K. Bare, Administrator, was not present but K. Jarvis, Lead Teacher, assisted me with the visit. Your program currently operates with a Notice of Compliance (NOC) effective 10/18/2025. Please contact me if you are ever interested in becoming fully licensed to earn a star rating. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety percent as of June 5, 2026. The NC Secretary of State website was reviewed on June 5, 2026, and Kingdom Impact Kids Academy, LLC was listed as current-active. The Notice of Compliance was not posted but is not required to be. Restrictions were in compliance. A walk-through of the facility was completed today; all indoor and outdoor areas were monitored. I observed children in the classroom. Supervision and staff/child ratios were in compliance. Children from both classrooms were grouped in Space 1 and were sleeping on linen-covered mats. When the children woke up, they used the bathroom and washed hands before afternoon snack. All food and beverages are brought from home. As they finished snack, they washed their hands and played with toys and materials available in the classroom. The teachers interacted warmly with the children, meeting their developmental needs. The lead teacher reported that no children are using any medication. Since the last visit, a sanitation inspection was completed on April 15, 2026. I observed and documented the following violation during today’s visit. Violation Number Comment Rule 716 All stationary equipment, more than 18 inches high, was not installed over protective surfacing. Grass and weeds were growing in the fall zone of the complex climber. .0605(j) * Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violations documented may impact the compliance history score. The violations documented must be corrected immediately. On or before June 22, 2026, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware that any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted. An example is: Jane Doe, Administrator AAA Child Care ID # 12345678 Technical Assistance With Documented Violation: Surfacing – Maintaining a resilient surfacing in the fall zone of outdoor play equipment helps to reduce injuries if children fall. The fall zone under and around your complex climber contained more than the required six inches of mulch; however, grass and weeds have encroached into the fall zone area, compromising the integrity of the surfacing. The lead teacher stated that the landscaper is supposed to treat and remove weeds from the mulch. Treat and remove the grass and weeds. In Northeastern North Carolina, this is an on-going playground maintenance item, especially when the weather is warm and plants are actively growing. For your compliance letter, state the date when the weeds and grass have been removed from the fall zone. *Also have your landscaper trim back the Knock-out Rose Bush that has grown over the front entrance ramp. Reminders: Sanitation and fire inspections are required at least annually by the end of the month of the previous inspection. Send fire inspections to me within seven (7) days of the inspection. To request a fire inspection in Currituck, contact the County Inspections Department, complete the appropriate form and pay the annual inspection fee. Have your fire and electrical systems tests and annual fire safety training completed before the inspection so that documentation is available for the fire inspector. Consider beginning this process at least three months before the actual fire inspection is due to ensure the final inspection is completed on time. Staff Worksheets – We reviewed your staff worksheet. I highlighted items that are coming due in the next year. Emergency Preparedness and Response (EPR) Requirements – You completed your EPR Plan in March 2026. I reviewed it today. Upload the center’s floor plan on page 9 and the center’s evacuation diagram on page 10 by the end of June 2026. Once you have added them, finalize the plan, print the final plan, and review with staff. Document your staff review with a roster that includes staff name and signature and date of review. Begin completing and documenting Emergency Drills (lock-down and shelter-in-place) in July 2026 and then every three months (July, October, December, April). You can do one or the other every three months. Continue to complete fire drills monthly. Consider doing an extra set of drills in September after new children have started to ensure they get to practice how to safely evacuate, lock down or shelter in place. Your ready-to-go file included children’s applications containing emergency information. Consider attaching a current picture of each child to the application. It also included the Staff Emergency Information form for each staff person. (Add one for S. Levy.) Your Emergency Preparedness and Response Plan (EPR Plan) along with your Ready-to-Go File, Emergency Medical Care Plan (EMCP) and consistent practice of Fire and Emergency Drills are your center’s foundation for emergency preparedness. *Review your EPR Plan at least annually in the NC Risk Management Portal and update, if needed. Print updated plan or updated cover sheet and page 28. (Per your request I will email instructions on how to update your EPR Plan. *At the same time, review and update your Emergency Medical Care Plan, if needed. *Ensure your Ready-to-Go File is current and includes the following: a copy of the Emergency Preparedness and Response Plan, child's Application for Child Care (includes health & emergency information, contacts, authorization to seek emergency medical care), medication authorizations and instructions, any action plans for children with special health care needs, a list of any known food allergies of children and staff, staff contact information, Incident Report forms, an area map, and emergency telephone numbers. Supervision – Because bathrooms are not located in the classrooms, teachers need to stand in the doorway to monitor both the bathroom and the children remaining in the classroom to ensure adequate supervision. Nutrition – Staff are required to model healthy eating habits when consuming food and beverages in front of children. Cans or bottles of soda pop (or cups from fast food restaurants) may not be visible in the classroom or in front of children. Staff may use opaque cups, if needed. Naptime – You have numbered your mats and created a mat chart identifying each child with a numbered mat. Because you clean and sanitize each mat after naptime, the Environmental Health Specialist stated that you can store mats so that the “colors” each other. During naptime, mats need to spaced at least 18 inches apart to reduce the spread of germs when children are sleeping and breathing hard. Storage of Open Food – When bags of food are opened but not completely eaten, they need to be dated and stored in a resealable container or Ziploc bag. This keeps food fresher and prevents pests. Use and Storage of Hand Sanitizer – You stated that you have received mixed messages on the use and storage of hand sanitizer (referred to as hand antiseptic in Sanitation Rules). Environmental Health Rules 15A NCAC 18A .2803 HANDWASHING (b) describes how hand sanitizer may be used. “The use of a hand antiseptic does not replace the requirements for handwashing in Paragraph (a) of this Rule except that an employee who is supervising a child or children outdoors may use a hand antiseptic while outdoors in lieu of handwashing, provided that the employee's hands are washed in accordance with Paragraph (e) of this Rule when the employee returns indoors. Hand antiseptic shall not be used in lieu of handwashing when the employee's action that necessitates handwashing is diapering, food preparation, or food service.” So, if soap and running water are available, staff and children need to wash their hands. The storage of hand sanitizer is addressed in Environmental Health Rule 15A NCAC 18A .2820 STORAGE (c) Non-aerosol sanitizing solutions, approved disinfectants, detergent solutions, hand antiseptics, and hand lotions shall be kept inaccessible to children when not in use but are not required to be in locked storage. These solutions shall be labeled as sanitizing, disinfecting, or detergent solutions. Hand soap other than that which is in bulk containers is not required to be kept inaccessible to children or in locked storage. Bulk soaps should be kept inaccessible to children. The goal is to empower teachers to create learning environments that promote pro-social skills, like self-control and problem solving. K. Bare, Administrator, assisted me with the visit. Your program currently operates with a Notice of Compliance (NOC) issued 3/19/2025 and updated on 10/18/2025. Please contact me if you are ever interested in becoming fully licensed to earn a star rating. This is your first annual compliance visit as you received your Notice of Compliance on 3/19/25. The sanitation inspection was completed 6/24/25 with a “Superior” classification following a “Provisional” classification on 6/18/25. The last fire inspection was conducted 11/6/2025 with a Satisfactory rating and approval for daytime care only. Sanitation and fire inspections are required at least annually by the end of the month of the previous inspection. Send fire inspections to me within seven (7) days of the inspection. Three-year water testing was completed 1/24/25 and is due again by 1/24/28. The center's compliance history was reviewed with the operator. The program’s compliance history was one hundred percent as of 2/13/2026. This facility is owned by Kingdom Impact Kids Academy, LLC. The limited liability company was listed as current- active on the NC Secretary of State website. License and contact information were current. Contact me to request any changes to your license or contact information. I monitored the classrooms and spaces used for child care and outdoor play areas for applicable child care center requirements using the Child Care Center Requirements, effective 7/1/2025 and Item Numbers Listing, effective 4/2025. LLC submit annual reports by April for the previous year. For example, by 4/15/26, you will submit your annual report for the year ending December 2025. Go to the NC Secretary of State to file your annual report. I monitored three staff records, two children’s records and all program records. Children in both classrooms were participating in free play with toys and materials. At group time they read stories. They ate lunches brought from home in the classrooms, washing their hands before and after eating. Nutrition Opt-out forms were available in children’s files. They rested on linen-covered mats. Interactions between staff and children were positive and nurturing. I observed and documented the following violations during today’s visit. Violation Number Comment Rule 114 A summary of the NC Child Care Law was not given to a parent of every child enrolled in the center. Documentation of receipt of the Summary of NC Child Care Law was not available for a child enrolled on January 6, 2026. GS 110-102 125 Daily records of arrival and departure times for children at the center were not maintained as children arrive and depart and/or were not made available for review. On multiple dates, parents documented and signed arrival times for their children but did not document departures times. 10A NCAC 09 .0302(d)(4) 601 Refrigerator(s) did not maintain a temperature of 45 degrees F. or below. A thermometer was not available in the refrigerator. 15A NCAC 18A .2806(j)(2) 1032 Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. A staff member hired on August 20, 2025 did not have a staff medical report on file until January 20, 2026. A staff member hired on August 18, 2025 did not have a staff medical report on file until January 28, 2026. A staff member hired on August 25, 2025 did not have a staff medical report on file until February 16, 2026. 10A NCAC 09 .0701(a) 1033 On or before the first day of work, all staff, including the director and individuals who volunteer more than once per week did not provide results indicating that they were free of active TB and/or TB test or screening was older than 12 months. A staff member hired on August 20, 2025 did not have documentation of a negative TB screening or test until January 20, 2026. .0701(a) 1041 Prior to employment a Criminal Background Check was not completed. A staff member hired on August 25, 2025 did not complete a criminal background check. G.S. 110-90.2(b) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. Documentation of current First Aid was not available for a staff member hired on August 20, 2025 and for a staff member hired on August 25, 2025. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Documentation of CPR training for an employee hired on August 20, 2025 was not available until January 21, 2026. Documentation of CPR training for an employee hired on August 25, 2025 was not available. .1102(d) 1320 Children's records that include an application for enrollment, medical and immunization records, and permission to seek emergency medical care was not on file for each child. A child enrolled on January 6, 2026 did not have a child's medical report on file. A staff member hired on August 25, 2025 did not have a Provisional Qualification Letter or a full Qualification Letter on file. An example is: Jane Doe, Administrator AAA Child Care ID # 12345678 Technical Assistance With Documented Violations: Children’s Files: Maintaining accurate records on the children in your care helps to ensure their health and safety, informs families of rules you are required to follow and your center’s policies, and provides a certain transfer of knowledge so that you can provide the best care possible for their children. A file for LH (Space 3 – Date of enrollment 1/6/26) did not include a statement of receipt of the Summary of NC Child Care Law or a Child’s Medical Report (top completed by parent/bottom completed by health care professional) or copy of a current health assessment. You stated that you somehow missed this paperwork when enrolling the child. Use Children’s File Checklist to verify you have received all required enrollment documents. Build in time to review enrollment documents with families explaining that they need to be fully completed, signed and dated. Check them before the parent leaves their children with you the first time. The empty blank might be the signature you need to seek emergency medical care if the child is injured. Have parents review and update the Child’s Application annually to ensure information is still current. For your compliance letter, state the date the parent signed receipt of the summary of law and the date you received the child’s medical report or a copy of a health assessment completed in the last 12 months. Do not send copies of these documents. Attendance – Having accurate attendance is important to have as documentation if there is a question about children’s attendance related to payments, subsidy, or even parental disputes. More importantly it helps to accurately account for children in the event of an emergency where you might have to evacuate the center. On multiple dates, documentation of arrival was documented but documentation of departure was not. The lead teacher stated that some of the parents had forgotten probably because there were multiple families looking to sign out at the end of the day. I explained that if the parent forgets to sign a child in or out, the staff can complete that but would need to sign their own name and not the parent’s name. She corrected the missing departure times. Documentation of attendance helps to ensure the safety of children and must be current throughout the operating day, including arrival and departure times. Your attendance records must reflect all children who are enrolled in and attend your program (full-time, part-time, drop-in, one-time) whether they are present or not. You can complete this information, but it is best practice to require parents to complete this daily at drop-off and pick-up and then sign or initial. Maintain attendance records for at least one year. This violation has been corrected, and no further action is required except to ensure that documentation of arrival and departure is maintained. Refrigerator Thermometer – A thermometer ensures that refrigerators are maintaining cold temperatures for the safe storage of food and beverages, including milk, formula, and human milk in order to prevent food borne illness. There was not a thermometer in the kitchen refrigerator which is used to store children’s lunches. You were not sure where it went but will purchase one. Remind staff to check their thermometers to ensure that they maintain a temperature of 45 degrees or colder. (Forty-one degrees is preferred by Environmental Health.) For your compliance letter, state the date you put a new thermometer in the refrigerator. Staff Medical Reports and TB Tests - Receipt of a Staff Medical Report and Documentation of a negative TB Test or a TB Screening received from new staff by their first day of working in your center verifies that they are healthy enough to work with children and other staff and complete the physical responsibilities needed to work in child care. The following staff did not have either a Staff Medical Report and/or documentation of a negative TB screening or TB test by their first day of work. B. Raitt (Date of Hire – 8/20/25): SMR – 1/20/26; TB Test 1/22/26 R. Gregory (Date of Hire – 8/18/25): SMR – 1/28/26 S. Levy (Date of Hire – 8/25/25): SMR – 2/16/26 You stated that you were not aware that these staff documents were required by their first day of work and will use a Staff File Check list to track receipt of required documents at employment to help you ensure required forms are received before a new employee starts. These violations have been corrected so no further action is required at this time except to ensure staff documents are received before the first day of work. Criminal Background Check (CBC) – Criminal Background Checks ensure that staff have not committed any crimes which could potentially make them unsafe to be around children and other staff. Because Abuse and Neglect registries are also checked, it helps to ensure that children are protected from physical, mental, and sexual abuse. Performing diligent background screenings also protects the child care facility against future legal challenges. Sarah Levy was employed on 8/25/2025. She was required to complete an out-of-state application but missed submitting the application for the Abuse/Neglect Registry Check. She also did not submit the Out of State Verification form with the required out of state documents. Her application was closed. She applied again in January 2026 but again did not complete the A/N Registry form or submit the Out of State Verification form. She did not have a Provisional Qualifying Letter or a full Qualifying Letter available for review and when I checked ABCMS, she had started the application and fingerprint process several times but had not submitted the required out-of-state documents. The administrator stated that this was her fault because she had not done an out-of-state application in the past year and did not scroll down far enough to see the Abuse/Neglect Registry Check. Sarah Levy left the premises and may not return until a Provisional Qualification Letter is available in her file. I provided technical assistance and the Out of State Verification Form to the administrator who will assist Ms. Levy with completing the Abuse/Neglect Registry application and then email the required out of state documents to the CBC Unit. Once these are received, Ms. Levy will receive a Provisional Qualification Letter that will allow her to return to the center where she may count in staff/child ratio but may not be alone with any child or a group of children until her full Qualification Letter is received and on file. Please note that repeated violations related to Criminal Background Checks may result in an administrative action. For your compliance letter, describe how you will ensure that staff have current DHHS Criminal Background Checks and qualifying letters. Include a copy of Ms. Levy’s Provisional Qualification Letter. First Aid and CPR – Early care and education programs with staff trained in pediatric first aid and CPR can mitigate the consequences of injury and reduce the potential for death from life-threatening conditions and emergencies. Repetitive training, coupled with the confidence to use these skills, is critically important to the outcome of an emergency. (Caring for Our Children, 4th Edition). B. Raitt was hired on 8/20/25 and required to complete CPR/First Aid by 11/20/25 Documentation available in the file reflects that she completed BLS CPR/AED training on 1/21/26. BLS certification does not include First Aid training (or first aid training is not reflected on the card.) Ms. Raitt stated that her class included first aid training. She needs to provide an additional card that reflects first aid certification or complete an approved class that includes first aid training. S. Levy was hired on 8/25/25 and did not have documentation of CPR/First Aid in her file. She stated that she has completed it but needed to find the email with the certification card. You stated that you confused licensing rules from Virginia where only one or two people in a center are required to have First Aid and CPR. You now understand that all staff are required to complete it within three months of hire and then every two years. Use your staff worksheets and create a “tickler” to send a reminder to your phone or email several months before CPR/First Aid expires to ensure you register for a class before your certifications expire. Blended classes with on-line instruction and face-to-face skills test are now allowed if provided by approved agency and trainer. Register the staff for an approved First Aid/CPR class so that it is completed by 3/6/26 or have them provide you with the documentation needed to verify they have a current First Aid and/or First Aid/CPR certification. Repeated CPR/First Aid violations may result in an Administrative Action. Send a copy of the CPR/First Aid cards with compliance letter. K. Bare, Administrator, assisted me with the visit. Your program currently operates with a Notice of Compliance (NOC) issued 3/19/2025 and updated on 10/18/2025. Please contact me if you are ever interested in becoming fully licensed to earn a star rating. This is your first annual compliance visit as you received your Notice of Compliance on 3/19/25. The sanitation inspection was completed 6/24/25 with a “Superior” classification following a “Provisional” classification on 6/18/25. The last fire inspection was conducted 11/6/2025 with a Satisfactory rating and approval for daytime care only. Sanitation and fire inspections are required at least annually by the end of the month of the previous inspection. Send fire inspections to me within seven (7) days of the inspection. Three-year water testing was completed 1/24/25 and is due again by 1/24/28. The center's compliance history was reviewed with the operator. I monitored the classrooms and spaces used for child care and outdoor play areas for applicable child care center requirements using the Child Care Center Requirements, effective 7/1/2025 and Item Numbers Listing, effective 4/2025. LLC submit annual reports by April for the previous year. For example, by 4/15/26, you will submit your annual report for the year ending December 2025. Go to the NC Secretary of State to file your annual report. I monitored three staff records, two children’s records and all program records. Children in both classrooms were participating in free play with toys and materials. At group time they read stories. They ate lunches brought from home in the classrooms, washing their hands before and after eating. Nutrition Opt-out forms were available in children’s files. They rested on linen-covered mats. Interactions between staff and children were positive and nurturing. I observed and documented the following violations during today’s visit. On multiple dates, parents documented and signed arrival times for their children but did not document departures times. 10A NCAC 09 .0302(d)(4) 601 Refrigerator(s) did not maintain a temperature of 45 degrees F. or below. A thermometer was not available in the refrigerator. 15A NCAC 18A .2806(j)(2) 1032 Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. A staff member hired on August 20, 2025 did not have a staff medical report on file until January 20, 2026. A staff member hired on August 18, 2025 did not have a staff medical report on file until January 28, 2026. G.S. 110-90.2(b) & (d) & .2703(e) * Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violations documented may impact the compliance history score. The violations documented must be corrected immediately. On or before 3/5/26, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware that any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted. A file for LH (Space 3 – Date of enrollment 1/6/26) did not include a statement of receipt of the Summary of NC Child Care Law or a Child’s Medical Report (top completed by parent/bottom completed by health care professional) or copy of a current health assessment. You stated that you somehow missed this paperwork when enrolling the child. Use Children’s File Checklist to verify you have received all required enrollment documents. Build in time to review enrollment documents with families explaining that they need to be fully completed, signed and dated. Check them before the parent leaves their children with you the first time. The empty blank might be the signature you need to seek emergency medical care if the child is injured. Have parents review and update the Child’s Application annually to ensure information is still current. For your compliance letter, state the date the parent signed receipt of the summary of law and the date you received the child’s medical report or a copy of a health assessment completed in the last 12 months. Do not send copies of these documents. Attendance – Having accurate attendance is important to have as documentation if there is a question about children’s attendance related to payments, subsidy, or even parental disputes. More importantly it helps to accurately account for children in the event of an emergency where you might have to evacuate the center. On multiple dates, documentation of arrival was documented but documentation of departure was not. The lead teacher stated that some of the parents had forgotten probably because there were multiple families looking to sign out at the end of the day. I explained that if the parent forgets to sign a child in or out, the staff can complete that but would need to sign their own name and not the parent’s name. She corrected the missing departure times. Documentation of attendance helps to ensure the safety of children and must be current throughout the operating day, including arrival and departure times. Your attendance records must reflect all children who are enrolled in and attend your program (full-time, part-time, drop-in, one-time) whether they are present or not. You can complete this information, but it is best practice to require parents to complete this daily at drop-off and pick-up and then sign or initial. Maintain attendance records for at least one year. This violation has been corrected, and no further action is required except to ensure that documentation of arrival and departure is maintained. Refrigerator Thermometer – A thermometer ensures that refrigerators are maintaining cold temperatures for the safe storage of food and beverages, including milk, formula, and human milk in order to prevent food borne illness. There was not a thermometer in the kitchen refrigerator which is used to store children’s lunches. You were not sure where it went but will purchase one. Remind staff to check their thermometers to ensure that they maintain a temperature of 45 degrees or colder. (Forty-one degrees is preferred by Environmental Health.) For your compliance letter, state the date you put a new thermometer in the refrigerator. Staff Medical Reports and TB Tests - Receipt of a Staff Medical Report and Documentation of a negative TB Test or a TB Screening received from new staff by their first day of working in your center verifies that they are healthy enough to work with children and other staff and complete the physical responsibilities needed to work in child care. The following staff did not have either a Staff Medical Report and/or documentation of a negative TB screening or TB test by their first day of work. B. Raitt (Date of Hire – 8/20/25): SMR – 1/20/26; TB Test 1/22/26 R. Gregory (Date of Hire – 8/18/25): SMR – 1/28/26 S. Levy (Date of Hire – 8/25/25): SMR – 2/16/26 You stated that you were not aware that these staff documents were required by their first day of work and will use a Staff File Check list to track receipt of required documents at employment to help you ensure required forms are received before a new employee starts. These violations have been corrected so no further action is required at this time except to ensure staff documents are received before the first day of work. Criminal Background Check (CBC) – Criminal Background Checks ensure that staff have not committed any crimes which could potentially make them unsafe to be around children and other staff. Because Abuse and Neglect registries are also checked, it helps to ensure that children are protected from physical, mental, and sexual abuse. Performing diligent background screenings also protects the child care facility against future legal challenges. Sarah Levy was employed on 8/25/2025. She was required to complete an out-of-state application but missed submitting the application for the Abuse/Neglect Registry Check. She also did not submit the Out of State Verification form with the required out of state documents. Her application was closed. She applied again in January 2026 but again did not complete the A/N Registry form or submit the Out of State Verification form. She did not have a Provisional Qualifying Letter or a full Qualifying Letter available for review and when I checked ABCMS, she had started the application and fingerprint process several times but had not submitted the required out-of-state documents. The administrator stated that this was her fault because she had not done an out-of-state application in the past year and did not scroll down far enough to see the Abuse/Neglect Registry Check. Sarah Levy left the premises and may not return until a Provisional Qualification Letter is available in her file. I provided technical assistance and the Out of State Verification Form to the administrator who will assist Ms. Levy with completing the Abuse/Neglect Registry application and then email the required out of state documents to the CBC Unit. Once these are received, Ms. Levy will receive a Provisional Qualification Letter that will allow her to return to the center where she may count in staff/child ratio but may not be alone with any child or a group of children until her full Qualification Letter is received and on file. Please note that repeated violations related to Criminal Background Checks may result in an administrative action. For your compliance letter, describe how you will ensure that staff have current DHHS Criminal Background Checks and qualifying letters. Include a copy of Ms. Levy’s Provisional Qualification Letter. First Aid and CPR – Early care and education programs with staff trained in pediatric first aid and CPR can mitigate the consequences of injury and reduce the potential for death from life-threatening conditions and emergencies. Repetitive training, coupled with the confidence to use these skills, is critically important to the outcome of an emergency. (Caring for Our Children, 4th Edition). B. Raitt was hired on 8/20/25 and required to complete CPR/First Aid by 11/20/25 Documentation available in the file reflects that she completed BLS CPR/AED training on 1/21/26. BLS certification does not include First Aid training (or first aid training is not reflected on the card.) Ms. Raitt stated that her class included first aid training. She needs to provide an additional card that reflects first aid certification or complete an approved class that includes first aid training. S. Levy was hired on 8/25/25 and did not have documentation of CPR/First Aid in her file. She stated that she has completed it but needed to find the email with the certification card. You stated that you confused licensing rules from Virginia where only one or two people in a center are required to have First Aid and CPR. You now understand that all staff are required to complete it within three months of hire and then every two years. Use your staff worksheets and create a “tickler” to send a reminder to your phone or email several months before CPR/First Aid expires to ensure you register for a class before your certifications expire. Blended classes with on-line instruction and face-to-face skills test are now allowed if provided by approved agency and trainer. Register the staff for an approved First Aid/CPR class so that it is completed by 3/6/26 or have them provide you with the documentation needed to verify they have a current First Aid and/or First Aid/CPR certification. The lead teacher stated that some of the parents had forgotten probably because there were multiple families looking to sign out at the end of the day. I explained that if the parent forgets to sign a child in or out, the staff can complete that but would need to sign their own name and not the parent’s name. She corrected the missing departure times. Documentation of attendance helps to ensure the safety of children and must be current throughout the operating day, including arrival and departure times. Your attendance records must reflect all children who are enrolled in and attend your program (full-time, part-time, drop-in, one-time) whether they are present or not. You can complete this information, but it is best practice to require parents to complete this daily at drop-off and pick-up and then sign or initial. Refrigerator Thermometer – A thermometer ensures that refrigerators are maintaining cold temperatures for the safe storage of food and beverages, including milk, formula, and human milk in order to prevent food borne illness. There was not a thermometer in the kitchen refrigerator which is used to store children’s lunches. You were not sure where it went but will purchase one. Remind staff to check their thermometers to ensure that they maintain a temperature of 45 degrees or colder. (Forty-one degrees is preferred by Environmental Health.) For your compliance letter, state the date you put a new thermometer in the refrigerator. Staff Medical Reports and TB Tests - Receipt of a Staff Medical Report and Documentation of a negative TB Test or a TB Screening received from new staff by their first day of working in your center verifies that they are healthy enough to work with children and other staff and complete the physical responsibilities needed to work in child care. The following staff did not have either a Staff Medical Report and/or documentation of a negative TB screening or TB test by their first day of work. B. Levy (Date of Hire – 8/25/25): SMR – 2/16/26 You stated that you were not aware that these staff documents were required by their first day of work and will use a Staff File Check list to track receipt of required documents at employment to help you ensure required forms are received before a new employee starts. These violations have been corrected so no further action is required at this time except to ensure staff documents are received before the first day of work. Criminal Background Check (CBC) – Criminal Background Checks ensure that staff have not committed any crimes which could potentially make them unsafe to be around children and other staff. Because Abuse and Neglect registries are also checked, it helps to ensure that children are protected from physical, mental, and sexual abuse. Performing diligent background screenings also protects the child care facility against future legal challenges. Sarah Levy was employed on 8/25/2025. She was required to complete an out-of-state application but missed submitting the application for the Abuse/Neglect Registry Check. She also did not submit the Out of State Verification form with the required out of state documents. Her application was closed. She applied again in January 2026 but again did not complete the A/N Registry form or submit the Out of State Verification form. She did not have a Provisional Qualifying Letter or a full Qualifying Letter available for review and when I checked ABCMS, she had started the application and fingerprint process several times but had not submitted the required out-of-state documents. The administrator stated that this was her fault because she had not done an out-of-state application in the past year and did not scroll down far enough to see the Abuse/Neglect Registry Check. Sarah Levy left the premises and may not return until a Provisional Qualification Letter is available in her file. I provided technical assistance and the Out of State Verification Form to the administrator who will assist Ms. Levy with completing the Abuse/Neglect Registry application and then email the required out of state documents to the CBC Unit. Once these are received, Ms. Levy will receive a Provisional Qualification Letter that will allow her to return to the center where she may count in staff/child ratio but may not be alone with any child or a group of children until her full Qualification Letter is received and on file. Please note that repeated violations related to Criminal Background Checks may result in an administrative action. For your compliance letter, describe how you will ensure that staff have current DHHS Criminal Background Checks and qualifying letters. Include a copy of Ms. Levy’s Provisional Qualification Letter. First Aid and CPR – Early care and education programs with staff trained in pediatric first aid and CPR can mitigate the consequences of injury and reduce the potential for death from life-threatening conditions and emergencies. Repetitive training, coupled with the confidence to use these skills, is critically important to the outcome of an emergency. (Caring for Our Children, 4th Edition). B. Raitt was hired on 8/20/25 and required to complete CPR/First Aid by 11/20/25 Documentation available in the file reflects that she completed BLS CPR/AED training on 1/21/26. BLS certification does not include First Aid training (or first aid training is not reflected on the card.) Ms. Raitt stated that her class included first aid training. She needs to provide an additional card that reflects first aid certification or complete an approved class that includes first aid training. S. Levy was hired on 8/25/25 and did not have documentation of CPR/First Aid in her file. She stated that she has completed it but needed to find the email with the certification card. You stated that you confused licensing rules from Virginia where only one or two people in a center are required to have First Aid and CPR. You now understand that all staff are required to complete it within three months of hire and then every two years. Use your staff worksheets and create a “tickler” to send a reminder to your phone or email several months before CPR/First Aid expires to ensure you register for a class before your certifications expire. Blended classes with on-line instruction and face-to-face skills test are now allowed if provided by approved agency and trainer. Register the staff for an approved First Aid/CPR class so that it is completed by 3/6/26 or have them provide you with the documentation needed to verify they have a current First Aid and/or First Aid/CPR certification. A file for LH (Space 3 – Date of enrollment 1/6/26) did not include a statement of receipt of the Summary of NC Child Care Law or a Child’s Medical Report (top completed by parent/bottom completed by health care professional) or copy of a current health assessment. You stated that you somehow missed this paperwork when enrolling the child. Use Children’s File Checklist to verify you have received all required enrollment documents. Build in time to review enrollment documents with families explaining that they need to be fully completed, signed and dated. Check them before the parent leaves their children with you the first time. The empty blank might be the signature you need to seek emergency medical care if the child is injured. Have parents review and update the Child’s Application annually to ensure information is still current. For your compliance letter, state the date the parent signed receipt of the summary of law and the date you received the child’s medical report or a copy of a health assessment completed in the last 12 months. Do not send copies of these documents. Attendance – Having accurate attendance is important to have as documentation if there is a question about children’s attendance related to payments, subsidy, or even parental disputes. More importantly it helps to accurately account for children in the event of an emergency where you might have to evacuate the center. On multiple dates, documentation of arrival was documented but documentation of departure was not. The lead teacher stated that some of the parents had forgotten probably because there were multiple families looking to sign out at the end of the day. I explained that if the parent forgets to sign a child in or out, the staff can complete that but would need to sign their own name and not the parent’s name. She corrected the missing departure times. Documentation of attendance helps to ensure the safety of children and must be current throughout the operating day, including arrival and departure times. Your attendance records must reflect all children who are enrolled in and attend your program (full-time, part-time, drop-in, one-time) whether they are present or not. You can complete this information, but it is best practice to require parents to complete this daily at drop-off and pick-up and then sign or initial. Maintain attendance records for at least one year. This violation has been corrected, and no further action is required except to ensure that documentation of arrival and departure is maintained. Refrigerator Thermometer – A thermometer ensures that refrigerators are maintaining cold temperatures for the safe storage of food and beverages, including milk, formula, and human milk in order to prevent food borne illness. There was not a thermometer in the kitchen refrigerator which is used to store children’s lunches. You were not sure where it went but will purchase one. Remind staff to check their thermometers to ensure that they maintain a temperature of 45 degrees or colder. (Forty-one degrees is preferred by Environmental Health.) For your compliance letter, state the date you put a new thermometer in the refrigerator. Staff Medical Reports and TB Tests - Receipt of a Staff Medical Report and Documentation of a negative TB Test or a TB Screening received from new staff by their first day of working in your center verifies that they are healthy enough to work with children and other staff and complete the physical responsibilities needed to work in child care. The following staff did not have either a Staff Medical Report and/or documentation of a negative TB screening or TB test by their first day of work. B. She was required to complete an out-of-state application but missed submitting the application for the Abuse/Neglect Registry Check. She also did not submit the Out of State Verification form with the required out of state documents. Her application was closed. She applied again in January 2026 but again did not complete the A/N Registry form or submit the Out of State Verification form. She did not have a Provisional Qualifying Letter or a full Qualifying Letter available for review and when I checked ABCMS, she had started the application and fingerprint process several times but had not submitted the required out-of-state documents. The administrator stated that this was her fault because she had not done an out-of-state application in the past year and did not scroll down far enough to see the Abuse/Neglect Registry Check. Sarah Levy left the premises and may not return until a Provisional Qualification Letter is available in her file. I provided technical assistance and the Out of State Verification Form to the administrator who will assist Ms. Levy with completing the Abuse/Neglect Registry application and then email the required out of state documents to the CBC Unit. Once these are received, Ms. Levy will receive a Provisional Qualification Letter that will allow her to return to the center where she may count in staff/child ratio but may not be alone with any child or a group of children until her full Qualification Letter is received and on file. Please note that repeated violations related to Criminal Background Checks may result in an administrative action. Blended classes with on-line instruction and face-to-face skills test are now allowed if provided by approved agency and trainer. I provided technical assistance and the Out of State Verification Form to the administrator who will assist Ms. Levy with completing the Abuse/Neglect Registry application and then email the required out of state documents to the CBC Unit. Once these are received, Ms. Levy will receive a Provisional Qualification Letter that will allow her to return to the center where she may count in staff/child ratio but may not be alone with any child or a group of children until her full Qualification Letter is received and on file. Please note that repeated violations related to Criminal Background Checks may result in an administrative action. For your compliance letter, describe how you will ensure that staff have current DHHS Criminal Background Checks and qualifying letters. Include a copy of Ms. Levy’s Provisional Qualification Letter. First Aid and CPR – Early care and education programs with staff trained in pediatric first aid and CPR can mitigate the consequences of injury and reduce the potential for death from life-threatening conditions and emergencies. Repetitive training, coupled with the confidence to use these skills, is critically important to the outcome of an emergency. (Caring for Our Children, 4th Edition). B. Raitt was hired on 8/20/25 and required to complete CPR/First Aid by 11/20/25 Documentation available in the file reflects that she completed BLS CPR/AED training on 1/21/26. BLS certification does not include First Aid training (or first aid training is not reflected on the card.) Ms. Raitt stated that her class included first aid training. She needs to provide an additional card that reflects first aid certification or complete an approved class that includes first aid training. S. Levy was hired on 8/25/25 and did not have documentation of CPR/First Aid in her file. She stated that she has completed it but needed to find the email with the certification card. You stated that you confused licensing rules from Virginia where only one or two people in a center are required to have First Aid and CPR. You now understand that all staff are required to complete it within three months of hire and then every two years. Use your staff worksheets and create a “tickler” to send a reminder to your phone or email several months before CPR/First Aid expires to ensure you register for a class before your certifications expire. Blended classes with on-line instruction and face-to-face skills test are now allowed if provided by approved agency and trainer. Register the staff for an approved First Aid/CPR class so that it is completed by 3/6/26 or have them provide you with the documentation needed to verify they have a current First Aid and/or First Aid/CPR certification. Repeated CPR/First Aid violations may result in an Administrative Action. Send a copy of the CPR/First Aid cards with compliance letter. Resources: As a licensed child care facility, it is your responsibility to be knowledgeable of the child care laws and rules and to teach your staff the rules to ensure your center remains in compliance. Reminders: ABCMS Portal - You have completed the ABCMS Provider Portal Training and are now authorized to log in the ABCMS Provider Portal https://ncabcms.nc.gov/DCDEE/ using your Business NCID. You have not connected your staff and created your roster. I am emailing the ABCMS Provider Portal Access Guide which will assist with the step-by-step process of working in the ABCMS Portal, accessing your Pay Codes, helping your staff complete their Connecting Applications, and eventually “hiring” staff to your roster. If you are still having difficulties working through this process, consider completing the ABCMS Provider Portal Training on Moodle again and ensure that you are using the correct Business NCID to access the ABCMS Provider Portal and Individual NCID to access your own ABCMS Applicant Portal. Staff will use their Individual NCID to log into the Applicant Portal and enter the Pay Code to connect to your center. You will then log into the ABCMS Provider Portal to add them to your roster. Complete your roster and maintain it by updating it as staff are hired or terminated. Staff Worksheets – We reviewed your staff worksheet. I highlighted items that were delinquent and items that are coming due in the next year. Centers Operating with a Notice of Compliance – Church-sponsored centers that operate with a Notice of Compliance and do not accept subsidy are exempt from rules related to new staff orientation (10A NCAC 09 .1101), Health & Safety Training (10A NCAC 09 .1102) including initial and five-year Health & Safety Training, Recognizing and Responding to Suspicions of Child Maltreatment, and playground safety training. They are required to maintain CPR/First Aid training and ITS/SIDS training (if licensed for infant care). Supervision – Because bathrooms are not located in the classrooms, teachers need to stand in the doorway to monitor both the bathroom and the children remaining in the classroom to ensure adequate supervision. Sex Offender Registry and Notification Email - North Carolina General Statute 14-208 requires sex offenders to register with the North Carolina Department of Justice. The law states that a sex offender shall not knowingly reside within 1,000 feet of the property on which any public or nonpublic school or child care center is located. This does not apply to child care centers that are located on or within 1,000 feet of property of an institution of higher education where the registrant is a student or is employed. All licensed child care centers must register to receive e-mail notification when a registered sex offender moves within a one-mile radius of the center.
