Upon arrival, I met with Madison Hatley, Assistant Director. I stated the purpose of my visit. The facility operates three (3) NC Pre-K classrooms. I conducted a walk-through of the facility. The facility is currently operating with a Special Provisional License. The facility had a 91% compliance history prior to this visit. The NC Secretary of State website was viewed before the visit and All Star Learning Center of Stanly Co of NC, Inc. was current and active. There are 142 children currently enrolled, and 101 children were present during the visit. I observed the children present engaged in the following activities: free play in activity areas, bathroom and handwashing routines, lunch and outdoor play. The staff and training worksheets were received prior to today’s visit. I selected fourteen (14) children's files for review. Four (4) of the fourteen (14) children’s files monitored were for currently enrolled NC Pre-K children. Six (6) new staff have been hired since the last monitoring visit. The files of the new staff were monitored today and the files of three (3) existing staff were monitored. The NC Pre-K monitoring was conducted during this visit. The last fire inspection was conducted on 5/20/24 and the facility was found to be satisfactory and approved for daytime care only. The last sanitation inspection was conducted on 11/22/24. The facility received a Superior rating with 0 demerits. The Emergency Drill Log report form was reviewed during the visit. Fire drills have been completed as required. The last fire drill was conducted on 3/11/25. Shelter-in-place or lockdown drills have not been conducted as required. The last shelter-in-place or lockdown drill was conducted on 2/19/25. The monthly outdoor inspections have been completed as required. The following items were posted for easy reference: license, NC Summary of Child Care Law, safe arrival and departure procedures, emergency medical care plan, emergency telephone numbers, daily schedules, current activity plans, CPR and First Aid poster, menu, applicable staff/child ratio charts, tobacco free signage, mat lists, and evacuation routes. One (1) van was monitored for transportation requirements. All transportation requirements are being met. I monitored the following items today: supervision, staff/child ratio, staff, health, safety, program records and management. NC Pre-K: This facility has three (3) NC Pre-K classrooms. The NC Pre-K program operates from 8:15a.m. until 2:45p.m. The facility uses the ‘Creative Curriculum’ for the NC Pre-K program, as well as the 'Brigance Early Childhood Screen' and the 'Teaching Strategies Gold' (TS Gold) developmental screening tools. The initial 'Brigance Screen' was completed for each child within the required time frame. Developmental screenings have been completed multiple times during the school year for all children. Parents participate in Family Engagement activities, monthly take home projects and other activities. The facility has hosted multiple family events throughout the school year. The next event is scheduled for April 2025. Newsletters are sent home monthly. Parent/teacher conferences are held three (3) times per year. Two (2) conferences have been held thus far for the school year. Progress reports are shared with parents during the parent/teacher conferences. The facility meets the required 1:9 ratios and operates the required 32.5 hours per week. Rated License: The facility had the environment rating scale assessments conducted in May 2024. The average score for the ECERS-R conducted on 5/1/2024 was 5.7 points. The average score for the ITERS-R conducted on 5/3/2024 was 4.8 points. The average score for the SACERS-U conducted on 5/8/2024 was 5.66 points. According to the program requirements for the NC Star Rated License: The overall average score (across 3 assessments) is 5.39 points. The facility earned six (6) points in program standards. The education evaluations in WORKS for the Lead Teachers and Teachers was not monitored prior to this visit. It is proposed that the facility will earn seven (7) points in staff education. The facility is currently meeting one of the quality options to earn a quality point. It is proposed that the facility will receive fourteen (14) points for a 5-star license. There were two (2) violations of child care requirements observed during today's visit. Violation Number Comment Rule 1052 Staff required to receive on-going training had not completed the required number of hours according to their education and experience. M. Hatley completed 6.5 on-going training hours of the 10 hours required. .1103(a) 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. A lockdown drill was completed on 10/10/24 and the next shelter-in-place drill was completed on 2/19/25. .0604(u);.0302(d)(8) The violations documented during the visit must be corrected immediately. To verify correction of the violations you shall submit a signed letter to me explaining in detail how you have corrected the violations described above. The letter should include the facility name and facility ID number. List each item using the item number located to the left of the violation description. When submitting an email for corrections, that email must be sent from the email address of the person designated to submit corrections (example: director) and must include this person’s name and title at the end of the email. I must receive the letter by email or at the address below on or before 4/1/25. You are required to maintain compliance with all applicable childcare rules and regulations at all times. NC GS 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past 18 months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. Violations cited during the visit may negatively affect this score. I met with Madison Hatley, Assistant Director and we conducted a walk-through of the facility. The program operates with a five-star license issued June 28, 2018. Restriction: Daytime care only; Meets enhanced ratios; Meets enhanced space. The facility had an 93% compliance history prior to today's visit. The facility operates three (3) NC PreK classrooms. There are 150 children currently enrolled and 101 were present during the visit. Of the 150 children enrolled 54 are NC PreK children. The children were observed engaged in the following activities: free-play in activity areas, outdoor free-play, small group teacher-directed activities, bathroom/handwashing routines, lunch, and nap. Prior to the visit I sent an email requesting the staff and training worksheets to Amy Hatley, Director. The worksheets were received prior to the visit. One (1) new staff member has been hired since the last monitoring visit was conducted. I monitored the file of the new staff and three (3) files of existing staff for 10% of staff files. I monitored twelve (12) children's records during the visit. Five (5) of the twelve (12) children’s files monitored were NC PreK children files. The following items were posted for easy reference: license, Summary of NC Child Care Law and Rules, safe arrival and departure procedures, emergency medical care plan, daily schedules, current activity plans, First Aid posters, menu, applicable staff/child ratio charts, tobacco free signage, and evacuation routes. The annual fire inspection was conducted May 24, 2023 and the facility was found to be satisfactory and approved for day time care only. The sanitation inspection was conducted November 15, 2023 with a score of 0 demerits for a Superior classification. The last documented fire drill was conducted on February 23, 2024. A shelter-in-place drill was conducted on January 10, 2024. The monthly playground inspections have been completed as required. NC Pre-K: This facility has three (3) NC Pre-K classrooms. The facility operates with a 5-star license earning 6 points in program standards, 7 points in education and 1 quality point. Prior to this visit the facility had a compliance history of 95%. There were 106 children present today. The children were observed during outdoor free play, indoor free play, large group teacher-directed activities and bathroom/handwashing routines. The last monitoring visit was conducted on 6/12/23. A superior sanitation inspection was completed on 11/5/23. The last fire inspection was conducted on 5/24/23. A fire drill was last conducted on 12/18/23. A lockdown drill was last conducted on 11/30/23. Playground Inspections have been completed as required. All indoor areas used by children were monitored today. The following items were posted for easy reference: license, NC Summary of Child Care Law, safe arrival and departure procedures, daily schedules, First Aid posters, applicable ratio charts, emergency medical care plan, emergency telephone numbers, evacuation routes, tobacco policy, cot lists, and menu. The following items were monitored today: supervision, staff/child ratios, storage of hazardous products and medications, adequate/approved space, new staff files, program files, posted license and license restrictions. Eight (8) new staff have been hired since the monitoring visit was conducted. These staff files were monitored today. I monitored the current criminal record qualifications, CPR/First Aid certification, and specialized trainings for all staff using the staff and training worksheets that were completed for the last annual compliance visit. I observed children in space #2 participating in free-play in activity centers. The teacher was engaged with the children. All interactions and language observed were positive. I observed the children in space #3 engaged in free-play on the outdoor learning environment. The teachers were engaged with the children in care and provided assistance as necessary. The interactions and language observed were positive. I observed children in space #6 engaged in a large group teacher-directed activity and transitioning to free play in activity areas. The teachers prompted engagement from the children by asking open-ended questions. All interactions were positive. In space #7b I observed the children engaged in free-play in activity areas. The teachers monitored the whole class, while working with small groups of children. Assistance was provided to the children as needed. All interactions and language were positive. There were two (2) violations of child care requirements observed during today’s visit. Violation Number Comment Rule 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. During today's visit Tylenol with a permission authorization that expired on 11/30/23 was observed. Ibuprofen with a permission authorization that expired on 11/5/23 was observed. Vaseline and desitin with permission authorization that expired on 12/30/23 was observed. .0803(12) 1899 Health and safety training topics were not included as part of on-going training within five years of completing the previous health and safety training topics.
