This visit was conducted by Karen Young, Childcare Consultant with Renee Thompson, NC Pre-K Lead Tacher. Your program currently operates with a Five (5) star license. Restrictions include 1st shift, daytime care only; meets enhanced space; meets enhanced ratios; S/A playground equipment cannot be used by preschoolers. Meets less than 1 enhanced ratios. The license was observed, and the restrictions were found in compliance. The program’s compliance history was 96% prior to today’s visit. All programs are required to maintain at least 75% compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. One violation was cited on the outdoor playground. There is rust on the poles of the paint station. This rust will need to be removed and the poles repainted. Indoors, I observed the cardboard blocks in space 124 need to be discarded and new cardboard blocks purchased. Staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed positive interactions today. I observed children having circle time, free choice center play, and outside outdoor play. I observed developmentally appropriate materials and activities were available to the children in the classrooms. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. Your facility uses the creative curriculum with Teaching Strategies Gold. These NC Pre-K children are learning about the Beginning of the Year. The last annual compliance visit was conducted on 11/4/22. A sanitation inspection was completed 9/11/23 with a Superior classification and eight (8) demerits. The last fire inspection was conducted 5/31/23. A copy of this fire inspection was received on 5/31/23 as compliance for the violation on 5/25/23 at the routine unannounced visit. Program records and required postings were monitored. A fire drill was conducted on 9/27/23 at 2:15 pm. There was no time to evacuate documented. A shelter in place drill was documented on 9/13/23 at 9:00 am. An outdoor inspection was documented on 10/2/23 with trees needing trimming noted as well as mulch needing to be raked to bottom of slide. Fence guide pole sticking through NC pre-K fence from the other fence was noted. Ms. Thompson has submitted a work order to the Wilkes county school maintenance department for repair of the fence and the trimming of the trees. She stated the maintenance department were out at the school yesterday to access. I requested Ms. Thompson send me an email once these items have been corrected. Staff files and children’s records were monitored per DCDEE procedures. I monitored two (2) staff members files as well as one (1) new staff member’s file today. I monitored two (2) children’s files, one (1) from space 124, and one (1) from space 125. There were no violations on staff or children’s files today. Nutrition requirements were monitored per DCDEE procedures. Storage of hazardous items was monitored today. Storage and administration of medication were monitored. The teachers reported no allergies and no medications in either classroom. It was reported that a child in space 124 has asthma, but the parent has not brought any medication to the school. The school nurse and teacher have called to remind parents to bring in any medication needed for asthma, but none has been brought in to this point. You stated that you do provide transportation at the am and pm for NC Pre-K children. During today’s visit, I monitored emergency information, permission to transport forms for all children being transported. I observed the Public-School Off-Site verification form signed by Westley Wood on 8/23/23. I observed the Transportation Policies for Parents, signed 8/23/23, by Eric Barker, Director of Transportation of Wilkes County Schools. The following violations were cited during today’s visit: Violation Number Comment Rule 721 All equipment and furnishings were not in good repair. The paint station poles on the playground were showing spots of rust. The poles should be sanded and repainted. The cardoard blocks in space 124 were coming apart and damaged. These damaged blocks should be replaced with new cardboard blocks. G.S. 110-91(6); .0601(b) 805 Fire drills were not practiced monthly and/or the drill record was incomplete. The fire drills practiced for 8/30/23 and for 9/27/23 did not have documentation of the time required to evacuate. .0604(t); .0302(d)(5) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. You must correct the violations cited during today's visit immediately and send me documentation verifying compliance on or before Tuesday, October 31, 2023. Technical assistance provided during today’s visit: • You are in Cohort 1 which means that your prep year is now July 1, 2023 through June 30, 2024 and your license reassessment year will be July1, 2024 through June 30, 2025. You were sent an email 6/28/23 telling you about the cohorts. In the email you received detailed information with tasks and activities to complete. You may contact the Wilkes Partnership to ask Stacy Daniels to come out next year and conduct a mock assessment to prepare for your reassessment next year. • Review and familiarize yourself with the new sanitation rules on the DCDEE website under “What’s New?” and DCDEE rules effective 7/1/23, buy going to DCDEE Moodle to do the new rules review. • Make sure you have your sanitation ”clean-up crew” and clean-up plan in place. • Go to the EPR portal, check your plan for any updates, print the1st page of the EPR plan for me to view and review the updated plan with staff. Pubic-Schools will follow their Emergency Preparedness Procedures for your site. • Be sure all staff are completing continuing education and submit official transcripts to WORKS for evaluation prior to your license reassessment year. • There is a new Summary of the Law dated September 2023 posted on the DCDEE website under Provider > Provider Forms and Documents > Summary of the Law. • There is also a new Staff & Training worksheet on the website changing the three-year renewal to five (5) years on the CBC. • Remember that teacher assistants in NC Pre-K must now receive at least fifteen (15) hours of in- service training hours annually. • Continue to rake excess mulch from under the slide to the bottom of the slides. • Remember to document the “time required to evacuate” on the Emergency Drill Log & Report form each time a fire drill is conducted.
