Mrs. Angelia McGeachy, owner/administrator, was present and assisted me during today’s visit. The last annual compliance visit was conducted on August 19, 2025. You operate with a 4-star permit issued October 5, 2023. License restrictions include Daytime and Overnight, meets enhanced ratios, children under 2 1/2 years old in rooms with direct exits only. I was unaccompanied during the walk-through of the indoor and outdoor areas. Staff/child ratio, group size, supervision, use of licensed space, space capacity, license restrictions, and the outdoor environment were monitored. The license and emergency care plan were posted. Fire drills, medication administration, and storage of hazardous products were also monitored. I observed children participating in teacher-directed activities, free choice play in activity areas, and daily routines. REQUIRED INSPECTIONS Your last sanitation inspection was dated October 29, 2025. You received 28 demerits and a Provisional classification. A violation was cited on 8/19/2025 for having a delinquent sanitation inspection. The violation is still pending correction. The Environmental Health Inspector has been made aware of the pending violation Your last fire inspection was completed on July 14, 2025. The Emergency Drill Log was reviewed during today’s visit. Two violations of child care requirements were observed and documented during today's visit. Each violation was thoroughly reviewed with Mrs. McGeachy. Violation Number Comment Rule 714 Openings in equipment, steps, decks, handrails, and fencing were not less than 3 1/2 inches or greater than 9 inches. Several post along the exit ramp to the playground are broken away from the railing. .0605(g) 807 A safe indoor and outdoor environment was not provided for the children. The following was observed on the outdoor play environment: 1)bolts were exposed on the railing of the metal shade structure, 2)nails were exposed on two of the broken post along the exit ramp, 3)a board on the picnic table top is splintered and nails are exposed. 10A NCAC 09 .0601(a) The violations documented must be corrected immediately. On or before April 30, 2026, I must receive a compliance letter that describes accurately and in detail, how and when the violations were corrected. Therefore, it is important for you to be knowledgeable of all the licensing requirements that apply to your type of program. It is your responsibility to maintain compliance with all the applicable laws and rules at all times. The best way for you to make sure that you are meeting all requirements is to periodically review the child care law and rules. Make sure you visit the Division of Child Development's website (www.ncchildcare.nc.gov) on a regular basis to find out what's new in childcare. You can also find any needed Child Care forms by going on the web site and clicking on the "Provider" tab and then clicking on the "Provider Documents" tab. You are required to maintain compliance with all applicable child care rules and regulations at all times. NC GS 110- 90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past 18 months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. Technical assistance with the outdoor learning environment was offered during the visit. To support the safety and maintenance of the outdoor play environment, it is recommended that steps be taken to control carpenter bee activity. They can create holes in wooden structures, which may lead to damage and potential safety concerns over time. You may want to consider implementing preventive measures such as regularly inspecting wooden surfaces, sealing or painting exposed wood, and using appropriate deterrents or treatments as needed. I also discussed with you ways you can enhance the outdoor play area with a variety of clearly defined center-based activity areas that can promote engagement, and support different types of play. Example: Art Area- set up a small table or easel with washable paints, chalk, or crayons. Clip paper to the fence or provide a chalkboard surface for drawing; Building/Construction area- add bins with large blocks, crates, cardboard boxes, or logs for stacking and building. QRIS Pathways to the STARS: You attended the Director's Meeting held by DCDEE on September 30, 2025 during which the new child care requirements in section .3200 pertaining to two through five stars were reviewed. You stated today that you are planning to apply for a new rated license and that your goal is to maintain a 4 and/or strive for a 5. I reviewed the pathways with you, and you stated that you felt like the facility would select Pathway 2-Classroom and Instructional Quality. I completed the QRIS Conversation Template during today's visit and you were given a copy. We briefly reviewed the requirements for Family and Community Engagement and Continuous Quality Improvement (CQI) for the facility and each individual staff member. These forms can all be found on the DCDEE website, linked from the Application for a Rated License located under the Provider Document tab. I encouraged you to become familiar with these forms and to share the individual CQI form with the staff members. We discussed the curriculum and formative assessment requirements. You will need to incorporate the curriculum for all ages of children you serve. I recommend that you have all staff members review their information in the WORKS database to determine if the information is up to date. If not, they will need to submit new official transcripts to the Workforce Unit. Please have them contact DCDEE WORKS unit directly for questions pertaining to their WORKS account. As we discussed, the goal will be for you to apply for a new rated license between today's visit and September 30, 2026. A sanitation inspection was completed on July 26, 2024, with 9 demerits and a Superior rating. A fire inspection was completed on July 14, 2025. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The facility received a fire inspection on July 14, 2025 and did not submit the original approved report to DCDEE within one week of the inspection visit. The program operates with a 4-star rated license issued October 5, 2023, earning 7 points for education standards, 2 points for program standards, and one quality point for complying with a programmatic option. License restrictions include: 1st, 2nd, and 3rd shift, meets enhanced ratios, children under 2 1/2 years old in rooms with direct exits only. Today's visit was conducted with Mrs. Angelia McGeachy, owner/director. The indoor and outdoor areas were monitored. Staff/child ratios, group size, capacity, use of space, and supervision were observed. I observed children playing in various activity centers, teacher lead activities, and eating lunch. Developmentally age-appropriate materials were used. Positive and nurturing communication occurred as teachers interacted with children. A fire inspection was completed on July 24, 2024. The program is approved for daytime care only. The Emergency Drill log was reviewed during today’s visit. During today’s visit, six children records were reviewed. The children’s records form was completed during the visit. Three violations of child care requirements were observed and documented during today's visit. Each were thoroughly reviewed with Mrs. McGeachy. The allegation summary included: There is a concern that staff transported a child from elementary school to the center that was not enrolled at the center. The child got in the vehicle by mistake. There is concern that procedures to account for each child were not followed or were inadequate. I reviewed the allegation summary with the director/owner, Angelia McGeachy and one staff. Both were given an opportunity to respond to the concerns. A walk-through of the facility was conducted, as well as the vehicle used for transporting was monitored. Limited monitoring of child care requirements occurred during today’s visit. Findings to the allegation: The van driver stated that on March 20, 2024, a child, who was no longer enrolled at the center, got on the van. She stated that the school released the child with her group of children and that she told the child "you are not supposed to be on this van". I verified the sign in log during today's visit as well as observed van drivers documenting that each child on the roster was signed in at the center. The program has safe arrival and departure procedures in place that are posted in the center. Upon the van arriving back at the center with the school-age children, the driver informed me that the school still has the child listed on Angel's Childcare & Learning Center, LLC pick up list. Ms. McGeachy tried to contact the mother letting her know she needed to inform the school of the change. Ms. McGeachy stated she has told the parent since his withdrawal that she needed to let the school know of which van he was to be released to. During today's visit, the child's mother informed Ms. McGeachy that she has completed the required paperwork at the school showing where the child is to be transported and that she has informed the child's teacher, as well. Ms. McGeachy stated she conducted a mandatory staff meeting that was held on March 23, 2024. She stated the following was covered during the meeting: General child care requirements, sanitation requirements and transportation procedures. Since the child was put on the van and brought to the center and the child was not on the roster to be transported, the complaint allegation is substantiated. Three violations of child care requirements were observed and documented. Computer-generated report was printed at the conclusion of today's visit. Violation Number Comment Rule 104 Center has not passed required sanitation inspection and received an approved or superior rating. The last sanitation inspection was completed on October 21, 2022. 10A NCAC 09 .0304(b) 1124 Emergency and identifying information, including the child's name, photograph, emergency contact information and/or a copy of the emergency medical care information form was not in the vehicle for each child being transported. Although the child was not enrolled in the center, the child was transported to the center and the emergency and identifying information was not in the vehicle. 10A NCAC 09 .1003(d) 1128 For routine transport of children to and from the center, staff did not use the list to document attendance as children boarded and departed the vehicle. Although the child was not currently enrolled at the center, the child was transported from school to the center and the child was not added to the attendance roster as he boarded and departed the vehicle. 10A NCAC 09 .1003(l) The above violations must be corrected immediately.
