C. Wissman, Administrator, assisted me with the visit. Your program currently operates with a three-star license, issued January 19, 202 5 (capacity change), earning 1 point in the education component, 5 points in the program standards component (meeting enhanced ratios and space) and 1 quality point for Staff Benefits and Enhanced Parent Involvement. The last annual compliance visit was conducted December 9, 2025. The sanitation inspection was completed April 15, 2026, with a “Superior” classification. The last fire inspection was conducted June 5, 2025, and your facility was approved for daytime care only but is getting ready to expire on June 30, 2026. Children in seven classrooms participated in developmentally appropriate activities. Infants played on the floor and were cared for on their own schedule. Documentation of safe sleep checks was available. Feeding plans were posted. The theme throughout the center was gardening. Children were painting flowers, playing flower match games, and identifying bugs. Friday is “dress as a gardener” day. Children went outdoors and played on one of the two fenced playgrounds. Lunch was observed and consisted of pasta with marinara meat sauce, peas, pears, and milk. I observed and documented the following violations during today’s visit. The operator corrected both of them during the visit. Violation Number Comment Rule 852 Incident reports were not completed each time a child was injured, it did not include all the information required in rule, it was not signed by the parent and/or it was not maintained in the child's file. Incident reports observed in six children's files did not include license number, consultant name, check of home or center, and county. .0802 (e) 1314 Emergency information did not name childs health care professional. In one of six children's files reviewed, the parent did not list the child's health care provider. .0802(c)(2) * Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violations documented may impact the compliance history score. Technical Assistance With Documented Violations: Child’s Application – The DCDEE form “Child’s Application for Enrollment” contains important information related to the care of children, including parent contact and emergency contact information, alternate pick-up designees, emergency medical information, and authorization to seek medical help if parents cannot be reached. In one of six children’s files reviewed, the parent did not include the child’s physician. You stated that you missed it when reviewing the file at enrollment. You had the parent complete it today. This violation has been corrected. Incident Reports – Incident Reports keep parents informed of injuries their child has at your facility and provide an opportunity for you and your staff to review your own safety and supervision practices to prevent similar injuries in the future. Incident reports reviewed in children’s files today did not include the information in the upper right corner: license number, check if home or center, consultant name, county. You corrected this by creating a template to use going forward. Reminders: Required Water, Lead Paint, and Asbestos Testing - The Clean Classrooms for Carolina Kids website reflects the following information about your center’s required three-year water testing and one-time lead paint testing, and asbestos testing: Three-year Water Testing – 12/19/24; due again 12/19/27 Lead Paint Testing – Survey Review by RTI pending Asbestos Testing – Survey Review by RTI pending *Your survey for lead paint testing and asbestos testing has been pending for over six months. Check in with RTI to determine if additional documentation is required to have your testing closed out. ABCMS Portal - You have completed the ABCMS Provider Portal Training and are now authorized to log in the ABCMS Provider Portal https://ncabcms.nc.gov/DCDEE/ using your Business NCID. You have not connected your staff and created your roster. I am emailing the ABCMS Provider Portal Technical Assistance instructions which will assist with the step-by-step process of working in the ABCMS Portal, accessing your Pay Codes, helping your staff complete their Connecting Applications, and eventually “hiring” staff to your roster. If you are still having difficulties working through this process, consider completing the ABCMS Provider Portal Training on Moodle again and ensure that you are using the correct Business NCID to access the ABCMS Provider Portal and Individual NCID to access your own ABCMS Applicant Portal. Complete your roster and maintain it by updating it as staff are hired or terminated. Staff Worksheets – We reviewed your staff worksheet. I highlighted items that were delinquent and items that are coming due in the next year. *Health and Safety Training and Five (5) Year Cycle Tracking Requirements – Review Row 17 on Staff Worksheet for initial and five-year H&S Training renewal dates. New staff need to complete Recognizing and Responding to Suspicions of Child Maltreatment and CPR/First Aid within three months of hire and the rest of the training modules within twelve months. *On-going Training Hours – Review Row 18 (# hours to carry over) on the Staff Worksheets to determine how many on-going training hours returning staff will need by December 9, 2026 (anniversary of last Annual Compliance visit). Lead Teacher Requirements – Lead Teachers who do not have the NC Early Childhood Credential (NCECC) or higher or an equivalent have six months to enroll in coursework that will lead to the NCECC (EDU 119 at the local community college) and eighteen months to complete it after enrollment. To remain qualified for a Lead Teacher position, E. Addaman is enrolling in EDU 119 and submit current education for assessment. Annual Inspections - Sanitation and fire inspections are required at least annually by the end of the month of the previous inspection. Send fire inspections to me within seven (7) days of the inspection. Additional Comments: STEP$ - DCDEE is also offering a new salary supplement program for lead teachers, teachers and family child care providers working 40 hours per week with children birth through age two. Early Years administers STEP$ (Supporting Teachers’ Earnings and Progress) on behalf of DCDEE and these supplements are available in every county! Applying is easy. QRIS Rated License Assessment: Prior to today’s visit, your program operated with a Three-Star License earning 1 point in the education component, 5 points in the program standards component (meeting enhanced ratios and space) and 1 quality point for Staff Benefits and Enhanced Parent Involvement. Your rated license re-assessment using the new QRIS rated license rules and procedures was due by December 31, 2026. You submitted your application for re-assessment and your Rated License Assessment Request Review using QRIS Pathway 2: Classroom and Instructional Quality today. 1. Jennifer Bradshaw-Garrett, Child Care Consultant, was present during the visit. C. Wissman, Administrator, assisted me with the visit. Your program currently operates with a three-star license, issued 1/19/25 (capacity change), earning 1 point in the education component, 5 points in the program standards component (meeting enhanced ratios and space) and 1 quality point for Staff Benefits and Enhanced Parent Involvement The last annual compliance visit was conducted 1/27/25. The sanitation inspection was completed 7/3/25 with a “Superior” classification. The last fire inspection was conducted 6/4/25 and your facility was approved for daytime care only. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-seven percent as of 12/5/25. This facility is owned by Foundation Child Development Center LLC. The NC Secretary of State website was reviewed on 12/5/25 and was listed as current- active. License and contact information were current. Contact me to request any changes to your license or contact information. I monitored the classrooms and spaces used for child care and outdoor play areas for applicable child care center requirements using the Child Care Center Requirements, effective 7/1/2025 and Item Numbers Listing, effective 4/2025. I monitored fourteen staff records, six children’s records and all program records. Infants were cared for on their own schedules and when not sleeping or eating were on the floor with toys and materials. Documentation of safe sleep checks was available; feeding plans were updated as needed and posted over cribs. Toddlers were gathered for a story before washing hands and getting ready for lunch. The children, two and three years of age, were using glue to make railroad tracks as a small group during free play. Lunch was observed and consisted of chicken pasta, peas, blueberries and milk. I observed the following violation during today’s visit. Violation Number Comment Rule 1065 Child care providers scheduled to work in the infant room, did not complete ITS-SIDS training within two months of employment or did not complete the training every three years. Child care administrators did not complete the ITS-SIDS training within 90 days of employment and every three years thereafter. A staff person due to complete ITS/SIDS by October 22, 2025 had not completed it. .1102(f) * Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violations documented may impact the compliance history score. The violations documented must be corrected immediately. On or before 12/23/2025, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware that any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted. C. Wissman completed ITS/SIDS training on 10/22/23 was due to renew it by 10/22/25 but has not completed it. You stated that you had signed up for several trainings that were cancelled and are currently registered for one on 12/17/25. Administrators and all staff working in the infant room (Lead Teachers, Teachers, Floaters, Substitutes and uncompensated employees counting in staff/child ratio) are required to complete Infant Toddler Safe Sleep/ SIDS Reduction Training within two months of employment and recertify every three years; however, if an infant is present, there must always be a staff member with safe sleep training in the room. For your compliance letter, state the date you completed the ITS/SIDS training and attach a copy of your certificate. Online payments were due by December 3, 2025. License fees are paid online only, and paper checks are not accepted for payment. Licensed child care facilities are assessed an annual license fee. The amount of your annual license fee is based on the licensed capacity printed on your license as of October 1, 2025, not the number of children enrolled at your facility. Sex Offender Registry and Notification Email - North Carolina General Statute 14-208 requires sex offenders to register with the North Carolina Department of Justice. The law states that a sex offender shall not knowingly reside within 1,000 feet of the property on which any public or nonpublic school or child care center is located. This does not apply to child care centers that are located on or within 1,000 feet of property of an institution of higher education where the registrant is a student or is employed. All licensed child care centers must register to receive e-mail notification when a registered sex offender moves within a one-mile radius of the center. (§14-208.19) To register for the e-mail notification, go to http://sexoffender.ncsbi.gov. If you have any questions, please contact your local sheriff's department. QRIS (Quality Rating Improvement System) Technical Assistance: C. Wissman, Owner/Administrator, and I discussed the facility’s plan for completing a rated license assessment using the new QRIS rules found in Section .3200 of the NC Child Care Rules. All information on the QRIS rated license assessments and documents will be posted at https://ncchildcare.ncdhhs.gov/Provider/Licensing/Star-Rated-License/QRIS-Modernization and will also be provided through emails and Raise NC newsletters. During the visit we completed the QRIS Conversation Template with a focus on the selected pathway. Pathway 2: Classroom and Instructional Quality Since you are electing to complete Pathway 2, we focused on those components. *We reviewed current staff education (50% of Lead Teaches and 50% of Other Educators) to determine the facility’s expected star level based on education currently available in Works is 3 Stars but will likely be 4 or 5 Stars once education is processed. Any staff person who will be evaluated as part of the Lead Teachers or Other Educators needs to register for a Works account and submit original, official transcripts or update their current Works account and submit new education. *We reviewed the Family and Community Engagement Plan, including the Foundational Practices and Optional Practices. This will be completed annually. The operator will create a QRIS notebook to collect documentation of the practices including pictures, flyers, newsletters, emails, etc. *We walked through the Individual and Facility Continuous Quality Improvement (CQI) forms (individual and center). Each staff member will complete an Individual CQI annually. (This will also count as their Professional Development Plan!) The administrator will also complete the Facility CQI Plan to create an overall goal for the center. Maintain these in your staff files and include a copy in the QRIS notebook. We reviewed the list of approved curriculums and formative assessments that are required for each age group. You are using Lilio for all ages, but the formative assessment is not yet approved. I will send information for you to send to the publisher so they can submit for approval. We discussed options for five hours of additional training (on-going training or CEUs) or coaching/mentoring for the administrator and lead teacher. Your rated license assessment is currently due by 12/31/2026 but your requested timeline is to complete this by the end of Spring 2026. I observed and documented the following violations during today’s visit. Violation Number Comment Rule 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. A staff person hired on June 25, 2023 did not complete first aid until December 9, 2023. A staff member hired on July 10, 2023 did not complete first aid until December 9, 2023. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. I monitored the classrooms and spaces used for child care, kitchen, and outdoor play area for applicable child care center requirements using the Child Care Center Requirements, effective 7/1/2023 and Item Number Listing, effective 6/2022. I monitored seven staff records and all program records. The facility does not offer transportation. Center Observations: Parent information was posted at the entrance. Parents sign in on ProCare, but teachers also document arrival and departure on a paper roster in the classroom. Infants were cared for on their schedules. Teachers were playing on the floor with the awake babies, being held for bottle feeding, and sleeping. Documentation of safe sleep checks was available; infant feeding plans were posted. The toddlers were on the playground. Shade from the trees and a breeze made the hot summer morning pleasant. The teachers were singing songs with them. They climbed on the portable climber and played with portable gross motor toys. The children, three years of age, from Space 2b were on the larger playground. The teacher played soccer with a few children. They dug in the sand and chased lady bugs. The preschool class was engaged in indoor free play with developmentally appropriate toys and materials. Lunch was chicken nuggets, French fries, corn, peaches, and milk. Children ate at child-sized tables with close supervision from teachers. Interactions were warm and nurturing. I observed and documented the following violations during today’s visit. A full qualification letter was not available for a staff member who was hired on March 15, 2023 with an initial provisional qualification letter. Children under six years of age account for over half of those potential poisonings. The substances most involved in poison exposures of children are cosmetics and personal care products, cleaning substances, and medications. Two bottles of bleach were stored on the floor in the unlocked mop closet. You stated that a staff member must have forgotten to relock the door this morning and will remind staff to lock all doors and cabinets used to store cleaning supplies. Thank you for correcting this during the visit. Check doors to storage closets frequently to make sure they are locked. Initial Criminal Background Check (CBC) – Criminal Background Checks ensure that staff have not committed any crimes which could potentially make them unsafe to be around children and other staff. Because Abuse and Neglect registries are also checked, it helps to ensure that children are protected from physical, mental, and sexual abuse. Performing diligent background screenings also protects the child care facility against future legal challenges. Two staff hired on 3/15/2023 and 6/3/2023 did not have full Qualification Letters in their files. You stated that they had emailed them to the Assistant Director, and she had not forgotten to print them. You printed them during the visit. Use the staff file checklist to verify that all required staff files are printed and available in staff files. On-going Training – On-going training and professional development provides early learning staff opportunities to learn new skills and/or refresh previously learned skills. During your Annual Compliance visit on 3/30/2023, I monitored on-going training for staff. C. Stevens and S. Ashburn had completed 2 of 10 hours of on-going training and had until 5/16/2023 to complete the other 8 hours. Today, I monitored their on-going training for an additional 8 hours, but documentation was not available. You stated that they intended to take training but had forgotten to follow through. Planning for on-going training as part of the annual Professional Development Plan helps the administrator and staff person map out completion of on-going training for the upcoming year. On-going training is tracked from Annual Compliance visit to the one year anniversary. Have them complete at least 8 hours of on-going training by August 21, 2023. You can now request to have your transcripts sent electronically from colleges attended. *When education is posted, staff need to log back into Works and print a copy of their Education Status Letter for their file. Nutrition – Consider having the cook plate food in the kitchen so that when children wash their hands and sit down at the table, their lunch can be served expeditiously to reduce the amount of waiting time. Have enough food to offer seconds upon request. When substitutions are needed to be made for menu items, they must be made on every posted menu in the center. Menus are only required to be posted in a central location where they are visible to parents and in the kitchen. Sunscreen – If you choose to administer sunscreen, consider purchasing one brand for all children to avoid the confusion of getting the right sunscreen on the right child. I monitored medication forms for both sunscreen and diaper cream. They were completed correctly. Playground - This time of year, check the playground equipment daily for wasp nests, vermin, spiders, ant hills and debris before children go out. Anchored climbing equipment with a critical height less than 5 feet is required to have at least 6 inches of wood mulch with no weeds or grass in the six-foot fall zone. (However, you are encouraged to begin preparations now so that you are ready when your cohort group is due!) Your child care center is due for a three-year rated license reassessment by 12/4/2024 and has been assigned to Cohort 3. The Cohort 3 Rated License Preparation Year is from 7/1/2025 – 6/30/2026. During your preparation year, if you are interested in having the Environment Rating Scales completed, review the ITERS-R and ECERS-R by obtaining the manuals for each classroom and looking for assessment related resources at ncrlap.org. Request technical assistance and training for your staff from Albemarle Alliance for Children and Families (AACF). You can request and complete an ITERS-R and ECERS-R during your preparation year. If your scores help you meet or exceed your current star rating and you are ready to move forward with your rated license assessment, you may choose to do so during your preparation year. Your preparation year is also the time to work on staff education and ensure that Works accounts are completed and up to date for staff members. In 2021, you scored the lowest on Subscale1 of the ECERS-R: Space and Furnishings. In addition, the reading center was next to the music center. By arranging your centers so that quiet and active centers are not next to each other, children in quiet centers can play undisturbed. Item #5 – Space for privacy – The tent was limited to use by one or two children; however, this area was not available for at least 3 hours and 30 minutes (SPOD required for program operating 10.5 hours per day). I monitored the classrooms and spaces used for child care, kitchen, and outdoor play area for applicable child care center requirements using the Child Care Center Requirements, effective 7/1/2023 and Item Number Listing, effective 6/2022. I monitored seven staff records and all program records. The facility does not offer transportation. Center Observations: Parent information was posted at the entrance. Parents sign in on ProCare, but teachers also document arrival and departure on a paper roster in the classroom. Infants were cared for on their schedules. Teachers were playing on the floor with the awake babies, being held for bottle feeding, and sleeping. Documentation of safe sleep checks was available; infant feeding plans were posted. The toddlers were on the playground. Shade from the trees and a breeze made the hot summer morning pleasant. The teachers were singing songs with them. They climbed on the portable climber and played with portable gross motor toys. The children, three years of age, from Space 2b were on the larger playground. The teacher played soccer with a few children. They dug in the sand and chased lady bugs. The preschool class was engaged in indoor free play with developmentally appropriate toys and materials. Lunch was chicken nuggets, French fries, corn, peaches, and milk. Children ate at child-sized tables with close supervision from teachers. Interactions were warm and nurturing. I observed and documented the following violations during today’s visit. Violation Number Comment Rule 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. Two bottles of bleach were stored on the floor in the unlocked mop closet. .2820(b) 1052 Staff required to receive on-going training had not completed the required number of hours according to their education and experience. Two staff members needing ten hours of on-going training by May 16, 2023 had documentation of only completing two hours of on-going training. .1103(a) 1757 A valid qualification letter was not on file and available to review at the facility. A full qualification letter was not available for a staff member who was hired on March 15, 2023 with an initial provisional qualification letter. A qualification letter for a staff member hired on July 3, 2023 was not available. G.S. 110-90.2(b) & (d) & .2703(e) * Child care programs are expected to achieve and always maintain compliance and are required by NC GS 110-90(4)(d) to achieve and maintain an 18-month compliance history score of at least 75%. Any violations documented may impact the compliance history score. I completed an electronic copy of the visit summary and provided a printed copy of it to C. Wissman, Administrator. We reviewed the visit summary and violation(s) documented during today’s visit. Correct these immediately. Send evidence of correction to me so that I receive it no later than 8/21/2023. The compliance verification letter needs to state the facility name and license number and list each violation by Item # and describe in detail when and how each violation has been corrected. TECHNICAL ASSISTANCE WITH DOCUMENTED VIOLATIONS: Storage of Hazardous Items - There are over two million human poison exposures reported to poison centers every year. Children under six years of age account for over half of those potential poisonings. The substances most involved in poison exposures of children are cosmetics and personal care products, cleaning substances, and medications. Two bottles of bleach were stored on the floor in the unlocked mop closet. You stated that a staff member must have forgotten to relock the door this morning and will remind staff to lock all doors and cabinets used to store cleaning supplies. Thank you for correcting this during the visit. Check doors to storage closets frequently to make sure they are locked. Initial Criminal Background Check (CBC) – Criminal Background Checks ensure that staff have not committed any crimes which could potentially make them unsafe to be around children and other staff. Because Abuse and Neglect registries are also checked, it helps to ensure that children are protected from physical, mental, and sexual abuse. Performing diligent background screenings also protects the child care facility against future legal challenges. Two staff hired on 3/15/2023 and 6/3/2023 did not have full Qualification Letters in their files. You stated that they had emailed them to the Assistant Director, and she had not forgotten to print them. You printed them during the visit. Use the staff file checklist to verify that all required staff files are printed and available in staff files. On-going Training – On-going training and professional development provides early learning staff opportunities to learn new skills and/or refresh previously learned skills. During your Annual Compliance visit on 3/30/2023, I monitored on-going training for staff. C. Stevens and S. Ashburn had completed 2 of 10 hours of on-going training and had until 5/16/2023 to complete the other 8 hours. Today, I monitored their on-going training for an additional 8 hours, but documentation was not available. You stated that they intended to take training but had forgotten to follow through. Planning for on-going training as part of the annual Professional Development Plan helps the administrator and staff person map out completion of on-going training for the upcoming year. On-going training is tracked from Annual Compliance visit to the one year anniversary. Have them complete at least 8 hours of on-going training by August 21, 2023. Send copies of their training certificates and their completed On-going Training Logs with your compliance letter. They will need an additional ten hours by 3/30/2024. *The Health and Safety Training available on Moodle on the DCDEE website and other free trainings can also be used to help staff meet on-going training hours. *Also available on Moodle is a 14-hour Infant/Toddler Orientation that you can complete for on-going training hours and a training on Supporting Children and Families Experiencing Homelessness. *Trainings on the environment rating scales that count as on-going training hours are available on the NC Rated License Assessment Project (NCRLAP), www.ncrlap.org *ECE college coursework or coursework leading to an ECE degree counts towards on-going training hours. Health and Safety Training and Five (5) Year Cycle Tracking Requirements – Review Row 17 on Staff Worksheet for staff members needing initial H&S Training. New staff are required to complete Health & Safety Training (H&S Training) within one year of hire (CPR/First Aid and Recognizing and Responding to Suspicions of Child Maltreatment within 3 months of hire) and then complete training in all topic areas every five years. On-going Training Hours – Review Row 18 (# hours to carry over) on the Staff Worksheets to determine how many on-going training hours returning staff will need by 3/30/2024 (anniversary of last Annual Compliance visit). New staff will complete Health & Safety in their first year and then will complete on-going training annually based upon their level of education posted in DCDEE Works. Staff Education and Works - Formal education (NCECC, college coursework, Principal and Teaching Licenses, BSAC training certificates, and Early Educator Certificates) are submitted to DCDEE through WORKS, an on-line portal where you can register, request to be approved for various positions, and access your education information once evaluated and posted. You can now request to have your transcripts sent electronically from colleges attended. *When education is posted, staff need to log back into Works and print a copy of their Education Status Letter for their file. Nutrition – Consider having the cook plate food in the kitchen so that when children wash their hands and sit down at the table, their lunch can be served expeditiously to reduce the amount of waiting time. Have enough food to offer seconds upon request. When substitutions are needed to be made for menu items, they must be made on every posted menu in the center. Menus are only required to be posted in a central location where they are visible to parents and in the kitchen. Sunscreen – If you choose to administer sunscreen, consider purchasing one brand for all children to avoid the confusion of getting the right sunscreen on the right child. I monitored medication forms for both sunscreen and diaper cream. They were completed correctly. Playground - This time of year, check the playground equipment daily for wasp nests, vermin, spiders, ant hills and debris before children go out. Anchored climbing equipment with a critical height less than 5 feet is required to have at least 6 inches of wood mulch with no weeds or grass in the six-foot fall zone.
