Your program currently operates with a 5-Star license effective December 5, 2018. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-seven (97) percent as of September 26, 2024. The NC Secretary of State website was reviewed on October 10, 2024, and Carteret County Public School Foundation was listed as current-active. The license was posted, and the restrictions were in compliance. Lead Teacher, Erika Fiorini, was available for consultation during the visit. There is one (1) approved indoor space and one (1) approved outdoor space. Eighteen (18) children, between the ages of four (4) and five (5) years old are enrolled and eighteen (18) were present today At least five (5) activity areas including language, blocks, manipulatives, dramatic play, and art were available in the classroom. Age-appropriate materials and activities were provided for all children. A walk-through of the facility was completed today. All indoor and outdoor areas were monitored. Current activity plans were observed posted in all classrooms. Upon arrival to your classroom, one (1) staff member was in the classroom assisting one (1) child using the bathroom, and one (1) staff member was on the playground with eighteen (18) children. The children in the outdoor space were running, climbing, playing in the sand box, playing basketball, riding scooters, and bouncing balls. The staff member and the child in the indoor space returned to the playground. The children transitioned from the outdoor space to the indoor space for lunch. Lunch consisted of cheese pizza, apples, baked beans, and milk. After lunch, the children transitioned to nap time. Each child was provided an individual cot with individual linens. Handwashing, toileting, food service and mat spacing were monitored, and requirements were met. The outdoor space had a variety of gross motor equipment including tricycles, large legos, scooters, hula hoops, balls, two (2) large manipulative mazes with gears, two (2) large shade umbrella structures, two (2) small rock climbers, an art easel, a large and small basketball goal, a large climber with a double slide, and a see saw. Health and Safety trainings and the Shaken Baby and Abusive Head Trauma Policy were monitored today and found to be in compliance. The following violation was observed and must be corrected immediately. Violation Number Comment Rule 1775 NC Pre-K program staff/child ratios and group sizes were not met. . One (1) staff member was in the outdoor space with seventeen (17) children, while one (1) staff member was in the indoor space with one (1) child. This program has highest voluntary enhanced staff/child ratios; therefore, the staff/child ratio is one (1) staff member to twelve (12) children. (1:12) .3009 Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation documented may impact the compliance history score. The violation documented must be corrected immediately. On or before Friday, October 18, 2024. I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violation was corrected. Maintaining staff/child ratios and maximum group sizes is also essential to the health and safety of children. If children are combined with similar age groups, such as in the early morning or later evening hours, the staff/child ratio and maximum group size for the youngest child in the group must be maintained. Staff must be aware of children’s ages and how many children are in each group at all times. Today I suggested that if you’re in the outdoor space and a child needs to return to the indoor space for toileting, other children need to return to the indoor space with a staff member as well to maintain staff child ratios. QRIS Update: Starting on February 1, 2025, the ECERS-3, ITERS-3, and FCCERS-3—also known as the "3s"—will be used for DCDEE environment rating scale assessments. The DCDEE and the North Carolina Rated License Assessment Project (NCRLAP) are collaboratively working on preparations for the transition to the third editions. These third editions come with a spiral binding at the top, replacing the current revised editions. Your program currently operates with a 5-Star license issued December 5, 2018. The last annual compliance was conducted on April 18, 2023. The sanitation inspection was completed December 19, 2023, with a “Superior” classification. The last fire inspection was conducted on October 31, 2023. Your facility was approved for 1st shift care, the facility meets enhanced ratios, enhanced space, and minus 1 child in each group. The center's compliance history was reviewed with the operator. The program’s compliance history was one hundred (100) percent. The NC Secretary of State website was reviewed on April 9, 2024, and Carteret County Public School Foundation, was listed as current-active. Sixteen (16) children, between the ages of four (4) and five (5) years old are enrolled and fourteen (14) were present today. There is one (1) approved indoor space and one (1) approved outdoor space. At least five (5) activity areas including language, blocks, manipulatives, dramatic play, and art were available in the classroom. Age-appropriate materials and activities were provided for all children. Children were engaged in free play activities which included planning a picnic, magnetic building cubes, animal, cars, wooden blocks, sand play, Lincoln logs, magnetic numbers, and Khan Academy Kids electronic application. The children transitioned from free play activities to outdoor play, and lunch. Lunch consisted of cheese pizza, corn, apple crisp, and milk, Toileting, handwashing, screen time, and food service procedures were monitored, and requirements were met. The outdoor space had a variety of gross motor equipment including tricycles, large legos, scooters, hula hoops, balls, two (2) large manipulative mazes with gears, two (2) large shade umbrella structures, two (2) small rock climbers, an art easel, a large and small basketball goal, a large climber with a double slide, and a new see saw. Health and safety trainings were monitored, and requirements were met. Violation Number Comment Rule 705 Equipment and furnishings were not sturdy, stable and free of hazards. In the outdoor space, the sand in the covered sand box area covered the majority of the sand box barrier. Only a small area of the barrier was visible around the sand causing a tripping hazard for the children. .0601(c) 1867 The depth of the loose surfacing was not based on critical height of the equipment. Child Assessment and Required Screenings: The assessment tool, GOLD, Teaching Strategies 2010-2011, is in place. Portfolios for each child are maintained in both indoor spaces and electronically on Teacher Strategies Gold online application. The facility uses the developmental screening tool, Brigance, for pre-screening and ongoing assessments. Documentation of health assessments, vision, hearing, dental screenings, and DIAL-4 pre-screenings were on file for each child. Curriculum: The Creative Curriculum for Preschool, 6th Edition, Teaching Strategies, 2016 has been implemented and the classroom has the required resources available for use. Materials to support the current activity plan were displayed throughout the classroom. Family Engagement: Parent involvement includes Open House, parent’s Night, Fall Festival, class Dojo application, parent teacher conferences, and parent volunteers for classroom events. Staff Requirements: Both the administrator and lead teacher met NC Pre-K education requirements. The teacher assistant has an associate degree in applied science; however, she is currently enrolled in courses to complete an associate degree in ECE. Technical Assistance: Importance of Resilient Surfacing Nationally, nearly 70% of playground related injuries are from falls. Fall injuries are the result of two things: 1) The height a child falls from and 2) The material/surface the child falls upon. Provide “soft” resilient surfacing under and around stationary playground equipment. Refer to 10A NCAC 09 .0605(h)(i). As we discussed today, the mulch around the climbing structure is compacted, has weeds, and grass growing up through it, and needs to be tilled. Fall Zones with Weeds As discussed, fall zones should be weeded regularly and loose parts including pine cones, gumballs and sticks should be collected daily to ensure fall zones are kept free of hazards. For the safety of the children, you must ensure weeds and debris are removed and sand is tilled or added to the appropriate depth to avoid potential injuries to children. I also urged you again to create a regular weeding and tilling schedule for the sand. To verify compliance, please submit a copy of an April playground inspection showing these hazards have been addressed.
