This facility has a five-star license and the compliance history percentage prior to this visit was 98%. Upon arrival, I was greeted by Ms. Kiksheal Shuford , Program Director. I shared the reason for the visit. Ms. Shuford assisted me with today’s visit. Ms. Sherri Bright, Administrator arrived soon after and assisted me with today’s visit as well. The following were monitored during this visit: supervision, staff/child ratio, safety, adequate/approved space, program records, new staff records, license posted, permit restrictions, discipline and storage of hazardous products and medications. There were a total of forty (40) children present during the visit. I observed children engaged in transitioning from off-premise trip to the movies, handwashing procedures and lunch time. Medication is currently administered at this facility. The storage of emergency medication was monitored. The last sanitation inspection was on 3/10/25 with zero (0) demerits and a superior classification. The last fire inspection was conducted on 11/19/24. There were three (3) new staff members hired since the facility’s last annual compliance visit on 10/7/24. I monitored the personnel and medical files for each new staff member. A staff and training worksheet was completed by the child care consultant and a copy was included in today’s visit summary packet. The violations cited during the visit were discussed with Ms. Bright and documented in the visit summary. The following violations were observed during the visit: Violation Number Comment Rule 1032 Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. Staff member, C.R. did not have the required medical report on file. 10A NCAC 09 .0701(a) 1033 On or before the first day of work, all staff, including the director and individuals who volunteer more than once per week did not provide results indicating that they were free of active TB and/or TB test or screening was older than 12 months. Staff member, C.R. did not have the required TB test or screening on file. .0701(a) All violations documented above must be corrected immediately. A letter needs to be sent to me stating how each violation was corrected and how compliance will be maintained in the future. Please include the violation number, statement of compliance, license ID and your signature in the letter. Your compliance letter must be received, via email before or by June 12, 2025. If needed, send your letter to: Stephanie Palmer PO Box 481335 Charlotte, NC 28269 Technical Assistance/Resources: • During the visit, I reminded Ms. Bright regarding the hold harmless status for licensed facilities until the new QRIS is implemented. • During the visit, Ms. Bright asked what the requirements were for a driver who is not included in staff/child ratios and does not provide care for the children. I discussed the requirements with Ms. Bright which included the medical and TB test and referenced rule .0701 on pages 22-23 of the NC Childcare Rules.
