Ms. Cozart assisted me during the visit. Your program currently operates with a Five-Star issued June 3rd, 2022. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety eight percent (98%) as of 2/6/26. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The NC Secretary of State website was reviewed on (2/6/26) and FRANKLIN VANCE WARREN OPPORTUNITY INC was listed as current- active. A full assessment and walk-through of the facility was completed today. The following was observed: license posted, and restrictions followed, summary of law posted, attendance taken daily, emergency medical care plan posted, activity plans posted, daily schedule posted, menus posted, allergies posted, storage of hazardous materials and substances, CPR/First Aid and training completed as required, and Criminal Record Checks completed as required. I also observed children in the indoor learning environment and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in morning snack, group time, centers, transitions, and personal care routines. The caregivers were interacting and meeting the developmental needs for each of the children. I reviewed all staff files during today’s visit. A sampling of Children’s files (ten percent of enrollment) was reviewed today. Emergency drill log: Shelter and lock down drills on 1/16/26. Fire drill: 1/9/26. Fire Inspection: 3/3/25. Sanitation Inspection: 1/6/26. The last documented playground checklist was on 2/4/26. A checklist was used to note the requirements I monitored today. Two violations were cited during today’s visit. Violation Number Comment Rule 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. K. Hunt did not take First Aid within 90 days of hire. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. K. Hunt did not complete CPR within 90 days of hire. .1102(d) The violation(s) documented must be corrected immediately. On or before 2-20-26, I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected. All staff who provide direct care or accompany children when they are off premises shall successfully complete certification in a cardiopulmonary resuscitation (CPR) and First Aid course appropriate to the ages of children in care. At all times when children are in care one staff member present must have successfully completed CPR and First Aid training. The training shall be completed by June 30, 2018, or for new staff hired on or after September 1, 2017, training must be completed within 90 days of employment. Distance learning shall not be permitted for CPR and First Aid training. CPR and First Aid training shall be renewed on or before the expiration of the certification. Verification of each staff member's completion of this course from an approved training organization shall be maintained in the staff member's file in the center. The Division shall post a list of approved training organizations on its website at http://ncchildcare.nc.gov/providers/pv_sn2_ov_pd.asp. Rated License Information The facility is due to have the Environmental Rating Scale Assessment completed by September 2026. This facility is currently operating with a Five -star center license with an effective date of 6/8/18. The last Environmental Rating Scale Assessment which was completed on 5/8/18. The overall score for this facility was 5.38. Once I receive the correction letter and the application from Ms. Floyd, I will submit the Application for an Assessment for a Two Component Star Rated License. Ensure that all education information is current in the W.O.R.K.S. database. Education evaluations can take some time to process so beginning the process early will help to ensure we have the accurate information needed when it is time to complete the rated license assessment. The children in care were observed in circle time, centers, and washing hands. Currently this facility operates with a Five (5) Star license that was issued June 3rd, 2022. The fire inspection was completed 3/3/25. The sanitation inspection was conducted on 2/17/25. The last fire drill conducted was 9/11/25. The last playground inspection conducted was 8/15/25. The next one is due by 9/30/25. The last shelter and lockdown drill was 7/19/25. Next one due by October 31st, 2025. During today’s visit, partial assessment of applicable Child Care Requirements was conducted. The following items were observed in compliance during today's visit: license posted, and restrictions followed, summary of law posted, supervision, staff: child ratio within compliance, First Aid/CPR training current. Daily schedules posted, activity plans posted, menus posted, allergies posted, and criminal record checks completed, Storage of hazardous materials were in compliance. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. An example is: Jane Doe, Administrator AAA Child Care ID # 12345678 Technical Assistance 10A NCAC 09 .0605 OUTDOOR LEARNING ENVIRONMENT IN CHILD CARE CENTERS (g) Any openings in equipment, steps, decks, handrails, and fencing shall be smaller than 3 ½ inches or greater than 9 inches to prevent entrapment. (h) All upright angles shall be greater than 55 degrees to prevent entrapment and entanglement. § 110-90.2. Mandatory childcare providers' criminal history checks. (b) Effective January 1, 1996, the Department shall ensure that, prior to employment and every three years thereafter, the criminal history of all childcare providers is checked and a determination is made of the childcare provider's fitness to have responsibility for the safety and well-being of children based on the criminal history. The Department shall ensure that all childcare providers are checked for county, State, and federal criminal histories. § 110-91. Mandatory standards for a license. (9) Records. – Each childcare facility shall keep accurate records on each child receiving care in the childcare facility and on each staff member or other person delegated responsibility for the care of children in accordance with a form furnished or approved by the Commission, and shall submit records as required by the Department. All records of any childcare facility, except financial records, shall be available for review by the Secretary or by duly authorized representatives of the Department or a cooperating agency who shall be designated by the Secretary and shall be submitted as required by the Department.
