This facility has a five-star rated license and the compliance history percentage prior to this visit was 99%. Upon arrival, I was greeted by Ms. Chassiny Jackson, Curriculum Coach. I shared the reason for the visit. Ms. Jackson assisted me with today’s visit. Ms. Kiksheal Shuford, Program Coordinator arrived soon after and assisted me with the visit as well. The following were monitored during this visit: supervision, staff/child ratio, safety, adequate/approved space, program records, new staff records, license posted, permit restrictions, discipline and storage of hazardous products and medications. One hundred and two (102) children were present during the visit. The children were engaged in outdoor activities on the playgrounds, transitioning to lunch, handwashing procedures, diaper changing, lunch and the individual needs of the infants and young toddlers were being met by the teachers. The last sanitation inspection was on 3/10/25 with zero (0) demerits and a superior classification. The last fire inspection was conducted on 11/19/24. The site monitoring tool for the one (1) NC Pre-K classroom was monitored during today’s visit. The classroom was monitored by the site administrator on 10-24-24. An onsite visit was made by the NC Pre-K Coordinator on 12-3-24. Medication is administered at this facility. I monitored the storage of the diaper creams. There were nine (9) new staff members hired since the facility’s last annual compliance visit on 9/6/24. The files were monitored for compliance with NC Childcare Rules. I reviewed the staff and training worksheets and files for the new staff members. Any notes or changes made by the child care consultant were initialed on the staff and training worksheets. The violations cited during the visit were discussed with Ms. Shuford and documented in the visit summary. The following violations were observed during the visit: Violation Number Comment Rule 721 All equipment and furnishings were not in good repair. On the toddler playground, a toy kitchen had a broken plastic toy drain. G.S. 110-91(6); .0601(b) 1035 Child care providers, including the director, uncompensated providers, substitute providers, and volunteers did not have the required Emergency Information Form on file on or before the first day of work, which included all the required information and/or the information on the form was not updated as changes occur and at least annually. Staff member, C.P. completed the emergency information form after the first day of employment. .0701(a) All violations documented above must be corrected immediately. A letter needs to be sent to me stating how each violation was corrected and how compliance will be maintained in the future. Please include the violation number, statement of compliance, license ID and your signature in the letter. Your compliance letter must be received, via email by April 24, 2025. Upon arrival I was greeted by Ms. S. Bright-Muniz and I shared the reason for the visit. She and Ms. Honeycutt accompanied me throughout the walk-through of the facility and the outdoor learning environments. Your program currently operates with a Five (5) Star Center License. The license was posted, with restrictions to first shift (1st) care, meets enhanced ratios, and meets enhanced space. Ownership: The facility owner is Quality Child Care Center, INC with SoSID #: 0327012 and it is current and active. If any changes to the ownership need to be made, you must notify your consultant at least thirty (30) days prior to any changes occurring. The license cannot be bought, sold, subleased, transferred to another person or location or inherited. I used the Child Care Center Item Number Listing as a basic monitoring tool to assess compliance with all applicable childcare requirements pertinent to this facility and the Annual Compliance Monitoring Checklist for Child Care Centers. I monitored the following items today: supervision, staff/child ratio, staff, health, safety, and program records. All classrooms, four playgrounds, and the kitchen were monitored. Program Records: We reviewed all the required records and were in compliance. Inspections: All inspections were monitored, and inspection dates are listed below. •Fire drill was conducted on 8/29/24. •Emergency drill was conducted on 6/28/24. •Fire inspection was completed on 11/20/23. •Playground inspection was completed on 8/15/24. •Sanitation inspection was conducted on 3/4/24 and received a Superior classification. •EPR manual was updated and completed •Incident logs and sheets were completed and up to date. Indoor Learning Environment: I observed children in the indoor learning environment and the following items were posted in the classrooms that were monitored: Daily schedules, activity plans, staff/child ratio worksheets, emergency numbers, emergency procedures, arrival/departure procedures, summary of law and menus. The children in the rooms that serves infants were participating in outdoor activities. The children in the rooms that serves toddlers and two (2) year old's were eating lunch and transitioning to rest time. The children in the room that serves the NC Pre K program were participating in self-directed free choice center time. The rooms that serves children ages three (3) and four (4) were participating in teacher lead activities and free choice activities on the outdoor learning centers. The NC Pre K classroom was in compliance with developmental screenings, using the Brigance and also use NC Foundations in conjunction with The Creative Curriculum. They facility reported they hosted several open houses for families in the months of July and August for the August 12th, 2024 school year start date. The facility also reported they have measures in place for children who miss school and they follow up with absent children on a daily basis if needed. Outdoor Learning Environment: The outdoor learning environments were monitored and in compliance. On the playground that serves infants and new walkers, there are some roots that are starting to be exposed and could be a tripping hazard. We discussed raking more mulch over the roots or marking them so children were less likely to trip and fall. Staff Records: The staff-training worksheet was completed prior to the visit. There were five (5) new staff files to review today, and six (6) existing staff file. Please refer to the staff/training worksheet to review which files were monitored. Children’s Records: There are one hundred and sixteen (116) children enrolled in this center. There were one hundred and two (102) in attendance today. Thirteen (13) children’s files were reviewed today, please refer to the children’s worksheet to review which files was monitored. We discussed NC Pre K vision and hearing screenings and what to do if the child is uncooperative or the provider cannot complete the assessment. Medication: All emergency and other medications were reviewed today and were in compliance. Nutrition: The facility was in compliance during today’s visit with Child Care Meal Patterns Requirements. The menu and allergy list were posted and the children were having cheese pizza, whole wheat bread, corn or kidney beans, salad and milk or milk alternatives for lunch. Weapons: Your facility reported that they were in compliance during today’s visit with Child Care Requirements regarding firearms. Transportation: The facility reported the does not provide transportation. One (1) violation was observed today and it was corrected during the visit. The violation is as follows: Violation Number Comment Rule 1043 All staff records, except financial records, were not made available for review. Staff member B.E.'s health questionnaire and TB test readings were not on file for review. G.S. 110-91( 9) Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent (75%). Any violation (s) documented may impact the compliance history score. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-nine percent (99%) prior to today’s visit. The compliance history could be impacted after today’s visit. Based on Child Care Rule 10A NCAC 09 Section .2200, the Division of Child Development and Early Education may take administrative action against the license and/or impose civil penalties based on the failure of the operator to correct any documented violations within the established time period. If you state in your letter that corrections have been made when they have not, it will be considered falsification of information. Rated License: •Hold Harmless has been extended until the new QRIS is implemented (SB 425). Providers in Cohort #1 (number one) are not required to go any further with a rated license assessment unless they want to, and providers in Cohort #2 (number two) do not need to start their preparation year unless they want to. Upon request, a childcare facility may be awarded a star-rated license based on accreditation from a national childhood education accreditation organization provided the facility maintains its accreditation and remains in good standing. •Use this time for staff to update their information in the WORKS database. Technical Assistance •Rule 15A NCAC 18A .2816 requires currently operating child care centers to test water initially and every three years. Family child care homes are required to test water for lead once. o DCDEE encourages you to utilize the Clean Classrooms for Children’s program. You can take the pre-enrollment webinars which is available at www.cleanwaterforUSkids.org/carolina then enroll – At www.cleanwaterforUSkids.org/carolina completing the enrollment surveys for three program sections: 1) lead in water, 2) lead-based paint, and 3) asbestos. Keep mindful that once you evaluate potential hazards and receive results if there are recommendations and/or water mitigation support is needed they will provide direction in how to mitigate and ways to fund said mitigation. • All facilities who want to be eligible for mitigation must enroll in in Clean Water for Kids by November 1st, 2024. Please enroll as soon as possible if you have not already done so. •Since the facility houses an NC Pre K, I shared DPI funding for a 2024 Safety Grant. The funding will be used for safety equipment, training and services for students in crisis. Safety equipment covered by this grant includes items such as cameras, vape detectors, radios, and weapons detection systems. Training can include professional development for school leaders to assist students who are experiencing anxiety, trauma and/or conduct problems. Services made available through this grant can include school-based mental health services. https://www.dpi.nc.gov/news/press-releases/2024/01/24/center-safer-schools-awards-35-million-school-safety-grants •I encouraged the facility to reach out to their local Alliance for Children or Child Care Resource facilities for help with challenging behaviors in the classrooms. Or assistance with children who have diagnosed needs or have an IEP (Individualized Education Plan). Therapist who work one on one with children in your facility also would be a good resource. At the completion of the visit, this visit summary was printed, reviewed, and a copy was left with you.
