    Name of Operation: OAK RIDGE ELEMENTARY ACES PROGRAM Facility ID: 41001397 Consultant: CHERISSA ALSTON Operation Type: Center Case Number: Visit Date: 11/8/2023 Number Present: 104 Completed Date: 11/8/2023 Age: From 5 To 11 Total Minutes: 105 Time In: 01:00 PM Time Out: 02:45 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today’s unannounced visit was to monitor the facility for compliance with child care rules. This facility has an 18-month compliance history of 100%. Upon arrival, I was greeted by Sandra Sumner, Site Coordinator. I observed children arriving and putting away their belongings. They were also observed washing their hands in preparation for snack and eating snack. Interactions between staff and children were positive. A sanitation inspection was conducted on 9/28/23, receiving a superior classification. The most recent fire inspection was conducted on 10/13/23. Emergency drills and playground inspections were not documented for the month of October. Required items were posted and current. There were four items of non-compliance observed during today’s visit. Violation Number Comment Rule 301 Minimum staff/child ratios and group sizes for the number and ages of children in care were not met. There was 1 group leader with 29 children which is more than the 1:25 maximum group size. GS 110-91(7);.0713(a-d) 805 Fire drills were not practiced monthly and/or the drill record was incomplete. A fire drill was not documented for October. .0604(t); .0302(d)(5) 844 Prescribed medicine was not in original labeled container or accompanied by signed and dated written instructions from prescribing physician or health care professional. Two epi pens were not in the original labeled container. .0803(2)(a) 859 Monthly playground inspections were not completed and/or they were not completed by an individual trained in playground safety requirements. The inspection for October was not completed. .0605(q) Violation(s) documented above must be corrected immediately. A signed and dated letter of compliance must be emailed to me by 11/22/23, describing how and when the violation(s) were corrected and how compliance will be maintained in the future. Please remember your letter should include facility name, facility ID#, and original signature. There were five groups of children today and four group leaders. When the fifth group was divided between the other four groups, there were too many children in one group, having a ratio of 1:29. I spoke with Mrs. Sumner regarding securing substitutes and who she is supposed to report to when she is short staffed. I told her she needs to have a plan in place for shortages like today and that knowingly being out of ratio is a violation of child care rules. Thank you for your time today.
