This family childcare home has a 3-star license issued on 7/11/2019 and the compliance history prior to this visit was 99%. The last annual compliance visit was conducted on 7/10/24. Upon arrival I was greeted by the provider, Lisa Fulcher. There are nine (9) children currently enrolled. Five (5) children were present today. During the visit I observed the children participating in free play with developmentally appropriate materials. The interactions between the children and provider were positive. The provider engaged with the children and attended to their needs. The following were posted for easy reference: license, NC Summary of Law, emergency phone numbers, emergency procedures, tobacco free signage, medical care plan, first aid poster, ITS-SIDS policy and ITS-SIDS poster, activity plan and daily schedule. Ms. Fulcher’s CPR and FA training are valid until August 2026. The provider's ITS-SIDS certification is valid through June 2026. All Health and Safety training topic areas were last completed in 2022. The Criminal Background Check qualifying letter for Ms. Fulcher expires on 5/6/27. The Criminal Background Check qualifying letter for the provider’s husband, Rufus Fulcher, who is a household member expires on 5/6/27. The following items were monitored today: supervision, capacity, specialized training, criminal record qualifications, storage of hazardous products, adequate/approved space, staff records, and program records. I observed all indoor areas used by the children. Fire drills and shelter-in-place/lockdown drills are being conducted as required. Playground inspections were monitored during the visit and are being conducted as required. The provider does not administer medication. Transportation is not provided. There were two (2) violations of childcare requirements observed during today’s visit. Violation Number Comment Rule 542 When screen time was provided, it was not offered to stimulate a developmental domain in accordance with NC Foundations for Early Learning and Development. During the visit a television in the caregiving space was observed on with cartoons playing. .1718(b)(1) 543 Screen time was not prohibited for children under the age of three. The operator did not offer alternative activities for children under the age of three years. During the visit a television playing cartoons was observed; children under the age of three were present. .1718(c) The violations documented during the visit must be corrected immediately. To verify correction of the violations you shall submit a signed letter to me explaining how you have corrected the violations described above, and how you will prevent these violations in the future. Technical Assistance: During the visit a first aid kit was observed stored 5 feet from the floor that contained triple antibiotic with a warning to keep out of the reach of children. You, Lisa Fulcher, stated the ointment is not used on the children and the tube was discarded during the visit. I observed the children engaged in the following activities: free-play with developmentally appropriate materials, fine motor activities and a teacher-directed activity. The program operates with a 3-star license issued July 11, 2019. The program earned four (4) points in education two (2) points in program standards and one (1) quality point. License restriction: Daytime care only; Maximum of 5 preschool children at any time; Serves no more than 2 infants under age one. The program has a compliance history of 97%. There are eleven (11) children currently enrolled and six (6) were present today. There are no infants enrolled at this time. I reviewed all children's records today. Two (2) of the eleven (11) children's records monitored contained documents that were submitted to the facility after the required timeframe. The Emergency Drill Log report indicated that the last fire drill was conducted on 7/1/24 and the last shelter-in-place drill was conducted on 6/4/24. Playground safety inspections were reviewed during the visit and monthly inspections were documented as required. The provider stated that transportation is not provided; therefore, no vehicles were monitored. The provider stated that she does not administer medication. A Verification of Required Information for Operator and Additional Caregivers form and FCCH and Centers Located in a Residence Operator’s Statements of Responsibility form were completed during today’s visit. Violation Number Comment Rule 1889 Products that are labeled 'keep out of reach of children' without any other warnings, were not stored on a shelf or in an unlocked cabinet that is five feet above the finished floor. During the visit a tube of neosporin cream was observed laying on the floor in the childcare space. .1719(a)(7) 2033 Signed discipline statement did not include the required information, as outlined in rule. A signed discipline statement for two (2) children enrolled did not contain all the required information. .1727(c )(1-3) The violations documented during the visit must be corrected immediately.
