L. Stevens, Early Childhood Coordinator, assisted me with the visit. Your program currently operates with a five-star license, issued 3/24/25, earning 7 points in the education component, 7 points in the program standards component and 1 quality point for 75% of lead teachers have a BA/BS or higher in ECE/CD. The last annual compliance visit was conducted 4/2/25. The sanitation inspection was completed 11/18/25 with a “Superior” classification. The last fire inspection was conducted on 11/7/24 and your facility was approved for daytime care only. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety six percent as of 2/17/26. This facility is owned by Craven County Schools. We reviewed your facility contact information listed on the Division of Child Development and Early Education (DCDEE) website and all information is current. Contact me if you need to make any changes to your contact information, e.g., phone numbers, mailing address. Each indoor and outdoor space was monitored. Children throughout the facility were participating in group time, free play in activity areas, transitions, and personal care routines. In space #1, children were finishing breakfast that consisted of golden graham cereal, 100% Orange Tangerine juice, and milk. Children in space #2, were creating shapes with playdough. The children in space #3 transitioned inside from the outdoor space, washed their hands, and danced on the carpet to music. Each classroom space transitioned to the outdoor space. Children were observed playing on the stationary climber, playing in the sand area with toys, and riding on trikes in the outdoor space. The outdoor space was observed to have a basketball hoop, balls, and riding toys for the children to engage in gross motor activities. The developmental day requirements in section .2900 of the child care rules were monitored for compliance. Children who receive developmental day services are currently cared for in all spaces. Children’s individual plans for care and developmentally appropriate activity plans are followed, and opportunities for inclusion with children who are typically developing are offered. All children have had Dial-4 screenings completed. Lunch was observed and consisted of hamburger, French fries, mixed fruit, bun, and milk. The following violations were documented. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The fire inspection on file was dated 11/7/24. A current fire inspection was not available. 10A NCAC 09 .0304(a) 319 Staff/child ratios applicable to a classroom, were not posted in each classroom. Staff/child ratios were not posted in space #1, space #2, and space #3. .0713(a)(10), (c) & (f)(3); .2818(e) 601 Refrigerator(s) did not maintain a temperature of 45 degrees F. or below. In the refrigerator in space #2, the thermometer was reviewed and read forty-eight (48) degrees. 15A NCAC 18A .2806(j)(2) 1755 Certified developmental day center staff/child ratios and group sizes were not met. In space #1, nine (9) children, 3-5 years of age, were in care with one staff member. In space #2, eight (8) children, 4-5 years of age were in care with one staff member. .2902 (f) 1835 The medical action plan was not updated on an annual basis or when changes to the plan were made by the child's parent or health care professional. A child enrolled in space #2, had a seizure medical action plan on file that was not signed by a parent. .0801(b) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before 3/9/26, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware any information submitted by you is legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. In Certified Developmental Day classrooms, the staff to child ratio is 1:6. In space #1, nine (9) children, 3-5 years of age, were in care with one staff member. A staff member had left the classroom space while an occupational therapist was in the room working with children. We discussed that therapists do not count in meeting ratios. You stated that you thought that since she had a qualification letter that a staff member could leave the classroom while she was present. In space #2, eight (8) children, 4-5 years of age were in care with one staff member. A staff member left the classroom space to throw away an orange peel in the trashcan located in the hallway. We discussed the importance of remaining in the classroom until someone is available to allow her to leave the classroom. Remind all staff members that the ratio is 1:6 in all the classrooms and to call for assistance on the phones located in the classroom. Staff/ child ratios were not posted in space #1, space #2, and space #3. It was reported that the posting had fallen and were replaced during the visit to correct the violation. Conduct classroom observations to ensure that ratios are met at all times. We discussed that repeated staff child ratio violations could warrant an administrative action. In addition, a follow visit will be conducted. Medical Action Plan: Medicines can be crucial to the health and wellness of children. They can also be very dangerous if the wrong type or wrong amount is given to the wrong person or at the wrong time. A medical action plan is required for children that need emergency medication, the form needs to be completed by the parent or a health care professional. A child enrolled in space #2, had a seizure medical action plan on file that was not signed by a parent. It was reported that a medical action plan had been sent home several times and was not returned. We discussed that the school’s medical action plan has the required information, but it needed to be discussed with the parent and the parent sign it. Use the children’s file checklist to ensure all required documents are on file and document when they are received. Sanitation: Safe handling of all food is a basic principle to prevent and reduce foodborne illnesses. To prevent bacterial growth, refrigerator(s) must maintain a temperature of 45 degrees F or below. In the refrigerator in space #2, the thermometer was reviewed and read forty-eight (48) degrees. During the visit, the staff member adjusted the temperature settings, and the temperature rose according to the thermometer. The staff member added an additional thermometer to see if the thermometer is not working or if the refrigerator is not holding the correct temperature. You stated that you will check in with the staff member tomorrow to see if the refrigerator needs to be replaced or removed. The milk that was in the refrigerator was thrown away during the visit. Check your refrigerator temperatures daily to ensure they are working properly. Fire inspection: Facility safeguarding is not achieved by one agency carrying out a single regulatory program. Total safeguarding is achieved through a multiplicity of regulatory programs and agencies including sanitation and fire inspectors. Fire inspections should be completed annually and within 12 months of the previous inspection. The fire inspection on file was dated 11/7/24. A current fire inspection was not available. You stated that a fire inspection had been completed and contacted the fire inspector to send you a copy of the fire inspection. Set a reminder in your calendar to request a fire inspection in advance so that they are able to schedule an inspection before 12 months has passed. In space #1, a staff member read a story to the children on the carpet. Each classroom space transitioned to the outdoor space. Children were observed riding tricycles on a paved path, building with large blocks, and digging in the sand with sand toys in the outdoor space. The outdoor space was observed to have a large sand area, balls, and riding toys for the children to engage in gross motor activities. J. Jordan, Lead Teacher, assisted me with today’s visit. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. Children throughout the facility were participating in free play in activity areas and creating thanksgiving crafts. In space #1, a staff member read the book The Paper Bag Princess while the children sat on the carpet in child sized chairs. After the children played outside, they transitioned inside to wash their hands before sitting at the table for lunch. Lunch was observed and included breaded chicken on buns, tater tots, baked beans, pears, and milk. The outdoor space was observed to have a bike path with bikes, stationary climber, swings, large sand area, and balls for the children to engage in gross motor activities. Because Abuse and Neglect registries are also checked, it helps to ensure that children are protected from physical, mental, and sexual abuse. Performing diligent background screenings also protects the child care facility against future legal challenges. All owners of licensed child care facilities and employees working in a licensed child care center are required to submit a Criminal Background Check (CBC) and receive their Qualifying Letter prior to their first day of employment and complete a three-year requalification (on-line application and fingerprints) so that the new Qualifying Letter is on file before the expiration date listed on their current Qualifying Letter. Multiple instances of failing to complete Criminal Background Checks may result in an Administrative Action. Diana Acuna, hired 11/18/24, Rebekah Wagner, hired 11/12/24, and Mary Shine Williams, hired 8/5/24, did not have a criminal qualification letter on file to review. We discussed that these three staff members must get a qualifying letter before returning to work. We discussed that a substitute provider working in the classroom with children must have a background check on file. You stated that you will talk to the NC Pre-K coordinator about getting a background check for anyone that will be working in the preschool classrooms and discuss how HR can aid in the background check process. Use the staff file checklist and staff and training worksheet when hiring new staff members to ensure that documents are received on time. Fire inspection: Facility safeguarding is not achieved by one agency carrying out a single regulatory program. Total safeguarding is achieved through a multiplicity of regulatory programs and agencies including sanitation and fire inspectors. Fire inspections should be completed annually and within 12 months of the previous inspection. The last fire inspection was dated for 10/31/23. A current fire inspection was not available. You stated that the fire marshal came to the facility to complete and inspection but was called away on an emergency and has not returned yet to complete and inspection. Attach a copy of the Qualification Letter in your compliance verification email. We discussed that using a digital calendar to send reminders in advance of due dates of background checks and trainings will prevent lapses in qualifications. First Aid and CPR: First Aid and CPR are required within three months of hire and then every two years. This ensures the staff are skilled at up-to-date techniques for first aid or other lifesaving skills. It also provides a refresher so that you are confident to act in a medical emergency. A person with current CPR/First Aid certification is required to be on-site and/or off-site with children away from the center. Three staff members did not have First Aid certificates on file. We discussed that the CPR certificates on file do not reflect that first aid was completed and it is important to ensure that both CPR and First Aid certificates are current and on file. You stated that you were unaware that First Aid was not completed during the CPR class. Examine certifications carefully when receiving them to ensure that it is correct before documenting on the staff and training worksheet. Recognizing and Responding to Suspicions of Child Maltreatment: Recognizing and Responding to Suspicions of Child Maltreatment available at no charge on the Prevent Child Abuse NC website www.preventchildabusenc.org is a self-guided online training that teaches citizens and professionals the signs and symptoms to look out for with child abuse and neglect, what to do when they suspect it is happening, NC’s reporting law, and what happens after a report is made. It is required to be completed by new staff within three months of hire. It also counts towards an employee’s initial Health & Safety Training which must be fully completed within twelve months of hire. Once this course is completed, it or any other training related to child maltreatment may be completed every five years along with the other Health & Safety training topics. Two staff members have not completed child maltreatment training. A staff member hired on 9/25/23 did not complete child maltreatment training until 3/1/24. Once a staff member is hired, put the due date of the training in a digital calendar that will send you an alert in advance of the due date so that it will be completed in time. The last annual compliance visit was conducted 4/17/23. The sanitation inspection was completed 11/6/23 with a “Superior” classification. The last fire inspection was conducted 10/31/23 and your facility was approved for daytime care only. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety four percent as of 3/26/2024. This facility is owned by Craven County Schools. We reviewed your facility contact information listed on the Division of Child Development and Early Education (DCDEE) website and all information is current. Children throughout the facility were participating in group time, free play in activity areas, transitions, and personal care routines. In space #5, children were observed placing lima beans in a plastic sandwich bag with a moist paper towel. Children in space #4, were planting sunflower seeds in pots filled with soil. In space #1, children were observed shoveling soil into small pots and planting seeds in a sensory bin. The children in space #3 transitioned to the cafeteria for the children to eat lunch. Each classroom space transitioned to the outdoor space. Children were observed playing on the stationary climber, kicking balls, and swinging in the outdoor space. The outdoor space was observed to have a basketball hoop, balls, and riding toys for the children to engage in gross motor activities. Lunch was observed and consisted of spaghetti bake, mashed potatoes, mixed fruit, and milk. The following violation(s) were documented. Because Abuse and Neglect registries are also checked, it helps to ensure that children are protected from physical, mental, and sexual abuse. Performing diligent background screenings also protects the child care facility against future legal challenges. All owners of licensed child care facilities and employees working in a licensed child care center are required to submit a Criminal Background Check (CBC) and receive their Qualifying Letter prior to their first day of employment and complete a five-year requalification (on-line application and fingerprints) so that the new Qualifying Letter is on file before the expiration date listed on their current Qualifying Letter. Multiple instances of failing to complete Criminal Background Checks may result in an Administrative Action. M. Lovick is a Lead Teacher in one of the NCPK classrooms. She started working in the licensed classroom on 8/17/2022 and her criminal qualifying letter expired 3/6/24. She has completed an on-line application for a Criminal Background Check and results are pending. Because Ms. Lovick has not received a CBC Qualification Letter, she may not work in the licensed Pre-K classroom until she is qualified and has provided you with a Qualification Letter. We discussed that using a digital calendar to send reminders in advance of due dates of background checks and trainings will prevent lapses in qualifications. First Aid and CPR: First Aid and CPR are required within three months of hire and then every two years. This ensures the staff are skilled at up-to-date techniques for first aid or other lifesaving skills. It also provides a refresher so that you are confident to act in a medical emergency. A person with current CPR/First Aid certification is required to be on-site and/or off-site with children away from the center. Three staff members did not have First Aid certificates on file. We discussed that the CPR certificates on file do not reflect that first aid was completed and it is important to ensure that both CPR and First Aid certificates are current and on file. You stated that you were unaware that First Aid was not completed during the CPR class. Examine certifications carefully when receiving them to ensure that it is correct before documenting on the staff and training worksheet. Recognizing and Responding to Suspicions of Child Maltreatment: Recognizing and Responding to Suspicions of Child Maltreatment available at no charge on the Prevent Child Abuse NC website www.preventchildabusenc.org is a self-guided online training that teaches citizens and professionals the signs and symptoms to look out for with child abuse and neglect, what to do when they suspect it is happening, NC’s reporting law, and what happens after a report is made. It is required to be completed by new staff within three months of hire. It also counts towards an employee’s initial Health & Safety Training which must be fully completed within twelve months of hire. Once this course is completed, it or any other training related to child maltreatment may be completed every five years along with the other Health & Safety training topics. Two staff members have not completed child maltreatment training. A staff member hired on 9/25/23 did not complete child maltreatment training until 3/1/24. Once a staff member is hired, put the due date of the training in a digital calendar that will send you an alert in advance of the due date so that it will be completed in time.
