You, Evelyn Cherry, Owner, were present for consultation during the visit. Your program currently operates with a 3-Star license, issued July 22, 2024. The facility’s compliance history was reviewed with the operator. The program’s compliance history was one hundred (100) percent as of January 6,2026. Restrictions on the permit include 1st, 2nd, and 3rd shift care, a maximum of five (5) preschool children cared for at any time, and the facility serves no more than five (5) preschoolers and three (3) school age children at any time The license was posted, and the restrictions were in compliance. A walk-through of your facility was completed today; all indoor and outdoor areas were monitored. Four (4) children between ages zero (0) and twelve (12) are enrolled and three (3) were present today. The indoor space was a safe environment, and a variety of activities were available for the children. All children were adequately supervised. Appropriate toys and materials were observed in this area. A bathroom is in close proximity to the childcare space. Upon arrival at your facility, you and the children were in the childcare space. One (1) preschool child was engaged in a variety of free play activities which included wooden animals, stacking cubes, blocks, and people. The infant was riding an indoor infant riding car. You fed the infant a morning snack which consisted of peaches and a bottle of milk. The preschool child had a snack which consisted on noodles, apples, and water. Safe sleep checks, toileting, handwashing, and food service requirements were monitored, and requirements were met. The outdoor space had a variety of gross motor equipment which included tricycles, small bicycles, large balls, a basketball goal, activity tables, a large swing set with a slide, and a table with an umbrella. Only the slide on the swing set is used for play. The facility’s Shaken Baby and Abusive Head Trauma and Safe Sleep Policies were reviewed, and requirements were met. Name of Operation: ALL MY CHILDREN CHILDCARE HOME Facility ID: 25000361 Consultant: ANDREA JOHNSON Operation Type: Family CC Home Case Number: Visit Date: 1/6/2026 Number Present: 3 Completed Date: 1/6/2026 Age: From 0 To 12 Total Minutes: 120 Time In: 09:30 AM Time Out: 11:30 AM Time In: Time Out: List to Use: Family CC Home Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced The purpose of today’s visit was to monitor your program for compliance with applicable child care requirements for a routine unannounced visit. You, Evelyn Cherry, Owner, were present for consultation during the visit. Your program currently operates with a 3-Star license, issued July 22, 2024. The facility’s compliance history was reviewed with the operator. The program’s compliance history was one hundred (100) percent as of January 6,2026. Restrictions on the permit include 1st, 2nd, and 3rd shift care, a maximum of five (5) preschool children cared for at any time, and the facility serves no more than five (5) preschoolers and three (3) school age children at any time The license was posted, and the restrictions were in compliance. A walk-through of your facility was completed today; all indoor and outdoor areas were monitored. Four (4) children between ages zero (0) and twelve (12) are enrolled and three (3) were present today. The indoor space was a safe environment, and a variety of activities were available for the children. All children were adequately supervised. Appropriate toys and materials were observed in this area. A bathroom is in close proximity to the childcare space. Upon arrival at your facility, you and the children were in the childcare space. One (1) preschool child was engaged in a variety of free play activities which included wooden animals, stacking cubes, blocks, and people. The infant was riding an indoor infant riding car. You fed the infant a morning snack which consisted of peaches and a bottle of milk. The preschool child had a snack which consisted on noodles, apples, and water. Safe sleep checks, toileting, handwashing, and food service requirements were monitored, and requirements were met. The outdoor space had a variety of gross motor equipment which included tricycles, small bicycles, large balls, a basketball goal, activity tables, a large swing set with a slide, and a table with an umbrella. Only the slide on the swing set is used for play. The facility’s Shaken Baby and Abusive Head Trauma and Safe Sleep Policies were reviewed, and requirements were met. Today, you reported that you are currently not providing transportation. Violation Number Comment Rule 2054 Home was not free of lead poisoning hazards as define in G.S. 130A-131.7(7). This facility has not completed the required testing for lead in water and lead-based paint through the Clean Classrooms for Carolina Kids. 10A NCAC 09 .1707(2) 2057 The family child care home was not free of asbestos hazards as defined in G.S. 130A-444(2) and 10A NCAC 09 .0102(4). This facility has not completed the required testing for asbestos through the Clean Classrooms for Carolina Kids. 10A NCAC 09 .1702(2) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violations documented may impact the compliance history score. Any violations documented must be corrected immediately. On or before Tuesday, January 20, 2026, I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected.
