Ms. T. Wilkerson, Director, assisted me during the visit. Currently this facility operates with a Four (4) Star license that was issued December 2nd, 2025. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety two percent (89%) as of 6/8/26. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. During today’s visit, a partial assessment of applicable Child Care Requirements was conducted. The following items were observed in compliance during today's visit: license posted, and restrictions followed, summary of law posted, supervision, staff: child ratio within compliance, supervision, First Aid/CPR/ITS-SIDS training current. Storage of hazardous materials was in compliance. The sanitation inspection was conducted 5/27/26. The fire inspection was completed on 10/31/25. The last fire drill was conducted on 5/11/26. The last outdoor inspection conducted was 5/8/26. The last shelter and lock down drill were conducted on 3/19/26. Next one due by June 30th, 2026. I monitored 2 summer employees files during today’s visit. Transportation Vehicle safety Inspection and Liability Insurance was current for both vans. One violation was cited during today’s visit and corrected. Technical assistance was given to the provider to ensure compliance. No correction letter is needed. The center currently operates with a Four (4) Star License that was issued on 10-14-2019 with 4 points in Staff education,5 points in Program standards and 1 quality point. During today’s visit, a full assessment of applicable Child Care Requirements was conducted. Children were observed participating in the following activities: indoor play, lunch and preparations for nap time. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety one percent (91%) as of 12/1/25. I monitored both transportation vehicles and the inspections, insurance and transportation requirements were current for both. I reviewed all staff files and (10 percent of enrollment) children's files. The last sanitation inspection was on 10/31/25. The last fire inspection was on 10/31/25. The last fir drill was on 11/26/25. The last outdoor inspection was on 11/17/25. The last shelter and lock down drill were conducted on 9/29/25. The following violations were observed, documented and technical assistance regarding maintaining compliance was offered. Two staff members did not complete the required number of hours, on was due by August 2025, and the other was by September 2025. .1103(a) 1882 Medication authorization, giving the caregiver standing authorization did not meet the specifications in rule. Medication authorization form did not have the name of the cream, the amount to apply or the expiration date. .0803(6)(a-i); .0803(7)(a-g); .0803(8)(a-d) The violation(s) documented must be corrected immediately. On or before 12-15-25, I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected. Any medication the parent fails to retrieve within 72 hours of completion of treatment, or withdrawal of authorization, shall be discarded. A parent may give a caregiver standing authorization for up to 12 months to apply over-the-counter, topical ointments, topical teething ointment or gel, insect repellents, lotions, creams, fluoridated toothpaste, and powders, such as sunscreen, diapering creams, baby lotion, and baby powder, to a child, when needed. The authorization shall be in writing and shall contain: (a) the child's name; (b) the names of the authorized ointments, repellents, lotions, creams, fluoridated toothpaste, and powders; (c) the criteria for the administration of the ointments, repellents, lotions, creams, fluoridated toothpaste, and powders; (d) the manner in which the ointments, repellents, lotions, creams, fluoridated toothpaste, and powders shall be applied; (e) the signature of the parent; (f) the date the authorization was signed by the parent; and (g) the length of time the authorization is valid, if less than 12 months. Protective surfacing shall cover the area under and around equipment where a child may fall, referred to as the fall zone. Equipment and furnishings shall be sturdy, stable, and free of hazards that may injure children including sharp edges, lead based or peeling paint, rust, loose nails, splinters, protrusions (excluding nuts and bolts on sides of fences), and pinch and crush points. The temperature in the room where infants aged 12 months or younger are sleeping does not exceed 75° F; When screen time is provided on any electronic media device with a visual display, it shall be: (A) offered to stimulate a developmental domain in accordance with the North Carolina Foundations for Early Learning and Development as referenced in Rule .0508 of this Section; (B) limited to 30 minutes per day and no more than a total of two and a half hours per week, per child; and (C) documented on a cumulative log or the activity plan that shall be available for review by the Division. RATED LICENSE ASSESSMENT STATUS The Environmental Rating Scale Assessment which was completed on 8/26/2025 and ,9/2/2025 as part of the three-year reassessment process for this facility. I did a visit on 9/19/25 to review the results. This facility is currently operating with a four-star center license with an effective date of 10/14/19. By meeting all enhanced requirements, the program is eligible for six points in Program Standards, four points in Education Standards with the Administrator at the 5-point level, lead teacher at the four-point level, teachers at the four-point level, group leader at the 7-point level and program coordinator at the five-point level. It appears that you should be eligible for eleven (11) points for a Four-Star Child Care License. I will submit your rated license information to the Division of Child Development and Early Education, who will make the final decision. Each infant's plan shall be modified in consultation with the child's parent or health care provider to reflect changes in the child's needs as he or she develops. The director and I also discussed making sure the teacher’s fill out the teacher section on the feeding schedules and ensuring all bottles have the correct date on them. On or before 8-3-23, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. If it is determined the information provided in the letter is not true, this may be considered falsification of information. The training shall be completed by June 30, 2018, or for new staff hired on or after September 1, 2017, training must be completed within 90 days of employment. Distance learning shall not be permitted for First Aid training. At all times when children are in care at least one staff member present must have successfully completed First Aid training, as evidenced by a certificate or card from an approved training organization. First Aid training shall be renewed on or before expiration of the certification. "Successfully completed" is defined as demonstrating competency, as evaluated by the instructor. Verification of each required staff member's completion of this course from an approved training organization shall be maintained in the staff member's file in the center. The Division shall post a list of approved training organizations on its website at http://ncchildcare.nc.gov/providers/pv_sn2_ov_pd.asp. (d) All staff who provide direct care or accompany children when they are off premises shall successfully complete certification in a cardiopulmonary resuscitation (CPR) course appropriate to the ages of children in care. At all times when children are in care one staff member present must have successfully completed CPR training. The training shall be completed by June 30, 2018 or for new staff hired on or after September 1, 2017 training must be completed within 90 days of employment. Distance learning shall not be permitted for CPR training. CPR training shall be renewed on or before the expiration of the certification. Verification of each staff member's completion of this course from an approved training organization shall be maintained in the staff member's file in the center. The Division shall post a list of approved training organizations on its website at http://ncchildcare.nc.gov/providers/pv_sn2_ov_pd.asp. (f) In centers that are licensed to care for infants, the child care administrator and any child care provider scheduled to work in the infant room shall complete ITS-SIDS training. ITS-SIDS training shall be completed within two months of an individual assuming responsibilities in the infant room and every three years thereafter. Child care administrators, as defined in G.S. 110-86(2a), shall complete ITS-SIDS training within 90 days of employment and every three years thereafter. Completion of ITS-SIDS training shall be included once every three years in the number of hours needed to meet on-going training requirements in this Section. At all times, one child care provider who has completed ITS-SIDS training shall be present in the infant room while children are in care. A certificate of each staff member's completion of this course shall be maintained in the staff member's file in the center.
