You, Wendy Hudson, Owner, were present for consultation during the visit. Your program currently operates with a 4-Star license, issued March 13, 2025. The facility’s compliance history was reviewed with the operator. The program’s compliance history was one hundred (100) percent as of September 16, 2025. Restrictions on the permit include 1st, 2nd, and 3rd shift care, a maximum of five (5) preschool children cared for at any time, and trampoline not in use during child care hours. The license was posted, and the restrictions were in compliance. A walk-through of your facility was completed today; all indoor and outdoor areas were monitored. Five (5) children between ages one (1) and six (6) are enrolled and three (3) were present today. The indoor space was a safe environment, and a variety of activities were available for the children. All children were adequately supervised. Appropriate toys and materials were observed in this area. The bathroom is close proximity to the childcare space. Upon arrival at your facility, you and the children were playing in the childcare space. The toddlers were playing with manipulative activity tables. The other child was playing with sea animals. You were actively engaged with the children as they played. The children transitioned from free play activities to lunch. Lunch consisted of cheese pizza, French fries, peaches, and milk. Routine handwashing and food service procedures were monitored, and requirements were met. The outdoor space had a variety of gross motor equipment which included three (3) small slides, a table with chairs, one (1) small basketball goal, one (1) large basketball goal, a wagon, hula hoops, and one (1) small maze climber. The facility’s Shaken Baby and Abusive Head Trauma and Safe Sleep Policies were reviewed, and requirements were met. Because infants are not enrolled at this time, safe sleep checks were not monitored. Today you reported that you currently do not provide transportation. Violation Number Comment Rule 1876 The operator did not review the EPR Plan annually or when information in the plan changed to ensure all information is current. The last date the operator reviewed the EPR Plan is April 3, 2024. .1714(e ) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Your family childcare home currently operates with a 4-star license issued May 2, 2022. The facility’s last annual compliance visit was conducted on April 11, 2023. The family child care home’s compliance history was reviewed with the operator. The program’s compliance history was one hundred (100) percent as of April 2, 2024. Restrictions on the permit include 1st, 2nd, and 3rd shift care, a maximum of five (5) preschool children cared for at any time, and trampoline not in use during child care hours. You, Wendy Hudson, were present and available for consultation during the visit. Three (3) children between the ages of one (1) and five (5) are enrolled and one (1) was present. All spaces used by the children, indoors and outdoors, were monitored. Ms. Hudson has a room set up exclusively for childcare. Upon arrival at your facility, you and one (1) child were engaged in a variety of free play activities. Activities included singing fingerplay songs, alphabet puzzles, and singing the alphabet song. You also counted with the child, and reviewed farm animal sounds. The outdoor space had a variety of gross motor equipment including a large basketball goal, small basketball goal, small climber maze, wagon, dramatic play sink and stove, hula hoops, balls, tables, chairs, three (3) small climbers with slides, and a sand box. Today all children files were monitored. Health and safety training were monitored, and requirements were met. Currently, no infants were enrolled. You reported that you currently are not providing transportation. Violation Number Comment Rule 1865 The EPR Plan did not include the date of the last revision of the plan. The facility’s EPR Plan was not updated within the last year.
