D. Rouse, Operator, assisted me with the visit. Your program currently operates with a four-star license, issued 11/27/2023, earning 7 points in the education component, 2 points in the program standards component and 1 quality point for having obtained an Infant/Toddler Certificate. The last annual compliance visit was conducted 10/7/24. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety eight percent as of 9/9/25. The NC Secretary of State website was reviewed on 9/9/25 and per your statements and the website, this facility is not currently owned or operated by a corporation. Please contact me prior to any changes regarding the ownership of this facility. We reviewed your facility contact information listed on the Division of Child Development and Early Education (DCDEE) website and all information is current. Contact me if you need to make any changes to your contact information, e.g., phone numbers, mailing address. You visited the indoor and outdoor spaces with me. Upon arrival, the operator was sitting at the table with the children in attendance while they read and colored pictures. The children transitioned to the outdoor center and ran and played on the see saw. The outdoor space was observed to have a basketball hoop, riding toys, and balls were observed for the children to engage in gross motor activities. A large, shaded area under the deck has sand tables and toys available for the children. The following violations was observed. Violation Number Comment Rule 1406 Authorization for up to 6 months to administer prescription or over-the-counter medication, when needed, for chronic medical conditions and for allergic reactions was not in writing and did not contain information required by rule 10 NCAC 09 .1720(c)(6)(A-I). Written permission for an Albuterol inhaler expired 2/23/25. 10A NCAC 09 .1720(b)(4) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. The operator assisted me with the visit. Your program currently operates with a four-star license, issued 11/27/2023, earning 7 points in the education component, 2 points in the program standards component and 1 quality point for having obtained an Infant/Toddler Certificate. The last annual compliance visit was conducted 11/9/23. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety eight percent as of 10/1/24. The NC Secretary of State website was reviewed on 10/1/24 and per your statements and the website, this facility is not currently owned or operated by a corporation. Please contact me prior to any changes regarding the ownership of this facility. We reviewed your facility contact information listed on the Division of Child Development and Early Education (DCDEE) website and all information is current. Contact me if you need to make any changes to your contact information, e.g., phone numbers, mailing address. You visited the indoor and outdoor spaces with me. No children were present during the visit. The children will arrive after the half day preschool program and schools they attend. The learning environment was observed to have a variety of toys and activities available. In the outdoor space, a basketball hoop, riding toys, and balls were observed for the children to engage in gross motor activities. A large shaded area under the deck has sand tables and toys available for the children. The following violations were documented. Violation Number Comment Rule 1401 Prescribed medications were not stored in their original containers in which they are dispensed with the pharmacy label. An Albuterol inhaler was not stored in a prescription labeled box. 10A NCAC 09 .1720(c)(1)&(2) 1897 Application did not have an a medical action plan attached for any child with health care needs such as allergies, asthma, or other chronic conditions that require specialized health services. A child enrolled did not have a medical action plan on file for asthma. .1721 (a )(4) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before 6/8/23, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. They can also be very dangerous if the wrong type or wrong amount is given to the wrong person or at the wrong time. A medical action plan is required for children that need emergency medication, the form needs to be completed by the parent or a health care professional. A child enrolled did not have a medical action plan on file for asthma. We discussed the importance of understanding the symptoms the child shows and the instructions to administer the Albuterol inhaler that is on site for the child. The Albuterol inhaler was not stored in a prescription labeled box. You stated that you will ask the parent for a box this afternoon and have them fill out the medical action plan. Ensure that when you accept emergency medication that you have the medical action plan in addition to the permission form.
