G. Turner, Operator, assisted me with today’s visit. Your program currently operates with a four-star license, issued 5/20/2024, earning 7 points in the education component, 2 points in the program standards component and 1 quality point for infant capacity restricted to no more than two children under one year of age. The last annual compliance visit was conducted 12/11/25. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety nine percent as of 7/29/25. We reviewed your facility contact information and per your statements, all information is current and correct. Contact me if you need to make any changes to your contact information, e.g., phone numbers, email address, mailing address. Check your facility email at least weekly to stay current with communication from DCDEE. The NC Secretary of State website was reviewed on 7/29/25 and this facility is not currently owned or operated by a corporation. Please contact me prior to any changes regarding ownership of this facility. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. Upon arrival, children were dancing to music before sitting at the table to color pictures with crayons and markers. The operator had circle time with the children that included a discussion about sea animals. After circle time, children transitioned outside and played under the covered porch. The outdoor space was observed to have a basketball hoop, riding toys, and balls for the children to engage in gross motor play. The following violation was documented. G. Turner, Operator, assisted me with today’s visit. Your program currently operates with a four-star license, issued 9/18/2020, earning 7 points in the education component, 2 points in the program standards component and 1 quality point for infant capacity restricted to no more than two children under one year of age. The last annual compliance visit was conducted 2/27/23. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety eight percent as of 7/25/23. We reviewed your facility contact information and per your statements, all information is current and correct. Contact me if you need to make any changes to your contact information, e.g., phone numbers, email address, mailing address. Check your facility email at least weekly to stay current with communication from DCDEE. The NC Secretary of State website was reviewed on 7/25/23 and this facility Is not currently owned or operated by a corporation. Please contact me prior to any changes regarding ownership of this facility. The license was posted, and the restrictions were in compliance. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. Children were observed playing in the outdoor space upon arrival. Children transitioned inside to wash their hands and sit at the table to color with crayons. The operator had circle time with the children that included singing songs and counting to ten. The outdoor space was observed to have a basketball hoop, riding toys, and balls for the children to engage in gross motor play. Lunch was observed and included ham, mixed vegetables, oranges, WW bread, and milk. The following violation was documented. Violation Number Comment Rule 2023 Operator and/or staff who work with children, did not complete health and safety training as part of on-going training so that every five years, all the topic areas were covered. The operator did not complete health and safety trainings by 6/27/23. .1703(d)(2) Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before 8/9/23, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. New staff are required to complete H&S Training within one year of hire (CPR/First Aid and Recognizing and Responding to Suspicions of Child Maltreatment within 3 months of hire) and then complete training in all topic areas every five years. Free H&S Training developed specifically for North Carolina is available in Moodle on the DCDEE website. Other options may be used if they are approved by DCDEE. I shared instructions on how to access the Moodle Health and Safety Trainings during the visit. These trainings can also be used to meet annual on-going training requirements. The operator did not complete health and safety trainings by 6/27/23. You stated that you were unable to log into Moodle to complete the trainings but will use the instructions that were given to her today to complete the trainings. Use the Health & Safety Training Log to document and track all H&S Training and attach certificate for each training topic area to log. Send the training log with your compliance letter to verify that all trainings were completed. ADDITIONAL COMMENTS/ REMINDERS: Rated License: Currently this center operates with a 4 Star License, issued September 18, 2020. The hold harmless in star rating for education will continue through 6/30/2024. All programs can voluntarily request Star Rating Assessment now to take advantage of the hold harmless period. Per our discussion, your facility is in Cohort 2 and your assessment year will be 7/1/25-6/30/26. You stated that you would start on your three-month self-study and send it to me when it is completed. I shared some self-study worksheets with you during the visit. Additional Caregiver: During the visit, I reviewed the staff file you are creating for an additional caregiver. Using the Additional Caregiver file checklist, I documented what you had on file and we discussed the documents that are needed before the caregiver starts.
