This visit was conducted by Karen Young, Childcare Consultant with Shannon Walker and Tamara Routh, NC Pre-K lead teachers. Your program currently operates with a FIVE (5) Star license. Restrictions include 1st shift, daytime care only; meets enhanced ratios; meets enhanced space; SA playground equipment cannot be used by preschool children; meets one less than enhanced standards. The license was observed, and the restrictions were found in compliance. The program’s compliance history was 95% prior to today’s visit. All programs are required to maintain at least 75% compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. On the outdoor areas, I observed no violations. Indoors, I observed outlet covers missing in both classrooms. One outlet was missing in Space 129 and three (3) outlet covers missing in Space 128. This violation was corrected during the visit. Staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed positive interactions today. I observed children having free choice center time, cleaning up, having circle time, and outside gross motor time. I observed developmentally appropriate materials and activities were available to the children in the classrooms. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. Your facility uses Creative Curriculum with Teaching Strategies Gold with developmental domains listed. These two (2) classrooms are learning about trees. The last annual compliance visit was conducted on 11/15/22. A sanitation inspection was completed 10/25/23 with a Superior classification and eleven (11) demerits. The last fire inspection was conducted 2/23/23 by N.Hamby. Program records and required postings were monitored. A fire drill was conducted on 10/30/12 at 2:47 pm with four (4) minutes, twenty-one (21) seconds to evacuate. A lockdown drill was documented on 10/23/23 at 2:40 pm with one (1) minute to lockdown. An outdoor inspection was documented on 10/23/23. Staff files and children’s records were monitored per DCDEE procedures. Two children’s files were monitored from each classroom for a total of four (4) children’s files. Three (3) staff files and one (1) substitute file were monitored today. Nutrition requirements were monitored per DCDEE procedures. Storage of hazardous items was monitored today. Storage and administration of medication were monitored. Medication authorization was monitored. There was a seizure medication in Space 128 that was not in the original box. There is a school nurse at this DPI program on Minday, Wednesday and Friday. You stated that you do provide transportation in the am and pm for the NC Pre-K children. During today’s visit, I monitored emergency information, permission to transport forms for all children being transported, and the bus log. I observed the Public-School Off-Site Verification Form signed by Westley Wood, assistant superintendent on 8/23/23. I also observed the Transportation Requirements, Bus Driver Responsibilities and Procedures Orientation for four (4) drivers, and the Transportation Policies for Parents. There is emergency information on the bus for each NC Pre-K child that is transported by Wilkes County School bus. The NC Pre-K requirements in section .3000 of the childcare rules were monitored for compliance today. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified in compliance. The current NC Pre-K staff are the lead teachers, T. Routh and S. Walker, and teachers, C. Walker, and S. Osborne. A selection of files was monitored for completed health assessments and developmental screenings. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. Your facility uses the Creative Curriculum with Teaching Strategies Gold and developmental domains. You stated the DIAL-4 screenings are conducted prior to the children entering NC Pre-K. You verified that you incorporate family engagement by sending at-home activities to families through the class Dojo app, as well as information regarding home activities connected to the current classroom themes, hold parent-teacher conferences twice yearly, email parents regularly, conduct a Parent Orientation Meeting at the beginning of the school year with individual families, host an Open House event for all families, send home folders with children’s work samples, invite parents to donate materials to and attend classroom special activities and celebrations. Doughnuts for dad, muffins for mom, and grapes for grandparents are all done annually. Pie day will be held on November 18th and parents are invited. There will also be an ice cream social at the end of the year. Parents also engage in Preschool Pyramid Model activities with their children to support their social/emotional health. The following violations were cited during today’s visit: Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. There was a power strip with three (3) outlets uncovered in Space 128. There was one outlet uncovered in Space 129. A sezuire medication in Space 128 was not in the original pharmacy container. .0803(2)(a) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. Consultation: Today we discussed that you are in Cohort 1 which means that your prep year is in process now, 7/1/23 – 6/30/24. Your reassessment year will be 7/1/24 – 6/30/25. You received an email from me on 6/28/23 giving you information on tasks and activities to complete in your prep year. Make sure all education of all employees is up to date and official transcripts have been sent to WORKS for evaluation. Teacher assistant C. Walker is in school for her Associates Degree. You completed the Environmental Rating Scale on 12/13/22 with a score of 6.43. Your packet was not processed due to a drop in teacher assistant points in the Education component. You can keep this score for three (3) years from the date of the ERS score. Today we discussed the Rated license reassessment. The teachers are waiting until Ms. C. Walker can get as much as education as possible, during this prep year, before proceeding with the rated license reassessment next year.
