. If a child needs assistance with holding their bottle, make sure to hold them while assisting them. Remember to wash hands after each feeding. When help is available, teachers should not hesitate to ask for help from administration. A good routine and support system can help reduce stressful situations. I highly recommend reviewing the center's safe sleep policy with staff often. I advise you to contact your health consultant to help train your staff with handwashing, sanitizing, disinfectant, SIDS, CPR, First Aid training and other good health practices. The NC Secretary of State website was reviewed on today and ISLAND GROVE BAPTIST CHURCH was listed as current- active. The license was posted, and the restrictions were in compliance. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in both the indoor and outdoor learning environments and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in group time, free play in activity areas, transitions, and personal care routines. The caregivers were interacting and meeting the developmental needs for each of the children. The infants were cared for in a caring and nurturing manner. Teachers interacted with the children on their level. Rated license requirements were reviewed to ensure compliance was maintained with current program and education components as well as the quality point. Breakfast consisted of corn flakes, bananas, and milk. Three violations were documented during today’s visit: Violation Number Comment Rule 887 Caregivers did not document compliance with visually checking on sleeping infants aged 12 months or younger and/or the documents were not maintained for a minimum of one month. The action was discussed with Ms. Jacobs on June 1, 2023, and with Normie Bullard on June 29, 2023. Normie Bullard, director assisted me with today’s visit. During today’s visit the Administrative Action was posted at the parent entrance of the facility. Stipulations are as follows: 1. The child care operator shall maintain compliance at all times with all applicable child care requirements including, but not limited to, the following: • North Carolina General Statute § 110-91 and Child Care Rule 10A NCAC 09 .1803(a) regarding discipline of children • Child Care Rule 10A NCAC 09 .1801(a)(1-5) regarding adequate supervision 2. Within one (1) week after this Notice is received, Normie Bullard, administrator, shall contact Amanda Lovette, Robeson County Partnership for Children, telephone number 910-738-6767 Ext 297, email alovette@rcpartnership4children.org, to arrange for training that will address positive guidance, behavior management, and discipline of children. All staff members, including administrative, full-time, part-time, auxiliary, substitute, and volunteer staff, shall participate in this mandatory training. At least two (2) of the Board Members responsible for the operation/oversight of the facility shall attend the training. The training shall include, but not be limited to, the following topics: • Effective management of children’s challenging behaviors • Techniques for de-escalating stressful situations • Appropriate response and alternatives for children who exhibit acting out behaviors Documentation of the training shall include an attendance roster with the printed name and signature of each staff member, Board Members, and administrator in attendance, the date, time, and length of the training, and minutes documenting the information discussed during the training. Documentation of training shall be maintained in facility files for review by representatives of the Division of Child Development and Early Education upon request. ** Amanda Lovette from the Robeson County Partnership for Children conducted a 2 hour training with William Locklear, Normie Bullard and the rest of the staff on 6/27/23. 3. Within one (1) week after completing the training, the Board of Directors, shall revise the facility’s discipline policies and procedures to incorporate strategies learned in the training. The policies and procedures should describe, in detail, the steps the facility will take to ensure appropriate treatment of children at all times. The policies and procedures shall include, but not be limited to, the following: • Age-appropriate behavior management and discipline techniques for children • Acceptable and unacceptable techniques used to handle children to prevent injuries • Procedures for how administration will ensure proper discipline and behavior management techniques are being implemented by staff • Procedures for staff members to confidentially report, without fear of reprisal, their observations or suspicions of coworkers’ inappropriate discipline, care, or treatment of children directly to the Board of Directors and to the administrator • Procedures for staff to notify Board of Directors and administrators of concerns in a timely manner, including when Board members and administrators are off premises • Procedures for how the facility’s administrators are to respond to all reports of inappropriate discipline, care, or treatment of children and the consequences mandated by the Board of Directors for noncompliance • Consequences for staff members who fail to comply with the facility’s policies and/or child care requirements • A plan for regular review of the discipline and mandatory reporting policies and procedures with all staff members including the review of the policies and procedures in the orientation of new staff members before they assume child care responsibilities. The written policies and procedures shall be submitted to Deanna Abraham, Child Care Consultant, PO Box 265, Raeford, NC 28376, telephone number 704-213-6714, email deanna.abraham@dhhs.nc.gov, for approval.
