You, Dawn Fraley, Operator, assisted with today’s visit. A checklist was used to note the requirements I monitored today. Your program currently operates with a three (3) star license issued August 28, 2025. The permit restrictions were in compliance including First Shift (daytime care), maximum of 5 preschool children at any time, fireplace/woodstove not used during operating hours, and no more than two (2) children under one (1) year of age. Your facility uses Gee Whiz curriculum. The Secretary of State website was checked on May 19, 2026, and your business, Dawn Fraley’s Home Day Care, was not listed. If any changes to the corporation need to be made or you decide to sell your business, then you must notify me at least thirty days prior to any changes occurring. The license cannot be bought, sold, subleased, transferred to another person or location, or inherited. A walk-through of the facility was completed today and all indoor and outdoor areas were monitored. I observed children in the indoor learning environment. Children throughout the facility were participating in routines, breakfast, nap, and free play during the visit. The last fire drill was conducted on May 4, 2026. The last emergency drill documented was conducted on April 15, 2026. The last playground inspection was conducted on May 6, 2026. You received the results of your most recent lead water testing showing no levels of lead above the allowed amount, on August 20, 2024. Per the facility data on the Clean Classrooms for Carolina Kids website, your facility is exempt from lead-based paint and asbestos testing. The following violations were observed/documented during today’s visit: Violation Number Comment Rule 706 Corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, and products under pressure in an aerosol dispenser and any substance that may be hazardous to a child if ingested, inhaled, or handled were not kept in locked storage when children were in care. One (1) bag of Hi Mini Bath Bombs with multiple warnings, was on a shelf above the toilet in the restroom used by children, accessible to children. One (1) bottle of Myers Clean Day Multi Surface Everyday Cleaner, with multiple warnings, was on the counter top to the left of the handwashing sink in the restroom used by children, accessible to children. .1719 (a)(7) 911 Copy of each child's immunization record, including the operator's own preschool child(ren), was not on file. One (1) child, enrolled on April 13, 2026, did not have an immunization record on file. GS 110-91(1); .1721(a)(2) 1405 Authorization for up to 12 months to apply over-the-counter medications, topical ointment, topical teething ointment or gel, insect repellents, fluoridated toothpaste, lotions, creams, and powders; such as sunscreen, diapering creams, baby lotion, and baby powder, to a child, when needed, was not in writing and did not contain information required by rule .1720(b)(7). There was no permission to administer medication form on file for one (1) tube of A&D Diaper Rash Ointment and Skin Protectant. 10A NCAC 09 .1720(c)(4) 1990 Infant feeding plans did not include the required information, including but not limited to the type and amount of milk, formula and food, and the frequency of feedings, and/or was not available for reference. The infant feeding plan for one (1) infant, nine (9) months of age, did not have the signature of the parent or date the form was completed and was not updated to include feeding baby food to the infant. The infant was fed baby food during the visit. .1706(i) The violations documented must be corrected immediately. On or before June 2, 2026, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected, and how compliance will be maintained in the future. Include the violation item number, statement of compliance, date of the visit, and license ID number in the letter. Please be aware any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, it may be considered falsification of information. If sufficient information is not received by the due date, an unannounced follow-up visit may be conducted. The Division of Child Development may take administrative action against the license and/or impose civil penalties based on the failure of the operator to correct any documented violations within the established time period. All hazardous items with multiple warnings and items in aerosol containers must be kept in locked storage. Items labeled “keep out of reach of children” with no other warnings can be stored five (5) feet off the ground. I suggested you check the bathroom used by children each morning before the child care children arrive to ensure all items are stored properly. The hazardous products were removed from the restroom and placed in locked storage during the visit. You stated hazardous products will remain in the locked cabinet under the kitchen sink. 2. The family child care home operator shall maintain the following health records for each enrolled child, including his or her own child(ren) who are not school-age: a copy of the child's immunization record. I suggested you ask parents to provide the immunization record or exemption during the enrollment process to ensure the documentation is on file in a timely manner. I also suggested you document all required documentation on the children’s file checklist found on the DCDEE website under the “Provider” tab to ensure all required documentation is on file. 3. 3. The following provisions apply to the administration of medication in family child care homes: No prescription or over-the-counter medication and no topical, non-medical ointment, repellent, lotion, cream, fluoridated toothpaste or powder shall be administered to any child: without written authorization from the child's parent; without written instructions from the child's parent, physician or other health professional; in any manner not authorized by the child's parent, physician or other health professional; after its expiration date; for non-medical reasons, such as to induce sleep; or with a known allergy to the medication. I suggested you keep blank copies of the permission to administer medication form near the locked box used to store medication and have the parent complete the form immediately when a medication is brought into the facility. I also suggested you keep the completed permission to administer medication forms in the locked box used to store medication to ensure they are on file and available to review. 4. The parent or health care professional of each child under 15 months of age shall provide the operator an individual written feeding plan for the child. This plan shall be followed at the home. This plan shall include the child's name, be signed by the parent or health care professional, and be dated when received by the operator. Each infant's plan shall be modified in consultation with the child's parent or health care professional to reflect changes in the child's needs as he or she develops. The feeding plans for each infant shall include the type and amount of milk, formula and food, the frequency of feedings and be available for reference by the operator. I suggested you review the infant feeding plan with each infant’s parent on a regular basis to ensure it is up to date. Consultation: We discussed the QRIS Modernization and the Pathway to the Stars. The most recent fire drill was conducted on November 5, 2025, the most recent emergency drill was conducted on October 15, 2025, and the most recent playground inspection was conducted on November 5, 2025. You received the results of your lead water testing showing no levels of lead above the allowed amount, on August 20, 2024. Per the facility data on the Clean Classrooms for Carolina Kids website, your facility is exempt from lead-based paint and asbestos testing. A walk-through of the facility was completed today and all indoor and outdoor areas were monitored. I observed children in the indoor learning environment. Children throughout the facility were participating in routines, breakfast, group time, and free play during the visit. Limited monitoring of your file was conducted for the purpose of reviewing training regarding CPR/First Aid, EPR, Playground Safety, ITS-SIDS, Special Training, and Criminal Background Checks. Criminal background checks were monitored for all household members. The following violation was observed/documented during the visit: Violation Number Comment Rule 706 Corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, and products under pressure in an aerosol dispenser and any substance that may be hazardous to a child if ingested, inhaled, or handled were not kept in locked storage when children were in care. The cabinet under the sink in the kitchen, containing many aerosol air fresheners, cleaning products, and pesticides, was unlocked and accessible to children. In the restroom, used by children, one (1) bottle of Lysol Power Clinging Gel Toilet Bowl Cleaner, with multiple warnings, was on top of the shelf above the toilet, accessible to children. In the restroom used by children, one (1) container of Member’s Mark Disinfecting Wipes, with multiple warnings, was on the counter to the right of the handwashing sink, accessible to children. .1719 (a)(7) You must maintain at least 75% Compliance History for each 18-month period as required by G.S. 110-90(4) (c). Prior to today's visit, your program maintained a compliance history of 94%. Please note any violations cited during future visits will negatively impact your compliance history. Repeated violations or violations left unresolved may lead to an administrative action. Technical assistance was provided on the following: 1. All hazardous items with multiple warnings and items in aerosol containers must be kept in locked storage. Items labeled “keep out of reach of children” with no other warnings may be stored five (5) feet off the ground. I suggested you check all areas of your home used for child care thoroughly each morning, prior to children arriving, to ensure all items are stored properly. I also suggested you check warning labels on all products to ensure they are stored properly. The hazardous products in the restroom used by children were moved to the cabinet under the kitchen sink, and the cabinet under the kitchen sink was locked during the visit. I suggested you provide parents with a copy of the permission to administer medication form prior to a medication being brought into the facility to ensure the form is completed when the medication is accepted. You, Dawn Fraley, Operator, assisted me with today’s visit. Your program currently operates with a three (3) star license issued December 22, 2022. The permit restrictions were in compliance including First Shift (daytime care), maximum of 5 preschool children at any time, fireplace/woodstove not used during operating hours, and no more than two (2) children under one (1) year of age. The Secretary of State website was checked on January 7, 2025, and your business, Dawn Fraley’s Home Day Care, was not listed. If any changes to the corporation need to be made or you decide to sell your business, then you must notify me at least thirty days prior to any changes occurring. The license cannot be bought, sold, subleased, transferred to another person or location, or inherited. The most recent fire drill was conducted on May 10, 2024, the most recent emergency drill was conducted on March 5, 2024, and the most recent playground inspection was conducted on May 20, 2024. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in the indoor learning environment. Children throughout the facility were participating in routines, lunch, and free play during the visit. Limited monitoring of your staff file was conducted for the purpose of reviewing training regarding CPR/First Aid, EPR, Playground Safety, ITS-SIDS, Special Training, and Criminal Background Checks. The following violations were observed/documented during the visit: Violation Number Comment Rule 714 Monthly check for hazards on the outdoor play area was not completed using a form supplied by the Division. The most recent playground inspection was conducted on May 10, 2024. 10A NCAC .1721(e)(5)(A-F) 1403 Prescription or over-the-counter medication and topical, non-medical ointment, repellant, lotion, cream, fluoridated toothpaste, or powder was administered to any child without the required information. There were no permission to administer mediation forms on file for two (2) containers of Triple Paste Skin Protectant, two (2) containers of Boudreauxs Butt Paste Max Strength Diaper Rash Ointment, one (1) tube of Boudreauxs Butt Paste Max Strength Diaper Rash Ointment, one (1) tube of Boudreauxs Butt Paste Original Diaper Rash Ointment, and one (1) tube of Desitin Maximum Strength Diaper Rash Paste, all of which the operator confirmed had been administered at the facility. 10A NCAC 09 .1720(c)(1&2) 1821 Caregivers did not document compliance with visually checking on sleeping infants aged 12 months or younger and/or the documents were not maintained for a minimum of 30 days. There was no documentation of visual sleep checks on file for one (1) infant, nine (9) months of age, enrolled on June 21, 2024, prior to December 16, 2024. .1724(a)(8)&(f) 1853 The operator did not conduct a monthly fire drill. The most recent fire drill was conducted on May 10, 2024. .1719(a)(15) & .1721( e)(2) 1854 The operator did not conduct a quarterly lockdown or shelter-in-place drill and or the drill record was incomplete. The most recent shelter-in-place or lockdown drill was conducted on March 5, 2024. .1719(a )(16) & .1721(e )(7) The violations documented must be corrected immediately. On or before January 21, 2025, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected, and how compliance will be maintained in the future. Include the violation item number, statement of compliance, date of the visit, and license ID number in the letter. Please be aware any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, it may be considered falsification of information. If sufficient information is not received by the due date, an unannounced follow-up visit may be conducted. The Division of Child Development may take administrative action against the license and/or impose civil penalties based on the failure of the operator to correct any documented violations within the established time period. A monthly check for hazards on the outdoor play area must be conducted and documentation must be maintained and available for review. I suggested you set a calendar reminder to ensure a playground inspection is conducted at least once per month. Once the inspection is conducted, complete the playground inspection form and place in your facility binder. 2. No prescription or over-the-counter medication and no topical, non-medical ointment, repellent, lotion, cream, fluoridated toothpaste or powder shall be administered to any child: without written authorization from the child's parent; without written instructions from the child's parent, physician or other health professional; in any manner not authorized by the child's parent, physician or other health professional; after its expiration date; for non-medical reasons, such as to induce sleep; or with a known allergy to the medication. No medication should be accepted without an accompanying permission to administer medication form. I suggested you provide parents with a copy of the permission to administer medication form prior to a medication being brought into the facility to ensure the form is completed when the medication is accepted. I confirmed you had extra copies of the permission to administer medication form to provide to parents. 3. You must visually check sleeping infants aged twelve (12) months or younger at least every fifteen (15) minutes and you must document compliance with visually checking on sleeping infants. Documentation that verifies compliance with visual checks on infants must be maintained for a minimum of one (1) month. I suggested you place each page of visual sleep check documentation in your facility binder once the page on your clipboard is full to ensure documentation is maintained and on file for review. 4. The operator of a family child care home (operator) shall provide a physically safe and healthy indoor and outdoor environment that meets the developmental needs of children in care by conducting a monthly fire drill. I suggested you set a calendar reminder each month to ensure a fire drill is conducted at least monthly. 5. The operator of a family child care home (operator) shall provide a physically safe and healthy indoor and outdoor environment that meets the developmental needs of children in care by conducting a shelter-in-place or lockdown drill at least once every three (3) months. Consultation: We reviewed child care rule 10A NCAC 09 .1725 SANITATION REQUIREMENTS FOR FAMILY CHILD CARE HOMES (a) To assure the health of children through proper sanitation, the family child care home operator shall: (1) collect and submit samples of water from each well used for the children's water supply for bacteriological analysis to the local health department or a laboratory certified to analyze drinking water for public water supplies by the North Carolina State Laboratory of Public Health every two years. Results of the analysis shall be on file in the home; (2) collect and submit samples of water from each water outlet used for drinking or food preparation for lead analysis to the local health department or a laboratory certified to analyze for lead in drinking water by the North Carolina State Laboratory of Public Health. Results of the analysis shall be on file in the home. For operators that submit an application for licensure after the effective date of this Rule, water samples shall be collected by the operator and tested during the application process. Per rule and your facility’s safe sleep policy, sleeping infants must be visually checked at least once every fifteen (15) minutes and the check documented on a safe sleep log. Consultation: We reviewed child care rule 10A NCAC 09 .1725 SANITATION REQUIREMENTS FOR FAMILY CHILD CARE HOMES (a) To assure the health of children through proper sanitation, the family child care home operator shall: (1) collect and submit samples of water from each well used for the children's water supply for bacteriological analysis to the local health department or a laboratory certified to analyze drinking water for public water supplies by the North Carolina State Laboratory of Public Health every two years. Results of the analysis shall be on file in the home; (2) collect and submit samples of water from each water outlet used for drinking or food preparation for lead analysis to the local health department or a laboratory certified to analyze for lead in drinking water by the North Carolina State Laboratory of Public Health. Results of the analysis shall be on file in the home. For operators that submit an application for licensure after the effective date of this Rule, water samples shall be collected by the operator and tested during the application process. You, Dawn Fraley, Operator, assisted me with today’s visit. A checklist was used to note the requirements I monitored today. I observed you and five (5) children present during activities and breakfast today. You use the living room, kitchen, and bathroom for childcare. The following violation was observed/documented during today’s visit: Violation Number Comment Rule 705 All medicines, including refrigerated and unrefrigerated, were not kept in locked storage.
