The visit was conducted with you, Donna Byrd and Rebecca Shuford, Lead Teachers. This program currently operates with a five star license issued on June 21, 2022 The permit restrictions include daytime care only, enhanced ratios, enhanced space, and enhanced ratios reduced by 1 in each group. Your program was also monitored for compliance with implementing an approved curriculum as required for all four and five star licensed facilities where four-year-old children are enrolled. You stated you use Creative Curriculum. The last annual compliance visit was conducted 9/2/25. The most recent sanitation inspection for your facility was conducted on 9/3/25. A superior sanitation classification was issued with 2 demerits noted on the grade card. The most recent approved fire inspection for your facility was conducted on 7/2/25 for daytime care only. The most recent monthly fire drill was conducted on 2/25/26. The most recent quarterly lockdown/shelter-in-place drill was conducted on 2/9/26. The facilities EPR Plan was reviewed on 8/22/25. The most recent monthly playground inspection was completed on 2/4/26. Two classrooms were visited and monitored and the outdoor gross motor area. I observed children in both the indoor and outdoor learning environments and found supervision and staff/child ratios to be in compliance. Children present today were participating in free play activities indoors, handwashing routines, lunch, and outdoor gross motor activities. A checklist was used to note the requirements I monitored today. The analysis date for the most recent lead water test was 5/24, 2024. Lead testing must be completed every three years. This website also indicated “Survey Review RTI" for required asbestos and lead-based paint testing. I suggested you, or the individual responsible, contact RTI to request an update on this facilities enrollment with required asbestos and lead based paint testing. You provided me with applicable program, staff, and children’s records for review. There were no new staff. Two existing staff, and applicable records for all staff were reviewed. All staff were listed on the ABCMS Provider Portal roster for this facility. A sample of four children’s records were reviewed. NC Pre-K requirements were monitored today. Medication was in compliance. One violation was documented and reviewed with you during the visit. Violation Number Comment Rule 1873 The Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy was not reviewed with existing staff who care for children ages 0-5 within 30 days of adopting the policy. One substitue staff, who worked on 12/2/25, did not have a signed statement regarding the Prevention of Shaken Baby and Abusive Head Trauma policy review on file. .0608 (d) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation documented must be corrected immediately. On or before April 6, 2026, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violation was corrected. Please be aware any information submitted by you is legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted. If you email the compliance letter, it must be sent from the email address registered with the DCDEE (this serves as your signature) and the following information must be included: name, position, facility name, and facility ID number. An example is: Jane Doe, Administrator AAA Child Care ID # 12345678 The center's compliance history was reviewed with the operator. The program’s compliance history was 100 percent as of 3/23/26. Technical Assistance was provided on the following: All staff members who care for children up to five (5) years of age must review the facility’s Prevention of Shaken Baby and Abusive Head Trauma Policy prior to the staff member caring for children. An acknowledgment of receipt and review must be on file. The acknowledgement of this review shall contain: (1) the individual’s name; (2) the date the center’s policy was given and explained to the individual; (3) the individual’s signature; and (4) the date the individual signed the acknowledgment. You stated the policy will be reviewed with each new staff member and the acknowledgement placed in each staff member’s file prior to caring for children. TA- We discussed reviewing the policy with each new staff member and all substitute staff . The acknowledgement signed immediately and then placed in each staff member’s file prior to caring for children. Consultation: The ABCMS provider portal facility roster should be updated in ABCMS on an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. Upon arrival, the purpose of today’s visit was reviewed with Lead Teachers, Rebecca Shuford and Donna Byrd. Health and Safety training was reviewed. A substitute was working in space #1. Your program currently operates with a five star license effective June 21, 2022. The permit restrictions were in compliance. Two classrooms and the outdoor gross motor area were monitored. Children in care were observed during indoor and outdoor free play activities, routine care activities, snack, and lunch. Supervision and staff/child ratios were in compliance. Positive interaction was observed. A sanitation inspection was conducted by Environmental Health on . November 8, 2023 with 2 demerits noted. The most recent fire inspection for your facility was conducted on October 2, 2023. The facility was approved for day time care. You provided me with a copy of the form. The latest fire drill was conducted on December 13, 2023. Staff and children’s records were reviewed using DPI verification forms with no violations. This facility uses the school Critical Management Plan. N C Pre-K requirements were in compliance. None of the enrolled children had medication. One violation was documented during today’s visit. Due to a connectivity issue (Regulatory issue) the summary was not computer generated. A handwritten summary was signed during the visit. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. A fire inspection was conducted on October 2, 2023. The previous inspection was conducted on August 2, 2022. This is past the 12 month timeframe. 10A NCAC 09 .0304(a) Compliance Verification: The above uncorrected violations must be brought into compliance immediately. You shall submit a written, signed and dated statement or email to me at the address below detailing specifically how the above violation was corrected. The violation must be corrected no later than February 6, 2024. Include the facility name and ID#. Failure to correct the violations and send the written statement by the due date listed above, noncompliance, may lead to an administrative action. I suggested the principal make arrangements for a new inspection by contacting the Fire Inspector at least 30 days in advance of the next due date. Consultation or other topics discussed today (the following items reviewed were in compliance today): -We discussed Action Plans. Be sure these forms and the medication associated with them are up to date. You currently do not have any children with medication. -I reminded you to recheck the incident report to ensure that all required information has been completed. -We discussed substitutes. Health & Safety training must be completed after the 10th day as substitute work.
