You, Michelle Price, Lead Teacher, and Leah Hayes, Administrator/Principal, assisted me with today’s visit. I reviewed the restrictions, space capacity, and age range on your permit and observed these being maintained today. This facility’s compliance history was 99% as of May 4, 2026 and was reviewed with you today. This program operates with a five-star license, issued on February 19, 2026 by meeting the requirements for Pathway to the Stars: Classroom and Instructional Quality. The last sanitation inspection was dated April 21, 2026 with a Superior classification and zero demerits. The lead water test results dated July 3, 2025 indicated that this facility’s drinking water source was within the required limits. The lead-based paint test results dated December 30, 2025 indicated that this facility had no lead-based paint hazards. The asbestos test results dated December 18, 2025 indicated that this facility had no asbestos hazards. The last fire inspection on file was dated April 23, 2026 and was received in my office on April 27, 2026. Monthly fire drills including the last fire drill dated April 27, 2026 were recorded on the Fire Drill Log. Emergency drills (shelter-in-place and lockdown drills) completed at least every three months including the last emergency drill dated March 6, 2026 were recorded on the Emergency Drill Log. I completed a walk-through of the classroom. Children transitioned from the classroom to the cafeteria where they ate breakfast. Children transitioned from the cafeteria back to the classroom where they participated in a whole group lesson based on outer space and lead by the teacher, participated in a small group craft activity lead by the teacher assistant, played in activity areas, and attended to personal care needs. A current menu was posted, documented appropriate nutritious snacks and meals, showed a variety of foods, and complied with Meal Patterns for Children in Child Care Programs. Meals were prepared and served in the school cafeteria. Cold food was stored at a temperature of thirty-six degrees Fahrenheit in a commercial refrigerator in the cafeteria. Today’s breakfast met meal pattern requirements. I monitored hazardous product storage, medication storage, medication administration, general safety, and discipline practices. I monitored program records, child record files, staff record files. I monitored health and safety requirements. I monitored NC PreK requirements during today’s visit. Leah Hayes, Administrator/Principal, does not serve as the lead teacher or assistant teacher in the classroom. This program uses Ages and Stages as the developmental screening tool, Teaching Strategies Gold as the formative assessment tool, and Creative Curriculum for Preschool, Fourth Edition, as the approved curriculum. The health assessment including the physical examination, immunizations, vision screening, hearing screening, and dental screening are used to refer children for additional services as needed. This program’s family engagement component included formal and informal parent-teacher conferences, classroom visits and options for parents and families to participate in classroom activities, parent education, opportunities for involvement in decision-making about the early childhood program, and opportunities to engage families outside the regular school day. You call or text the parent on the day of absence and at least by the third day of any child’s absence to inquire about the reason for the absence and report absences to Ms. Hayes and the Catawba County Partnership for Children. This NC PreK program followed the Newton-Conover City Schools calendar for the 2025 – 2026 school year. Supervision, reduced enhanced staff-child ratios and group sizes, approved space use, minimum space capacities, and permit restrictions were maintained during today’s visit. I completed the “Annual Compliance Monitoring Checklist for Child Care Centers” during today’s visit. As discussed today, this facility completed the star rated license assessment process in February 2026 by completing the requirements for Pathways to the Stars: Curriculum and Instructional Quality (Pathway 2). Today, I completed an informal evaluation of this facility’s five-star rated license to verify that the required components for the Pathways to the Stars: Curriculum and Instructional Quality (Pathway 2) license listed below were being maintained as expected: 1. Family and Community Engagement Practices: • Foundational: The family and community engagement foundational practices were in process. • Communication Practice 1: Emails, text messaging, and phone calls were used daily as means of two-way communication between the facility and the families. • Communication Practice 4: Connection to local community services and resources were shared with families at least quarterly through informational flyers that were posted at the school and sent to families through email messages. • Engagement and Leadership Practice 3: At least two family events were offered to families in during the 2025 – 2026 school year including a “Trunk or Treat” on October 24, 2026 and “Around the World in 90 Minutes” on March 12, 2026. • Educational Opportunity 1: The administrator offered at least one family event during the 2025 – 2026 school year that included an educational opportunity for staff and families to learn together including “The Dangers of Technology: Part 1” on November 13, 2025 and “The Dangers of Technology: Part 2” on January 15, 2026. 2. Facility Continuous Quality Improvement Plan: The facility continuous quality improvement plan was in place and in process. 3. Individual Continuous Quality Improvement Plans: Individual continuous quality improvement plans were in process. 4. Approved Curriculum: I observed Creative Curriculum for Preschool, 6th Edition, the approved curriculum of choice for this facility, being implemented by the activities documented on the activity plan and being implemented with students today. 5. Five Star Education Standards: The administrator, lead teacher and teacher assigned to this facility during the rated license assessment process in February 2026 had not changed. During the on-going monitoring of the rated license components listed above, I determined this facility continues to maintain a five-star rated license through ongoing compliance with the Pathways to the Stars: Curriculum and Instructional Quality (Pathway 2) requirements. I observed and cited the following violation during today's visit: Violation Number Comment Rule 1805 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. The Division was not notified of the employment/assignment of the administrator and the lead teacher currently employed/assigned to this facility when the ABCMS staff roster was established. G.S. 110-90.2 & .2703(r) TECHNICAL ASSISTANCE 1. As stated in G.S. 110-90.2 & .2703(r), child care operators have five business days to notify the Division of any new child care providers working who were hired or moved into the child care facility. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Today, I observed a staff roster for the facility established on the ABCMS portal established. However, the administrator and the lead teacher currently employed by the facility were not listed on the facility’s ABCMS staff roster. Therefore, DCDEE was not notified within five business days of employment when the ABCMS staff roster was established. To correct this violation, the administrator and the lead teacher should be added to the facility staff roster in the ABCMS Provider Portal. To maintain compliance with this child care requirement, you should update the information in the ABCMS portal regarding new hires and staff members as they separate from employment. This information should be updated in ABCMS in an ongoing basis as staff members are hired and when their employment is terminated. These actions satisfy the requirement to notify the Division within five business days of new child care providers working who were hired. Please note, the hard copy of the Change of Information form will no longer be needed or accepted. Please be sure the five foundational and the four additional Family and Community Engagement practices are included in the facility’s written policies (handbook). 2. To assist with the ongoing monitoring of the rated license components, please add the Individual CQI Plan for the lead teacher and the teacher assistant to the “QRIS Notebook” kept in Ms. Hayes’ office or in the individual’s staff file. 3. Please be reminded that the five hours of coaching and mentoring required for the administrator and the lead teacher are due to be completed by February 19, 2026 and should be documented including the name and signature of the coach/mentor, the name and signature of the staff member, the date of the coaching/mentoring session, and a summary of the topics discussed. 4. Per NC General Statute 110-102, the current version of Summary of North Carolina Child Care Law should be posted in a prominent place in the facility and included in the enrollment packet to be given to parents prior to the child’s first day of attendance. The most current Summary of North Carolina Child Care Law was published in November 2025. Today, I observed the date of the posted Summary of North Carolina Child Care Law February 2025. Please print the updated version, post it in your lobby, and add it to your new enrollment packets. I also suggest you regularly check the DCDEE website for updated versions and especially when you receive notification of child care or sanitation rule updates. 5. Failure to correct the violations and send the written statement by the due date listed above may result in an unannounced follow-up visit being conducted or an administrative action may be recommended. Based on Child Care Rule 10A NCAC 09 Section .2200, the Division of Child Development and Early Education may take administrative action against the license and/or impose civil penalties based on the failure of the operator to correct any documented violations within the established time period. Child care programs are required to maintain compliance with all applicable child care rules and regulations at all times. North Carolina General Statute 110-90(4)(d) requires all child care facilities to maintain a compliance history of at least seventy-five percent (75%) for the past eighteen (18) months or during the length of time the facility has operated, whichever is less. Carolyn Conley, Lead Child Care Consultant, issued your temporary license and will continue to be your point of contact through the remainder of your temporary license period. I completed this TTP#1 visit upon Ms. Conley’s request and as a courtesy to Ms. Conley. I will be your point of contact for this TTP#1 visit only. Upon arrival, I was greeted by Mercedes Warren, NC PreK Teacher Assistant, and Michelle Price, NC PreK Teacher, who assisted me with today’s visit. Later in the visit, Leah Hayes, Administrator/Principal, greeted me in the classroom and later met with me in her office to discuss the QRIS Pathways to the Stars information and options and assist me with today’s visit. Currently, your facility operates with a temporary license issued from August 18, 2025 through February 18, 2026. The facility’s temporary license, NC Summary of Child Care Law poster dated February 2025, Classroom Staff to Child Ratio charts, safe arrival and departure procedures, emergency medical care plan, evacuation plan, daily schedule, activity plan, and emergency phone numbers were posted in a prominent area. The last monthly fire drill dated September 24, 2025 was documented on the Fire Drill Log. The last emergency drill dated August 27, 2025 was a shelter-in-place drill documented on the Emergency Drill Log. The last monthly playground inspection dated October 1, 2025 was documented on the Playground Inspection Checklist. The current and posted menu documented appropriate nutritious snacks and meals, showed a variety of foods, and complied with Meal Patterns for Children in Child Care Programs. Meals were prepared and served in the school cafeteria where children had many choices that met meal pattern requirements. Cold food was stored in a commercial walk-in refrigerator in the school kitchen at a temperature of thirty-four degrees Fahrenheit. Milk was stored in a milk cooler at a temperature of thirty-five degrees Fahrenheit. Cold food and beverages served to the children in the classroom were stored in a compact refrigerator at a temperature of twenty-five degrees Fahrenheit. I completed a walkthrough of the indoor and outdoor environment. The children were engaging in a whole group language activity centered around a farm theme, transitioning to activity areas, playing in activity areas of their choice, attending to personal care routines as needed and as part of their daily routine, transitioning from the classroom to the cafeteria, choosing meal items as they went through the line in the cafeteria, eating lunch, transitioning from the cafeteria to the classroom to wash hands and then transitioning to the playground for outdoor play, and playing outdoors. Positive interactions between the staff and children took place throughout the visit today. I monitored supervision, adequate approved enhanced space use, enhanced capacity and group size, enhanced staff/child ratios and permit restrictions. I monitored hazardous product storage, medication storage, medication administration, general safety, and discipline practices. I monitored a portion of the children files. I monitored a portion of the staffs files. No Staff and Training Worksheet was provided today. Please keep the Staff and Training Worksheet current and available for review by DCDEE staff during all monitoring visits. I discussed the rated license process with Ms. Hayes. It is very important that you familiarize yourself with the QRIS Pathways to the Stars. There are now three options available to demonstrate quality and earn a two through five star rated license. It includes updates to education standards for early childhood educators to expand options; and adds standards for both educators and facilities that support continuous quality improvement and family/community engagement practices that promote communication and collaboration. Please be reminded that a compliance history score of seventy-five percent or above must be maintained to receive a rated license. The following violation was observed, cited, and corrected during today's visit: Violation Number Comment Rule 508 Special diet or food allergy information was not posted where they can be seen in food preparation and eating areas. One child with a food allergy was not listed on the posted allergy list in the classroom. .0901(g) TECHNICAL ASSISTANCE: 1. Per child care requirement 10A NCAC 09 .0901 (g), children's special diets or food allergies should be posted where they can be seen in the food preparation area and in the child's eating area. During today’s visit, one child with a food allergy was not listed on the allergy list posted in Room A117. The NC PreK teacher added the child to the allergy posting and placed it back on the classroom refrigerator which corrected this violation today. To maintain compliance with this child care requirement, I suggest you review this child care requirement and the children’s files to be sure all food allergies are posted. CONSULTATION: 1. As discussed today, the temporary license was issued for a capacity of twenty children ranging from three to five years of age, and eighteen children are currently enrolled and attending. While classroom A117 has a licensed capacity for forty-eight children at the enhanced indoor space requirement of thirty square feet per child, the preschool playground has a licensed capacity for eighteen children at the enhanced outdoor space requirement of one hundred square feet per child using the playground at any given time. As discussed, if you choose to increase the classroom enrollment to twenty children ranging from three to five years of age, you would need to maintain enhanced outdoor space capacity for the playground by revising the classroom daily schedule to include two forty-five minute outdoor play times that allowed two different groups of children to be taken outside daily or by expanding the fence around the outdoor play area to include an additional one hundred square feet per child for a total of two hundred additional square feet. 2. All staff must have their current education evaluated by the WORKS Unit in Raleigh. Instructions for the DCDEE WORKS system, including establishing a NCID, is located on our website at https://ncchildcare.ncdhhs.gov/. Remember to track the date information is submitted by staff. Cold food was stored in a commercial walk-in refrigerator in the school kitchen at a temperature of thirty-four degrees Fahrenheit. Milk was stored in a milk cooler at a temperature of thirty-five degrees Fahrenheit. Cold food and beverages served to the children in the classroom were stored in a compact refrigerator at a temperature of twenty-five degrees Fahrenheit. I completed a walkthrough of the indoor and outdoor environment. The children were engaging in a whole group language activity centered around a farm theme, transitioning to activity areas, playing in activity areas of their choice, attending to personal care routines as needed and as part of their daily routine, transitioning from the classroom to the cafeteria, choosing meal items as they went through the line in the cafeteria, eating lunch, transitioning from the cafeteria to the classroom to wash hands and then transitioning to the playground for outdoor play, and playing outdoors. Positive interactions between the staff and children took place throughout the visit today. I monitored supervision, adequate approved enhanced space use, enhanced capacity and group size, enhanced staff/child ratios and permit restrictions. I monitored hazardous product storage, medication storage, medication administration, general safety, and discipline practices. I monitored a portion of the children files. I monitored a portion of the staffs files. No Staff and Training Worksheet was provided today. Please keep the Staff and Training Worksheet current and available for review by DCDEE staff during all monitoring visits. I discussed the rated license process with Ms. Hayes. It is very important that you familiarize yourself with the QRIS Pathways to the Stars. There are now three options available to demonstrate quality and earn a two through five star rated license. It includes updates to education standards for early childhood educators to expand options; and adds standards for both educators and facilities that support continuous quality improvement and family/community engagement practices that promote communication and collaboration. Please be reminded that a compliance history score of seventy-five percent or above must be maintained to receive a rated license. The following violation was observed, cited, and corrected during today's visit: Violation Number Comment Rule 508 Special diet or food allergy information was not posted where they can be seen in food preparation and eating areas. One child with a food allergy was not listed on the posted allergy list in the classroom. .0901(g) TECHNICAL ASSISTANCE: 1. Per child care requirement 10A NCAC 09 .0901 (g), children's special diets or food allergies should be posted where they can be seen in the food preparation area and in the child's eating area. During today’s visit, one child with a food allergy was not listed on the allergy list posted in Room A117. The NC PreK teacher added the child to the allergy posting and placed it back on the classroom refrigerator which corrected this violation today. To maintain compliance with this child care requirement, I suggest you review this child care requirement and the children’s files to be sure all food allergies are posted. CONSULTATION: 1. As discussed today, the temporary license was issued for a capacity of twenty children ranging from three to five years of age, and eighteen children are currently enrolled and attending. While classroom A117 has a licensed capacity for forty-eight children at the enhanced indoor space requirement of thirty square feet per child, the preschool playground has a licensed capacity for eighteen children at the enhanced outdoor space requirement of one hundred square feet per child using the playground at any given time. As discussed, if you choose to increase the classroom enrollment to twenty children ranging from three to five years of age, you would need to maintain enhanced outdoor space capacity for the playground by revising the classroom daily schedule to include two forty-five minute outdoor play times that allowed two different groups of children to be taken outside daily or by expanding the fence around the outdoor play area to include an additional one hundred square feet per child for a total of two hundred additional square feet. 2. All staff must have their current education evaluated by the WORKS Unit in Raleigh.
