B. Rodgers, Administrator, assisted me with the visit. Your program currently operates with a five-star license, issued 11/27/23, earning 7 points in the education component, 6 points in the program standards component and 1 quality point for the administrator having at least 10 years of documented child care administration work experience in a licensed program. The last annual compliance visit was conducted 10/21/24. The sanitation inspection was completed 5/1/25 with a “Superior” classification. The last fire inspection was conducted on 5/7/25 and your facility was approved for daytime care only. The center's compliance history was reviewed with the operator. The program’s compliance history was eighty nine percent as of 10/7/25. The NC Secretary of State website was reviewed on 10/7/25 and Dayspring Ministries of New Bern, Inc. was listed as current- active. We reviewed your facility contact information listed on the Division of Child Development and Early Education (DCDEE) website and all information is current. Children were engaged in free play in activity centers and group time. A group of children were observed playing card games at the table with the staff member. In the block center, one child was building structures while another child used the balance board. The outdoor space was observed to have a stationary climber, swings, riding toys, and balls for the children to engage in gross motor activities. Lunch was observed and consisted of pizza, apples, peas, carrots, and milk. The following violation was documented. Violation Number Comment Rule 826 Gates to the fenced outdoor play area did not remain closed while children occupied the area. The handle of the gate located at the back of the outdoor space is broken and is unable to stay closed. .0605(i) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. The outdoor learning environment offers a sense of freedom for children. Children are able to play freely with peers, expand their imagination beyond restraints of indoor activities, release energy and explore their sense of touch, smell, taste and their sense of motion. It is important that the outdoor area is checked daily to ensure that the gates to the playground are closed, and proper upkeep is conducted to ensure the environment is safe for use. This standard helps to ensure proper supervision and protection, prevention of injuries, and control of the area. An effective fence is one that prevents a child from getting over, under, or through it and keeps children from leaving the fenced outdoor play area, except when supervising adults are present. Although fences are not childproof, they provide a layer of protection for children who stray from supervision. We reviewed the ABCMS portal and roster for your facility. We discussed that the roster shows the expiration date for the letters of each staff member in one place that can be added to a calendar for help with reminders. Add all staff members on the ABCMS portal and add the expiration dates to a calendar that is reviewed frequently. Fire Inspection: Facility safeguarding is not achieved by one agency carrying out a single regulatory program. Total safeguarding is achieved through a multiplicity of regulatory programs and agencies including sanitation and fire inspectors. Fire inspections should be completed annually and within 12 months of the previous inspection. A fire inspection was completed on 3/26/24 and the following fire inspection was completed on 5/7/25. You stated that you had a hard time finding someone to inspect the fire alarm system so that you could complete the inspection but have found a new company for the testing. We discussed setting a reminder in your calendar months in advance so that you have time to schedule the testing before the inspection and 12 months have passed. Additional Comments: NC Child Care Commission’s QRIS Modernization Plan (QRIS Reform): Today, we looked on the Division’s website at the proposed rules and pathways. Your program currently operates with a five-star license, issued 11/27/23, earning 7 points in the education component, 6 points in the program standards component and 1 quality point for the administrator having at least 10 years of documented child care administration work experience in a licensed program. The last annual compliance visit was conducted 11/13/23. The sanitation inspection was completed 6/28/24 with a “Superior” classification. The last fire inspection was conducted 3/26/24 and your facility was approved for daytime care only. The center's compliance history was reviewed with the operator. A large shade structure was available with a sand box underneath along with a stationary climber, swings, scooters, and a basketball hoop. Snack was observed and consisted of pretzels and peaches. Upon arrival, the children were in the outdoor space playing basketball and climbing on the stationary climber. The children transitioned inside, washed their hands, and were engaged in free play in activity centers. A group of children sat at the table and built structures with Legos, while another group of children played a game of Uno. The outdoor space was observed to have a basketball goal, stationary climber, swings, and riding toys for the children to engage in gross motor activities. Strawberries were observed growing in a raised garden. The following violation(s) were documented. All medications for children present at the center must have a current, completed medication permission form even if there is a Medical Action Plan. The medication permission forms provide additional information that could be critical to the administration of the medication and the care of the child. Medication permission forms for OTC, prescription, and emergency medication have an administration log as part of the form. This must be completed any time the medication is given. Medication permission forms are valid for a defined length of time and must be updated, if needed. In space #2, the written permission form for a Ventolin inhaler expired on 6/6/24. A child enrolled in space #2 had a medical action plan for asthma on file dated 6/2/23. In space #2, an Epi-Pen was on file without a medical action plan for food allergy. You stated that you had started updating medication forms in May and must have overlooked one and will have the parent complete the forms this afternoon. We discussed that medical action plans need to be updated annually and to check the dates when updating written permission forms to ensure that they are updated every year. The posted food allergy list was not updated to reflect that a child enrolled has a food allergy to shellfish. You stated that you do not serve shellfish in the facility, but we discussed that someone could bring in a shellfish product and need to be aware that there is a child enrolled that has an allergy to shellfish. During the visit, you updated that allergy list to reflect that there is an allergy to shellfish. Emergency medications were stored in an unlocked cabinet below five feet from the floor. You stated that the cabinet is supposed to be locked with the combination lock that is attached to the cabinet. We discussed that the emergency medications can be stored in a location that is five feet above the floor and is not required to be locked. Ensure that emergency medications are stored so that they are inaccessible to children. Transportation: Motor vehicle crashes are the leading cause of death in children two to fourteen years of age in the United States. Tire tread, the ribbed pattern on a tire that makes contact with the road, is important for a vehicle's safety, performance, and efficiency. Tread provides friction, which helps with acceleration, braking, and turning. It also helps keep a vehicle connected to the road and provides traction, which is especially important in dangerous conditions like rain or snow. The tire tread of the four (4) tires on the Ford 350 XLT Transit van was less than 2/32 of an inch. I used a penny to check the depth of the tires and the tread was flush with Lincoln’s head or below on all four tires. You stated that you use a tire service nearby to inspect and replace tires when needed and contacted them during the visit to schedule a appointment with them tomorrow. You stated that the tire service recommends using the wear bar as the indicator that tires need to be changed, but the tires are worn very close to the wear bar. The tires on the other vehicles have recently been replaced and have ample tread available. Check tires regularly to ensure that the tire tread is above 2/23 of an inch. Additional Comments: Natural Learning Initiative: Time outdoors is a critical tool for supporting mental and physical health in young children. Adoption of best practices for outdoor play and learning spaces is critical to increase daily time spent outside. The four components of this project include: 1. Relaunch a supportive network for outdoor play and learning environments in North Carolina; 2. Research nature-based programs and spaces and develop a plan to embed these in licensed childcare in North Carolina; 3. Motivate and train early educators and specialists to promote best practices and increase time in outdoor play and learning in NC childcare facilities; 4. Increase time outdoors playing and learning in NC family child care homes through professional development and technical assistance. Today, we discussed ways to create a natural learning environment in the outdoor spaces. Some of the ideas included a music wall made from kitchen accessories and a science center. B. Rodgers, Administrator, assisted me with today’s visit. Your program currently operates with a five-star license, issued 11/29/23, earning 7 points in the education component, 6 points in the program standards component and 1 quality point for the administrator having at least 10 years of documented child care administration work experience in a licensed program. The last annual compliance visit was conducted 11/13/23. The sanitation inspection was completed on 11/6/23 with a “Superior” classification. The last fire inspection was conducted 3/26/24. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety five percent as of 6/4/24. The NC Secretary of State website was reviewed on 6/4/24 and Dayspring Ministries of New Bern, Inc. was listed as current- active. All indoor and outdoor areas were monitored. B. Rodgers, Administrator, arrived during the visit. I observed children in both the indoor and outdoor learning environments. Upon arrival, the children were in the outdoor space playing basketball and climbing on the stationary climber. The children transitioned inside, washed their hands, and were engaged in free play in activity centers. A group of children sat at the table and built structures with Legos, while another group of children played a game of Uno. The outdoor space was observed to have a basketball goal, stationary climber, swings, and riding toys for the children to engage in gross motor activities. Strawberries were observed growing in a raised garden. The following violation(s) were documented. Violation Number Comment Rule 508 Special diet or food allergy information was not posted where they can be seen in food preparation and eating areas. The posted food allergy list was not updated to reflect that a child enrolled has a food allergy for shellfish. .0901(g) 841 Medications including prescription and non-prescription items were not stored in a locked cabinet or other locked container. Emergency medications were stored in an unlocked cabinet below five feet from the floor. 15A NCAC 18A .2820(d) 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. In space #2, the written permission form for a Ventolin inhaler expired on 6/6/24. .0803(12) 1123 All vehicles used to transport children were not free of hazards. The tire tread of the four (4) tires on the Ford 350 XLT Transit van were less than 2/32 of an inch. 10A NCAC 09 .1002(a) 1834 Application did not have a medical action plan attached for any child with health care needs such as allergies, asthma, or other chronic conditions that require specialized health services. In space #2, an Epi-Pen was on file without a medical action plan for food allergy. .0801(b) 1835 The medical action plan was not updated on an annual basis or when changes to the plan were made by the child's parent or health care professional. A child enrolled in space #2 had a medical action plan for asthma on file dated 6/2/23. .0801(b) Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before 6/26/24, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit may be completed. An example is: Jane Doe, Administrator AAA Child Care ID # 12345678 Technical assistance with violations: Medication: Administering medication requires skill, knowledge, and careful attention to detail. All medications for children present at the center must have a current, completed medication permission form even if there is a Medical Action Plan. The medication permission forms provide additional information that could be critical to the administration of the medication and the care of the child. Medication permission forms for OTC, prescription, and emergency medication have an administration log as part of the form. This must be completed any time the medication is given. Medication permission forms are valid for a defined length of time and must be updated, if needed. In space #2, the written permission form for a Ventolin inhaler expired on 6/6/24. A child enrolled in space #2 had a medical action plan for asthma on file dated 6/2/23. In space #2, an Epi-Pen was on file without a medical action plan for food allergy. You stated that you had started updating medication forms in May and must have overlooked one and will have the parent complete the forms this afternoon. We discussed that medical action plans need to be updated annually and to check the dates when updating written permission forms to ensure that they are updated every year. The posted food allergy list was not updated to reflect that a child enrolled has a food allergy to shellfish. You stated that you do not serve shellfish in the facility, but we discussed that someone could bring in a shellfish product and need to be aware that there is a child enrolled that has an allergy to shellfish. During the visit, you updated that allergy list to reflect that there is an allergy to shellfish. Emergency medications were stored in an unlocked cabinet below five feet from the floor. You stated that the cabinet is supposed to be locked with the combination lock that is attached to the cabinet. We discussed that the emergency medications can be stored in a location that is five feet above the floor and is not required to be locked. Ensure that emergency medications are stored so that they are inaccessible to children. Transportation: Motor vehicle crashes are the leading cause of death in children two to fourteen years of age in the United States. Tire tread, the ribbed pattern on a tire that makes contact with the road, is important for a vehicle's safety, performance, and efficiency. Tread provides friction, which helps with acceleration, braking, and turning. It also helps keep a vehicle connected to the road and provides traction, which is especially important in dangerous conditions like rain or snow. The tire tread of the four (4) tires on the Ford 350 XLT Transit van was less than 2/32 of an inch. I used a penny to check the depth of the tires and the tread was flush with Lincoln’s head or below on all four tires. You stated that you use a tire service nearby to inspect and replace tires when needed and contacted them during the visit to schedule a appointment with them tomorrow. You stated that the tire service recommends using the wear bar as the indicator that tires need to be changed, but the tires are worn very close to the wear bar. The tires on the other vehicles have recently been replaced and have ample tread available. Check tires regularly to ensure that the tire tread is above 2/23 of an inch. Additional Comments: Natural Learning Initiative: Time outdoors is a critical tool for supporting mental and physical health in young children. Adoption of best practices for outdoor play and learning spaces is critical to increase daily time spent outside. The four components of this project include: 1. Relaunch a supportive network for outdoor play and learning environments in North Carolina; 2. Research nature-based programs and spaces and develop a plan to embed these in licensed childcare in North Carolina; 3. Motivate and train early educators and specialists to promote best practices and increase time in outdoor play and learning in NC childcare facilities; 4. Increase time outdoors playing and learning in NC family child care homes through professional development and technical assistance. Today, we discussed ways to create a natural learning environment in the outdoor spaces. Some of the ideas included a music wall made from kitchen accessories and a science center.
