The facility has a 1-star license and had a 86% compliance history prior to today’s visit. All applicable requirements were monitored today. There are eight (8) children enrolled, and seven (7) children were present today. The children were observed during indoor free play. The staff and training worksheet was received via email prior to today’s visit. Two (2) staff files were monitored today. All staff have current CPR and First Aid certification, and specialized training. I monitored the files of two (2) children. I monitored the regulatory system to verify current criminal record qualification letters for all staff. An approved sanitation inspection was last conducted on 11/13/25 with 10 demerits. The last fire drill was conducted on 2/6/26. The last fire inspection was conducted on 11/25/25. A shelter in place drill was conducted on 1/6/26. All emergency drills are documented as required. Playground Inspections have been documented as required. The facility does not provide transportation and medication is not administered. The following items were posted for easy reference: license, NC Summary of Child Care Law, safe arrival and departure procedures, emergency medical care plan, emergency telephone numbers, daily classroom schedules, First Aid poster, applicable staff/child ratio charts, and tobacco free signage. The following violations of child care requirements were observed today. Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. A current activity plan was not posted for either group of children. GS 110-91(12); .0508(a) 533 Human milk, formula and other bottled beverages including sippy cups, sent from child's home were not fully prepared, dated, and labeled for the appropriate child. A child was observed eating a bottle that was not labeled with a name or the date. 15A NCAC 18A .2804(d) 604 Lavatories were not kept clean, in good repair and kept free of storage. The bathroom was observed as being used for storage of items to include a mop, mop bucket, and several other materials stacked in a corner. 15A NCAC 18A .2818(a) 1301 Center did not maintain a record of daily attendance. The attendance record posted indicated that the attendance for the children had not been recorded since 2/11/26. GS 110-91(9) 1329 Application for enrollment did not include all required information. One (1) of two (2) children's files monitored did not have responses to the questions regarding particular fears, allergies, unique behaviors, and health care concerns. .0801(a)(1-7) 1792 Staff did not model appropriate eating behaviors by consuming food or beverages that meet the nutritional requirements specified in the Meal Patterns for Children in Child Care Programs in the presence of children in care. Upon arrival an open can of reign storm beverage for observed sitting on a table in space #1b/c. .0901(i) The violations documented during the visit must be corrected immediately. To verify correction of the violations you shall submit a signed letter to me explaining in detail how you have corrected the violations described above. The letter should include the facility name and facility ID number. List each item using the item number located to the left of the violation description. When submitting an email for corrections, that email must be sent from the email address of the person designated to submit corrections (example: director) and must include this person’s name and title at the end of the email. This center operates with a 1-star license. Prior to today's visit this facility had a compliance history of 89%. Upon arrival, I was greeted by the owner, Linda Little. There are eight (8) children currently enrolled, and eight (8) children were present today. The children present during this visit were observed in during free play in activity areas. All interactions and language observed was positive. The following were posted for easy reference: license, NC Summary of Child Care Law, safe arrival and departure procedures, menu, daily classroom schedules, emergency telephone numbers, emergency medical care plan, tobacco free signage, First Aid poster, applicable staff/child ratio charts, evacuation route, ITS-SIDS safe sleep policy, and activity plan. The following items were monitored today: supervision, staff-child ratios, CPR/FA certification, specialized training, criminal record qualifications, storage of hazardous products and medications, adequate/approved space, and program records. All indoor areas used by the children were monitored. The last documented fire drill was conducted on 11/3/25. The last shelter-in-place or lockdown drill was last conducted on 8/13/25. Prior to 8/13/25, the last shelter-in-place or lockdown drill was conducted on 4/3/25. The monthly playground inspections were monitored and have been completed as required. The last fire inspection was conducted on 11/5/24, the facility was approved for daytime care only. The last sanitation inspection was conducted on 4/8/25. During today's visit Ms. Little expressed that she is interested in having herself removed as the facility administrator and would like to have A. Barrett, to be the administrator. We discussed that this request would need to be made in writing and sent to the consultant along with the pre-service for administrators form. A copy of the form was given to Ms. Little during today's visit. The consultant discussed briefly the pathways to the stars with Ms. Little, she stated that she wishes to remain a 1 star facility. The following violations of child care requirements were observed today. There are nine (9) children currently enrolled, and seven (7) children were present today. The children present during this visit were observed in activity areas engaged in free play. The teachers were observed interacting with the children and provided positive guidance when needed. The following were posted for easy reference: license, NC Summary of Child Care Law, safe arrival and departure procedures, daily classroom schedules, emergency telephone numbers, emergency medical care plan, tobacco free signage, First Aid poster, applicable staff/child ratio charts, evacuation route, ITS-SIDS safe sleep policy, and daily schedules. The following items were monitored today: supervision, staff-child ratios, CPR/FA certification, specialized training, criminal record qualifications, storage of hazardous products and medications, adequate/approved space, and program records.
