D. Durham, Operator, assisted me with the visit. Your program currently operates with a four-star license, issued 12/3/2023, earning 7 points in the education component, 2 points in the program standards component and 1 quality point for infant capacity restricted to no more than two children under the age of one. The last annual compliance visit was conducted 1/6/25. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety eight percent as of 12/12/2025. The NC Secretary of State website was reviewed on 12/12/2025 and this facility is not currently owned or operated by a corporation. Please contact me prior to any changes regarding the ownership of this facility. We reviewed your facility contact information listed on the Division of Child Development and Early Education (DCDEE) website and all information is current. Contact me if you need to make any changes to your contact information, e.g., phone numbers, mailing address. All indoor and outdoor spaces were monitored. Upon arrival, the children were completing puzzles before group time that consisted of going over the alphabet and the letters in the children’s names. After group time, the children created the letter A out of playdough before creating Christmas crafts. After lunch, the operator read books to the children before they laid down on child sized mats for rest time. The outdoor space was observed to have portable houses, a climbing structure, slides, see saws, riding toys, and balls for the children to engage in gross motor activities. Lunch was observed and consisted of spaghetti, corn, applesauce, and milk. The following violation was documented. Violation Number Comment Rule 1720 Operator did not give written notice of the written plan of care amendment to parents of all children enrolled, at least 30 days before the change was implemented. Each parent did not sign a statement acknowledging the receipt and explanation of the amendment. The operator did not retain the acknowledgment in the child's record for as long as the child was enrolled in the home and a copy was not maintained on file for review by Division representatives. The written plan of care was not updated to reflect the routine task of transporting children before and after school. .1712(f) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. If this information will change, amend the written plan of care immediately and review with parents 30 days prior to implementing. The written plan of care was not updated to reflect the routine task of transporting children before and after school. You stated that you only transport two of the children enrolled and must have overlooked adding the routine task during the last update. During the visit you printed out copies of the written plan of care to update. Update the written plan of care and have all parents sign before adding to all children’s files. D. Durham, Operator, assisted me with the visit. Your program currently operates with a four-star license, issued 2/12/2024, earning 7 points in the education component, 2 points in the program standards component and 1 quality point for having obtained an Infant/Toddler Certificate. The last annual compliance visit was conducted 1/6/25. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety nine percent as of 5/27/25. The NC Secretary of State website was reviewed on 5/27/25 and this facility is not currently owned or operated by a corporation. Upon arrival, the children finished lunch that consisted of spaghetti, salad, watermelon, and milk. After personal care routines were completed, the children transitioned to the outdoor space. In the outdoor space, the children were observed swinging and kicking balls back and forth. The children transitioned inside and washed their hands before getting on their cots for rest time. The operator read a story to the children about vegetables before they laid down to rest. The outdoor space was observed to have portable houses, swings, basketball hoop, bikes, and balls for the children to engage in gross motor activities. A large shade structure has been added with activity centers beneath it. The following violation was documented. New staff are required to complete H&S Training within one year of hire (CPR/First Aid and Recognizing and Responding to Suspicions of Child Maltreatment within 3 months of hire) and then complete training in all topic areas every five years. Free H&S Training developed specifically for North Carolina is available in Moodle on the DCDEE website. We discussed that both operators need to complete the health and safety trainings as they due for the 5-year renewal this year. ITS/SIDS Training: Completing ITS/SIDS training at hire and every three years provides staff with the most current information and required practices to protect infants from Sudden Infant Death Syndrome and other accidents that may occur while sleeping. We discussed that your SIDS certification expires 2/9/25 and you registered for a training during the visit. Clean Classrooms for Carolina Kids: Due to the devastation caused by Hurricane Helene, the deadline for enrolling in the Clean Classrooms for Carolina Kids testing for lead in water, lead paint, and asbestos has been extended to May 31, 2025. If you have not completed the online enrollment, online training, and initial survey, complete this for your facility as soon as possible. Prior to enrollment of children in a family child care home, and before new animals that will be in the home come into the family child care home, a parent of each child must sign a form acknowledging the type of animal and where the animal will be during operating hours. This documentation shall be maintained in each child’s file. You stated that you will be getting a dog, and we discussed that you would need to have shot records on file and have parents sign the pet acknowledgement form that I provided you today. Health & Safety Training: Health & Safety (H&S) Training includes training in 9 topic areas plus Recognizing and Responding to Suspicions of Child Maltreatment and CPR/First Aid. New staff are required to complete H&S Training within one year of hire (CPR/First Aid and Recognizing and Responding to Suspicions of Child Maltreatment within 3 months of hire) and then complete training in all topic areas every five years. Free H&S Training developed specifically for North Carolina is available in Moodle on the DCDEE website. We discussed that both operators need to complete the health and safety trainings as they due for the 5-year renewal this year. ITS/SIDS Training: Completing ITS/SIDS training at hire and every three years provides staff with the most current information and required practices to protect infants from Sudden Infant Death Syndrome and other accidents that may occur while sleeping. We discussed that your SIDS certification expires 2/9/25 and you registered for a training during the visit. Clean Classrooms for Carolina Kids: Due to the devastation caused by Hurricane Helene, the deadline for enrolling in the Clean Classrooms for Carolina Kids testing for lead in water, lead paint, and asbestos has been extended to May 31, 2025. If you have not completed the online enrollment, online training, and initial survey, complete this for your facility as soon as possible. D. Durham, Operator, assisted me with the visit. Your program currently operates with a four-star license, issued 2/12/2024, earning 7 points in the education component, 2 points in the program standards component and 1 quality point for having obtained an Infant/Toddler Certificate. The last annual compliance visit was conducted 1/10/24. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety nine percent as of 5/28/24. The NC Secretary of State website was reviewed on 5/28/24 and this facility is not currently owned or operated by a corporation. Please contact me prior to any changes regarding the ownership of this facility. All indoor and outdoor areas were monitored. Upon arrival, the children were finishing a breakfast of cereal, oranges, and milk. After personal care routines were completed, the children sat at the table for group time. During group time, the children sang songs, went over the alphabet, and shapes. Each child put on a cat mask while the operator discussed how to spell cat and the differences between house cats and large cats. The outdoor space was observed to have portable houses, swings, tunnel, bikes, and balls for the children to engage in gross motor activities. A large shade structure has been added with activity centers beneath it. Lunch was observed and included spaghetti, cream corn, applesauce, and milk. The following violation(s) were documented. Three (3) canisters of Great Value Disinfecting wipes were located on a shelf in the caregiving space. .1719 (a)(7) 720 Adequate supervision was not provided for children in the Family Child Care Home. Upon arrival to the facility, the operator was observed outside bringing trash to the trash can. Four (4) children, 2-5 years of age, were in the facility unattended. .1711(a) 1854 The operator did not conduct a quarterly lockdown or shelter-in-place drill and or the drill record was incomplete. An emergency drill was not completed in the first quarter of 2024. .1719(a )(16) & .1721(e )(7) Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Parents/guardians have a contract with caregivers/teachers to supervise their children. To be available for supervision or rescue in an emergency, an adult must be able to hear and see the children. Upon arrival to the facility, the operator was observed outside bringing trash to the trash can. Four (4) children, 2-5 years of age, were in the facility unattended. You stated that your additional caregiver is usually always in attendance and had just left to run an errand. We discussed that staying with the children is more important than taking the trash outside. You stated that you had a long weekend and went outside without thinking first that you were the only caregiver at the time. During the visit, the additional caregiver arrived. We discussed that if you need to complete a task outside, you will ensure that someone is available to remain with the children before going outside. Emergency Drills: You must conduct either a shelter-in-place or a lockdown drill every three months to ensure that the operator and children are aware of what actions to take in the event of a life-threatening emergency. An emergency drill was not completed in the first quarter of 2024. You stated that it was hectic in the first few months of moving back to your home and must have forgotten to write down the drill. You also stated that you usually complete the emergency drill on the same day as the fire drill every three months. We discussed putting a reminder in your electronic calendar that will send you reminders each quarter to complete a drill. Conduct a drill and notate this on the emergency drill log. Storage of Hazardous Items: Intentional planning of the environment ensures a safe environment has been created which prevents and reduces injuries to young children. Three (3) canisters of Great Value Disinfecting wipes were located on a shelf in the caregiving space. We discussed that these wipes need to be in locked storage when not in use. You moved the disinfecting wipes to a locked cabinet during the visit to correct the violation. I provided you with a hazardous storage flyer during the visit to post for review. Check the caregiving space each day before the children arrive to ensure that all hazardous products are stored properly. Additional Comments: Clean Classrooms for Carolina Kids: Today, you registered for the pre-enrollment webinar. 1. Enroll – At www.cleanwaterforUSkids.org/carolina using the PIN and complete the enrollment surveys for three program sections: 1) lead in water, 2) lead-based paint, and 3) asbestos. 2. Evaluate Hazards – The program will ship your facility a water sample kit and may coordinate an on-site visit by a professional to assess for lead-based paint and asbestos hazards, if needed. 3. Receive Results – Receive your results, recommendations, and water mitigation support, if needed. 4. Request Reimbursement – If you choose to conduct lead-based paint or asbestos mitigation, you can request reimbursement from NC DHHS. 5. Communicate – Throughout the process, you can use our communication toolkit to communicate to your center’s community about the program. Research nature-based programs and spaces and develop a plan to embed these in licensed childcare in North Carolina; 3. Motivate and train early educators and specialists to promote best practices and increase time in outdoor play and learning in NC childcare facilities; 4. Increase time outdoors playing and learning in NC family child care homes through professional development and technical assistance. Today, we discussed ways to create a natural learning environment in the outdoor spaces. Some of the ideas included a music wall made from kitchen accessories, a garden that the children can help plant, a science center, and stepping stone made from logs.
