This visit was conducted with Tory Anderson, Lead teacher. Your program currently operates with a FIVE (5) Star license. Restrictions include 1st shift daytime care; meets enhanced ratios; meets enhanced space; meets one less than enhanced; School age playground cannot be used by preschoolers. These two (2) NC Pre-K classrooms are at North Wilkesboro Elementary School, a public school in Wilkes County. The license was observed, and the restrictions were found in compliance. The program’s compliance history was 95% prior to today’s visit. All programs are required to maintain at least 75% compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed staff/child interactions today. I observed children having outdoor play, having morning snack, and having free choice center play. I observed materials and activities that were available to the children in the classrooms. Your facility uses Creative Curriculum with Teaching Strategies Gold. The last annual compliance visit was conducted on 4/10/25. A sanitation inspection was completed on 8/29/25 with a Superior classification and eight (8) demerits. The last fire inspection was conducted on 10/27/25. A copy of this inspection was received during today’s visit. Program records and required postings were monitored. A fire drill was conducted on 2/27/26 at 2:45 pm with fourteen (14) seconds to evacuate thirty-two (32) children and four (4) adults by M. Steele. A shelter-in-place emergency drill was documented on 1/6/26 at 10:00 am with eight (8) seconds to shelter-in-place by T. Anderson. An outdoor inspection was documented on 2/24/26 at 12:00 noon by A. McKinney. One (1) lead teacher file, one assistant teacher file, and four (4) children’s records were monitored per DCDEE procedures today. Nutrition requirements were monitored per DCDEE procedures. Storage of hazardous items was monitored today. Hazardous items were stored in locked closets. Storage and administration of medication were monitored. Medication authorization was monitored. Today I observed two (2) Epi-pens in Space 118, both for fish allergies. Medication paperwork was complete with the medication. There is a school nurse at this site twice per week. The program does provide transportation through Wilkes County school system buses. Today I monitored the Off-Site Transportation Records Verification signed by Eric Barker on 8/25/25. I observed the Transportation Policies for Parents of children who are transported by buses in the children’s files. The NC Pre-K requirements in section .3000 of the childcare rules were monitored for compliance. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified in compliance. The current NC Pre-K staff are lead teachers Tory Anderson and A. McKinney and teacher assistants, M Steele and C. Ebey. A selection of files was monitored for complete health assessments and developmental screenings. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. Your facility uses Creative Curriculum with Teaching Strategies Gold. You stated you completed the Dial screenings in March at the Preschool screenings for all children and as new children are enrolled, they are screened at that time. The center uses the Teaching Strategies Gold instrument to document evidence of children's ongoing progress and documentation was viewed today. The checkpoint assessments are conducted three (3) times per year: at the beginning, middle and end of the school year. The first day of school was 8/28/25. The classroom operates from 7:30 a.m. to 3:05 p.m. Parent conferences are held two (2) times per year or more often as needed or requested. You stated that you use Remind App, notebooks, phone calls, and face to face communication to communicate with parents. In addition, a daily and weekly report is sent home to the parents which includes any information about the child’s day and any reminders, along with calendars and newsletters. You verified that you incorporate family engagement by parent meetings, P.T.O. meetings, family night, and four (4) parenting sessions per year. The environment rating scale score received on 9/20/18 was 5.58 points. The following violations were cited during today’s visit: Violation Number Comment Rule 714 Openings in equipment, steps, decks, handrails, and fencing were not less than 3 1/2 inches or greater than 9 inches. Today I observed, as you enter the playground area, to the left is a brown building and a green fence. The opening between the green fence and the brown building is greater than 3.5 inches. .0605(g) 1805 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. There is no record of North Wilkesboro Elementary Pre Kindergarten on the ABCMS portal as of today 3/10/26. G.S. 110-90.2 & .2703(r) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. You must correct the violations cited during today's visit immediately and send me documentation verifying compliance on or before Tuesday, March 24, 2026. This procedure is to ensure that all staff are registered in the ABCMS (Automated Background Check Management System) for the children’s safety. Refer to Childcare rule 10A NCAC 09 .2703(r). • The space between two (2) objects is not to exceed 3.5 inches, otherwise causing an entrapment hazard to the children. The space beside the brown building and the green fence on the outdoor playground, on the left side of the playground as you are entering the playground space exceeds 3.5 inches. On 1/16/26, a meeting was held at the county office with me and Allison Wingerson to review all the Pathways. Ms. Wingerson stated at that meeting that she will be discussing Pathway 1 at the summer institute and teachers will begin work on the 3-month self-study once school starts back in August 2026. The timeline would be to go through Pathway 1 in the fall of 2026 after the 3-month self-studies are complete. Today you stated that you are planning to do Pathway 1 since you are an NC Pre-K program. Beginning in April, you may start to apply for your reassessment if you are ready and any required self-studies are complete. Or you may apply early if you are already accredited and know your Pathway. • The Application for Assessment for a Rated License is now live on DCDEE’s website under Provider Documenters. You are welcome to view this form and see all the required forms for each pathway. You can access the applications below for a Center. Once the application opens, you can click on the specific forms under each pathway to see the forms you will be using for the new QRIS process. Specifically, under the Orientation tab, I would encourage all staff to complete the training for “Child Development Module.” This training module provides a basic overview about child development and is designed to help early educators support infants, toddlers, and preschool children to reach their next developmental milestone. In addition to gaining introductory knowledge about child development, early educators will learn about the many resources and professional agencies available in North Carolina to help with promoting a responsive and engaging learning environment to support all children served. Restrictions include 1st shift daytime care; meets enhanced ratios; meets enhanced space; meets one less than enhanced; School age playground cannot be used by preschoolers. These two (2) NC Pre-K classrooms are at North Wilkesboro Elementary School, a public school in Wilkes County. The license was observed, and the restrictions were found in compliance. The program’s compliance history was 95% prior to today’s visit. All programs are required to maintain at least 75% compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed staff/child interactions today. I observed children having outdoor play, having morning snack, and having free choice center play. I observed materials and activities that were available to the children in the classrooms. Your facility uses Creative Curriculum with Teaching Strategies Gold. The last annual compliance visit was conducted on 4/10/25. A sanitation inspection was completed on 8/29/25 with a Superior classification and eight (8) demerits. The last fire inspection was conducted on 10/27/25. A copy of this inspection was received during today’s visit. Program records and required postings were monitored. A fire drill was conducted on 2/27/26 at 2:45 pm with fourteen (14) seconds to evacuate thirty-two (32) children and four (4) adults by M. Steele. A shelter-in-place emergency drill was documented on 1/6/26 at 10:00 am with eight (8) seconds to shelter-in-place by T. Anderson. An outdoor inspection was documented on 2/24/26 at 12:00 noon by A. McKinney. One (1) lead teacher file, one assistant teacher file, and four (4) children’s records were monitored per DCDEE procedures today. Nutrition requirements were monitored per DCDEE procedures. Storage of hazardous items was monitored today. Hazardous items were stored in locked closets. Storage and administration of medication were monitored. Medication authorization was monitored. Today I observed two (2) Epi-pens in Space 118, both for fish allergies. Medication paperwork was complete with the medication. There is a school nurse at this site twice per week. The program does provide transportation through Wilkes County school system buses. Today I monitored the Off-Site Transportation Records Verification signed by Eric Barker on 8/25/25. I observed the Transportation Policies for Parents of children who are transported by buses in the children’s files. The NC Pre-K requirements in section .3000 of the childcare rules were monitored for compliance. Today I observed trash on the playground area, weeds and vines growing through the fence on the playground area, the large equipment needs power washing including the roof tops, the tops of the tunnel and the covered slide, as well as the two (2) playhouses. I observed weeds and vines growing through the fence and onto the playground area from the opposite side of the fence. The large playground equipment should be power washed. This includes the roof tops, the top of the tunnel, the top of the covered slide, and the playhouses. The ground cover at the yellow climber is showing through which creates a tripping hazard for the children and teachers. I observed children having free choice center play, cleaning up centers to go outside, and outside gross motor play. One (1) new staff was reported at this program. Limited monitoring of staff files was conducted for the purpose of reviewing training regarding CPR/First Aid, EPR, Playground Safety, ITS-SIDS, Special Training including Health and Safety Training and Criminal Background Checks. Storage and administration of medication were monitored. Medication authorization was monitored. I observed one (1) Epi Pen in Space 118 for a fish allergy. A fire drill was recorded 8/29/25 at 9:00 am with one (1) minute, twenty (20) seconds to evacuate thirty-two (32) children and four (4) adults by principal, L. Plain-Mamon. A lockdown and shelter in place drill for the facility were recorded at 9:00 am and 9:15 am by T. Anderson. The most recent fire inspection was on 3/18/25 by N. Hamby. The program’s compliance history was 98% prior to today’s visit. All programs are required to maintain at least 75% compliance. I observed children having free choice center play, cleaning up, and outdoor gross motor play. I observed materials and activities that were available to the children in the classrooms. The last annual compliance visit was conducted on 5/10/23. A sanitation inspection was completed 1/18/24 with a Superior classification and fourteen (14) demerits. The last fire inspection received was dated 2/2023. Program records and required postings were monitored. A fire drill was conducted on 4/15/24 at 8:05 am with 31 seconds to evacuate thirty-one (31) children and four (4) adults. A lockdown drill was documented on 4/15/24 with thirty(3) seconds to lockdown by T. Anderson. An outdoor inspection was documented on 4/5/24 with the sandbox roof noted. The sandbox roof has been noted to be repaired on the playground inspection sheets since September of 2023. This was noted in my documentation on 5/10/23 as closed by sanitation. Staff files and children’s records were monitored per DCDEE procedures. Nutrition requirements were monitored per DCDEE procedures. Storage of hazardous items was monitored today. No medications were observed nor reported. You stated that you do provide transportation am and pm for Pre-K children. During today’s visit, I observed the Public Schools Off-site Records Verification signed by Westley Wood on 8/23/23. I observed the Public-School Off-Site Records Verification for Transportation Requirements signed by Eric Barker on 8/22/23. I also observed three (3) school bus driver Responsibilities and Procedures Orientation signed by all three (3) bus drivers. I observed the Transportation Policies for Parents in children’s files who ride the buses. The NC Pre-K requirements in section .3000 of the childcare rules were monitored for compliance. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified in compliance. The current NC Pre-K staff are lead teachers, T. Anderson, and A. Taylor, and teachers, M. Steele and C Ebey. Ms. Anderson and Ms. Taylor both have a BS degree in B-K Early Childhood. A selection of children’s files was monitored for completed health assessments and developmental screenings. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. Your facility uses Creative Curriculum with Teaching Strategies Gold. These NC Pre-K classrooms are learning about insects. You stated you have completed the Dial-4 screenings for February. The center uses the Teaching Strategies Gold instrument to document evidence of children's ongoing progress and documentation was viewed today. The checkpoint assessments are conducted three (3) times per year: at the beginning, middle and end of the school year. The first day of school was 8/21/23. The classroom operates from 7:50 a.m. to 3:05p.m. Parent conferences are held two (2) times per year and more often if needed. You stated that you use Class Dojo to communicate with parents. In addition, a daily report is sent home to the parents which includes any information about the child’s day and any reminders. You verified that you incorporate family engagement by inviting parents into the classroom for special activities and allowing parents to send materials and snacks into the classroom for special projects. The last environment rating scale score received on 9/20/18 was 5.58 The following violations were cited during today’s visit: Violation Number Comment Rule 714 Openings in equipment, steps, decks, handrails, and fencing were not less than 3 1/2 inches or greater than 9 inches. At the fence gate on the lower back left corner I observed there was a 4 inch opening between the fence pole and the fence gate. I observed there was an opening at the right end of the fence between the fence and the black ground barrier that measures 4.5 inches and the opening on the right end and measures 6.5 inches on the left end and there is a drop off from the ground between the fence and the ground. .0605(g) 721 All equipment and furnishings were not in good repair. I observed the sandbox roof was damaged. On the AC visit 5/10/23 the sandbox had been closed by sanitation. The sandbox was not in use on the last RU visit in October. Today the sandbox was open for children but no repairs have been made on the sandbox roof.
