Your program currently operates with a 1-Star license effective August 5, 2020. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-nine (99%) percent as of August 13, 2025. The license was posted, and the restrictions were in compliance. The most recent sanitation inspection was conducted on February 4, 2025, with a superior rating. The most recent fire inspection was conducted on July 8, 2024. There are three (3) approved indoor spaces and two (2) approved outdoor spaces. Sixteen (16) children between the ages of zero (0) and four (4) are enrolled and ten (10) children were present today. A walk-through of the facility was completed today. All approved indoor and outdoor spaces being used by the children were observed. Indoor spaces are arranged into activity areas that include but are not limited to language, blocks, art, math, science, sensory, dramatic play, and manipulatives. A variety of age-appropriate learning materials were observed in each classroom. The attendance for each classroom was accurately completed. Staff/Child ratios were in compliance, and all children were adequately supervised. Good interactions were observed between caregivers and children. Upon arrival at your facility, all children were napping on individual cots with individual linens. Soft music was playing in the background. After nap time, the children transitioned to routine handwashing and afternoon snack which consisted of vegetable straws and water. Cot spacing, handwashing, and food service procedures were monitored and found to be in compliance. Health and Safety trainings, Shaken Baby and Abusive Head Trauma Policy, and Safe Sleep Policies were monitored today, and requirements were met. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. Your program currently operates with a 1-Star license issued August 5, 2020. Due to travel time, I arrived at your facility at 9:45am. The last annual compliance was conducted on March 5, 2024. The sanitation inspection was completed February 24, 2056, with a “Superior” classification. The last fire inspection was conducted on July 8, 2024. Your facility was approved for 1st shift care and a maximum of thirty-five (35) children and six (6) staff members. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-nine (99%) percent as of February 27, 2025. The NC Secretary of State website was reviewed on February 27, 2025, and Little GEMS Daycare Center, Inc. was listed as current-active. Your program currently operates with a 1-Star license issued August 5, 2020. Due to travel time, I arrived at your facility at 9:45am. The last annual compliance was conducted on March 5, 2024. The sanitation inspection was completed February 24, 2056, with a “Superior” classification. The last fire inspection was conducted on July 8, 2024. Your facility was approved for 1st shift care and a maximum of thirty-five (35) children and six (6) staff members. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-nine (99%) percent as of February 27, 2025. The NC Secretary of State website was reviewed on February 27, 2025, and Little GEMS Daycare Center, Inc. was listed as current-active. Fourteen (14) children, between the ages of one (1) and four (4) years old are enrolled and seven (7) were present today. There are three (3) approved indoor spaces and two (2) approved outdoor spaces. At least five (5) activity areas including language, blocks, manipulatives, dramatic play, and art were available in the classroom. Age-appropriate materials and activities were provided for all children. Toddlers assigned to indoor space #1 were seated at a table with a staff member coloring with crayons. Older preschool children assigned to indoor space #2b were transitioning to the outdoor space. While in the outdoor space, children were observed running, playing basketball, riding cars, and reading books. Water was available for the children while they played in the outdoor space. Outdoor space one (1) had a variety of gross motor equipment including, two (2) small climbers with slides, two (2) small push car ramps, a log cabin playhouse, riding cars, a covered dramatic play area, a small basketball goal, and picnic tables. wooden sandbox. Violation Number Comment Rule 1232 Each employee's personnel file did not contain an annual staff evaluation and a staff development plan. Three (3) out of three (3) employee’s personnel files did not contain an annual staff evaluation. 10A NCAC 09 .0514(f) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation documented may impact the compliance history score. Violation number 1232 was corrected during the visit; therefore, a compliance letter is not required. Technical Assistance: Staff Annual Evaluations/Staff Development Plans: Child Care Rule 10A NCAC 09 .2805(f) requires that each employee’s personnel file contains an annual staff evaluation and staff development plan. I encourage you to streamline this process by selecting certain months to conduct evaluations and update staff development plans. This will result in easy tracking and ensure that all evaluations and development plans are completed within the yearly timeframe. Background Checks: North Carolina child care administrators can now view and edit their facility’s staff roster in ABCMS, the DCDEE’s criminal background check system. Today, I verified that you have completed the required training to access the ABCMS portal. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS on an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. Your facility was approved for 1st shift care and a maximum of thirty-five (35) children and six (6) staff members. The center's compliance history was reviewed with the operator. The program’s compliance history was one hundred (100) percent. The NC Secretary of State website was reviewed on March 5, 2024, and Little Gems Daycare Center, Inc. was listed as current-active. Eighteen (18) children, between the ages of zero (0) and four (4) years old are enrolled and eleven (11) were present today. There are five (5) approved indoor spaces and two (2) approved outdoor spaces. At least five (5) activity areas including language, blocks, manipulatives, dramatic play, and art were available in the classroom. Age-appropriate materials and activities were provided for all children. Infants and toddlers assigned to indoor space #1 were receiving care based on their individual needs. Diaper changing and handwashing routines were monitored, and requirements were met. An infant was placed in a bouncy activity center which had a variety of infant sensory toys on it. A staff member was seated on the carpet and read a book to the toddler. Younger preschoolers assigned to indoor space #2a were observed in group time. A staff member and the children took turns naming colors on a color chart. After group time the children transitioned to toileting routines. Older preschool children assigned to indoor space #2b were observed transitioning from toileting routines to group time. During the group time activity, a staff member assisted each child with putting on their individual Cat in The Hat mask and then she read the book, “Green Eggs and Ham”. Toileting and handwashing procedures were monitored and found to be in compliance. All staff members were actively engaged with the children.
