Currently, this facility operates with a Four (4) Star License issued June 16, 2021, earning two (2) points in Program Standards, seven (7) points in Staff Education, and one (1) quality point. The last sanitation inspection was completed on October 3, 2025 earning a “Superior” classification. The last fire inspection was completed on February 6,2026 with a “Satisfactory” classification and was approved for day time and night care. The last annual compliance visit was completed June 3, 2025. You, P. Swain, Owner/Operator, was available to assist with the visit. Fifteen(15) children were present age ranging zero (1) to five (5) years of age. Children were engaged in free choice play, teacher directed activities, and toileting and handwashing routines. All staff interacted with children in a positive and nurturing manner. I used the Annual Compliance Monitoring Checklist for Child Care Centers and the Child Care Center Item Number Listing as a basic monitoring tool to assess compliance with all applicable child care requirements pertinent to this facility. Ten percent of staff records and ten percent of children’s records, and all program records were monitored. All children, staff, and program records were found to be in compliance. Three (3) violations were documented during the visit. The following violations of child care requirements were documented today. Violation Number Comment Rule 533 Human milk, formula and other bottled beverages including sippy cups, sent from child's home were not fully prepared, dated, and labeled for the appropriate child. Four (4) sippy cups, and two (2) bottles containing milk were unlabeled in the refrigerator stored in the space where infants and toddlers were cared for. 15A NCAC 18A .2804(d) 841 Medications including prescription and non-prescription items were not stored in a locked cabinet or other locked container. One (1) bottle of adult Tylenol was observed in the drawer, under the changing table, in the space where infants and toddlers were cared for. 15A NCAC 18A .2820(d) 858 Plastic bags, materials that could be torn apart and toy parts small enough to be swallowed were accessible to children under three years of age. One (1) plastic grocery bag, and three (3) Ziploc bags were observed in a drawer, under the changing table, in the space where infants and toddlers were cared for. .0604(q) Child Care Licensing Requirements are established to ensure a safe, healthy, and developmentally appropriate environment for all child care facilities. Therefore, it is important for you to be knowledgeable of all licensing requirements that apply to your facility. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy five percent. It is your responsibility to maintain compliance with the applicable laws and rules; always whether they have been monitored or specifically discussed with you in the past. The best way for you to ensure that you are meeting all requirements is to periodically review the childcare laws and rules on the Division of Child Development and Early Education (DCDEE) website. NUTRITION Four (4) sippy cups, and two (2) bottles containing milk were unlabeled in the refrigerator stored in the space where infants and toddlers were cared for. Today, we did a review of the forms and requirements, and discussed any questions and concerns you still have pertaining to Pathway 2. You stated you are still waiting for one staff’s information to be updated in DCDEE WORKS. On or before August 24, 2023, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware any information submitted by you is accurate documentation. If any information provided in the letter is not accurate, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted. COMPLIANCE HISTORY Prior to today's visit, this facility has maintained an 18-month compliance score of 95%. The compliance history score was printed and you were given a copy at the conclusion of the visit. TECHNICAL ASSISTANCE/ CONSULTATION During the consultation, we reviewed violation(s) documented and the checklist used during the monitoring process. CHILDREN’S RECORD KEEPING One (1) child whom you stated was a drop-in child, only had an application on file. The documents that are required at the first day of enrollment were not on file for review. We discussed the children’s file checklist that details the required documents and when they are due. You stated you did not know all the documents were required for a drop-in child. The child’s folder was missing a signed and dated statement of receipt of the Summary of the Child Care Law, receipt of operational policies and parent participation plan, signed and dated Discipline policy, notification of smoking and tobacco policy, and a signed and dated statement by the parent of receipt of Shaken Baby and Abusive Head Trauma policy. One (1) child’s application was missing the parent’s signature. The parent came in the center to sign the application during the visit. STORAGE OF HAZARDOUS ITEMS One (1) can of Lysol aerosol spray was observed in the room serving infants, in an unlocked cabinet, under the changing table. I explained that any aerosol container needs to be stored in a locked space or cabinet. The Lysol spray was removed and locked in a cabinet in another room. STAFF RECORD KEEPING One (1) staff member did not have Recognizing and Responding to Suspicions of Child Maltreatment within the requires 90 days after date of employment. The staff member was hired on January 1, 2023 and should have taken the training by April 1, 2023. You stated the staff member is a substitute and you and you were unaware that she needed the training. I explained that Recognizing and Responding to Suspicions of Child Maltreatment is required for all staff working with the children. The same staff member did not complete CPR and First Aid training before it expired. The training expired on August 9, 2023. I explained that any staff member who is left alone with the children is required to have an updated CPR and First Aid Training on file for review by the Division. One (1) staff member’s Health Questionnaire and Emergency Information had not been completed for the current year. I explained that the Health Questionnaire and the Emergency Information for each staff member should be updated annually to maintain compliance with child care requirements. The staff member updated both documents during the visit. RESOURCES HEALTH AND SAFETY TRAININGS DCDEE has partnered with the NC Child Care Health and Safety Resource Center to offer health and safety trainings in MOODLE. These trainings will remain at no charge for staff. The contract with ProSolutions ended on September 30, 2020. Health and safety trainings are due every five (5) years, including the Recognizing and Responding to Suspicions of Child Maltreatment. The Recognizing and Responding to Suspicions of Child Maltreatment training must be completed via the Prevent Child Abuse NC website. The enrollment process and completion of course materials tend to work best with a windows laptop or desktop using Firefox, but other devices and Google Chrome should also work. You can access the health and safety trainings available in Moodle and Frequently Asked Questions by logging in using NCID and completing the following steps: • From the Moodle home page, students need to log-in with their NCID and password. The log-in is in the upper right corner of the screen. If you do not have a current log-in you will need to create one. • After log-in, the student should scroll to the bottom of the page. In the ‘Search courses’ box, the student should type in the short name “Year 2023 CCDF” and then click go. The student should not register for prior year courses as they are no longer available. • After completing the course search for “Year 2023 CCDF”, the student should be able to register for the course. • Once registered, the student will then need to use the “ADDITIONAL DIRECTIONS” for step-by-step instructions. Should a student encounter an issue, we are requesting they report the CCDF 2023 course issue (i.e. they are unable to complete the course evaluation or receive a training certificate), to the North Carolina Child Care Health and Safety Resource Center using the following link https://healthychildcare.unc.edu/contact-us-form/. STAR RATED LICENSE I informed you information regarding the preparation for the star rated license assessment process is located on the Division of Child Development (https://ncchildcare.ncdhhs.gov/) and North Carolina Rated License Project website. I recommended you and staff visit ncrlap.org to obtain detailed information about how the process will be implemented and review prep year activities and ideas offered. More information will be provided regarding your programs rated license assessment timeframe. INFANT-TODDLER EDUCATOR AWARD$ PLUS PROGRAM: Infant-Toddler Educator AWARD$® Plus is a salary supplement program, available in all NC counties, designed to increase the education, retention and compensation of teachers and family childcare educators working with our youngest children. The Infant Toddler Educator AWARD$ program has recently gone through some exciting programmatic updates. The original AWARD$ program was only available to teachers working full-time in B-3 classrooms that had an AA in ECE or higher degree and were employed by 4- and 5-star programs. The education requirement has been reduced as an incentive to support teachers seeking to increase their education in any star rated licensed program. To be eligible for AWARD$ Plus, applicants must: • Work at least 30 hours/week with infants, ones, or twos. • Earn at or below $20/hour. • Work in a licensed child care center or home. • Have at least 12 semester hours of birth to five focused coursework.
