The visit was conducted with you, Kayla Woody, owner/operator. This program currently operates with a Four-star license issued on April 29, 2022. The permit restrictions include 1st shift, a maximum of 5 preschool children at one time, no more than 2 children under one year of age, and trampoline may not be used during operating hours. The last annual compliance visit was conducted January 5, 2026. The most recent monthly fire drill was conducted on June 1, 2026 at 10:10am. The most recent quarterly shelter-in-place drill was conducted on June 1, 2026 at 11:00am. The most recent monthly outdoor play area inspection was completed on May 1, 2026. The center's compliance history was reviewed with the operator. The program’s compliance history was 98 percent as of June 8, 2026. A walk-through of the home was completed today, all indoor and outdoor areas were monitored. I observed children in the indoor learning environment and found supervision and staff/child ratios to be in compliance. Children were participating in screen time and free play activities indoors. The analysis date for the most recent lead water test was April 20, 2022 We discussed that you need to follow-up with the RTI program through the Clean Water for US Kids program to complete the remediation tasks for both lead-based paint and asbestos hazards. You reported that you have made multiple calls and are awaiting a returned call from the RTI program. I encouraged you to contact the program again, if you have not heard from the program within a week of your call. You provided me with applicable program and staff files for review. Limited monitoring of staff files was conducted for the purpose of reviewing training regarding CPR/First Aid, EPR, Playground Safety, and Special Training. The following violations were observed: Violation Number Comment Rule 543 Screen time was not prohibited for children under the age of three. The operator did not offer alternative activities for children under the age of three years. Screen time was offered to two children two years between 9:25am-10:05am. .1718(c) 801 Bacteriological analysis indicating the water is free of bacteria had not been performed on a sample of water from each well used for the children's water supply every two years. Results were not kept on file in the home. The last bacteriological analysis results on file were dated February 26, 2024. 10A NCAC 09 .1725(a)(1) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. A bacteriological analysis indicating the water is free of bacteria must be performed on a sample of water from each well used for the children's water supply every two years. Results must be kept on file in the home. I suggested that you contact the Health Department immediately to get information about how to submit your water sample. This task must be completed within the next two weeks. Consultation or other topics discussed today: You reported that you have decided not to apply for a rated license. I explained that programs that are due for a rated license reassessment must apply for a rated license by September 2026. If you do not wish to apply, you will be choosing a reduction in stars to the one-star level. You, Kayla Woody, owner/provider, assisted with the visit. Currently this center operates with a four star license that was issued on April 29, 2022. Restrictions were in compliance. A checklist was used to note the requirements monitored today. You use your living room, dining room, kitchen, bathroom and two bedrooms to care for children. When I arrived, there were three (3) preschool age children present. You and the children present were outdoors having gross motor play. One child left shortly after my arrival. The children in care were also observed during hand washing/ bathroom routines, and nap. Fire: A fire drill was conducted on February 2, 2024. EPR drills had been conducted and recorded. The fire extinguisher was fully charged and is located in a lower kitchen cabinet. Medications were in compliance. Children’s records were reviewed during today’s visit with no violations. Your records and a substitute’s records were reviewed with no violations. One violation was cited. Due to a connectivity issue the visit summary could not be computer generated at the time of the visit. A handwritten summary was signed during the visit.
