A checklist was used to note the requirements I monitored today. Kim Barnes Child Care Consultant assisted with today’s visit. Staff and training worksheets were received during the visit. You, L. Smith, Administrator, were present when I arrived and you were notified of the purpose of today’s visit. This facility currently operates with a GS 110-106 Notice of Compliance issued August 13, 2012. The last annual compliance visit was conducted June 3, 2025. The sanitation inspection was completed August 25, 2025, with an “Superior” classification. The fire inspection was conducted June 7, 2025, and the facility was approved for day and night - time care. Fifty-seven(57) children were present. Infants were observed interacting with assigned caregivers, taking naps, and participating in play. Other children present were observed playing outside, and having routine care needs met. Prior to today’s visit, the program’s compliance history was 88% over the current 18- month period. A copy of your compliance history report was left with you. The contact information that is on file with DCDEE was verified to be current. Cutting Edge Revelational Ministries, INC., is this facility’s current owner. Upon arrival, Lee Ann Smith, Child Care Administrator, was present and available to assist. I informed Ms. Smith of the purpose for my visit. This facility currently operates with a GS 110-106 Notice of Compliance issued June 9, 2008. Limited, but required monitoring was completed. Enrollment was not completed. The classroom for two (2) year old children and the outdoor environment was observed today. When asked about any recent parent or staff concerns, you, Mrs. Smith, stated that you have no concerns, and no one has expressed any concerns to you. The teachers in the classroom for two (2) year old children were interviewed. ALLEGATION 1: There is a concern that the gate door of the fence surrounding the playground is not always shut or locked and the classroom is not a safe environment. A walk-through of the facility was completed, specifically the classroom for two-year-old children and the outdoor environment. I observed all the gates to the fences secured by the latch and closed. The two-year old children were observed in the outdoor environment. When asked, Ms. Smith stated that the gates remain closed, especially if children are on the playgrounds. It was further stated that the teachers ensure that the gates are closed prior to the children going outside. A teacher stated that prior to the children going outside, the playgrounds are checked for hazards. It was stated that the gates are always checked to ensure the latches are secure and the gates are closed. It was stated that no children have ever walked outside of the gates. When asked if any children had received any bruises or had any incidents in the last month, it was stated that the classroom did have two children that were biters. It was further stated that the children that bit would be separated from children if needed and it was discussed with parents. It was stated that a child was not watching where he was going and ran into a cot while the teacher was putting the sheets on the cot. Shortly after the child ran into the cot, a bruise appeared on the eye of the child. The teacher stated she messaged the parent to let them know the status of the child. When asked, the teacher stated an incident report was not completed. Based on my observation, staff report, and my investigation, this allegation is not substantiated. Although the allegation was not substantiated, a violation was documented concerning incident reports. I reviewed this information with Ms. Smith at the conclusion of the visit. RESOURCES: BITING IN THE CLASSROOM: Biting is a typical behavior often seen in infants, toddlers, and 2-year olds. As children mature, gain self-control, and develop problem-solving skills, they usually outgrow this behavior. Intervene immediately between the child who bit and the bitten child. Stay calm don’t overreact, yell or give a lengthy explanation. Use your voice and expression to show that biting is not acceptable. Look into the child’s eyes and say calmly but firmly, “I do not like it when you bite people.” For a child with more limited language, just say “No biting people.” Point out how the biter’s behavior affected the other person. “You hurt him and he’s crying.” Encourage the child who was bitten to tell the biter “You hurt me.” Encourage the child who bit to help the other child by getting the ice pack, etc. Offer the bitten child comfort and first aid. Wash broken skin with warm water and soap. Observe general precautions if there is bleeding. Apply an ice pack or cool cloth to help prevent swelling. If the bitten child is a guest, tell the parents what happened. Suggest the bitten child be seen by a health care provider if the skin is broken or there are any signs of infection (redness or swelling). Reinforce desired behavior. Notice and acknowledge when you like what your child is doing, especially for showing empathy or social behavior, such as patting a crying child, offering to take turns with a toy or hugging gently. Do not label, humiliate or isolate a child who bites. Discourage play which involves “pretend” biting, or seems too rough and out of control. Help the child make connections with others. This is a legislatively mandated effort to address lead and asbestos hazards in North Carolina public schools, licensed child care centers, and licensed family child care homes. Participation allows facilities to meet all rule requirements for identified lead and asbestos hazards, as required by law. We reviewed your lead in water test results were received on March 8, 2024; therefore, you are not required to retest until March of 2027. We reviewed that you are exempt from completing the lead in paint according to the information retrieved from the Clean Water for US Kids website. The lead in asbestos survey is being reviewed. CHILDREN AND STAFF RECORDS: Ten percent (10%) or more of children records were monitored. Children records reviewed were in compliance. The last annual compliance visit was conducted June 5, 2023. The sanitation inspection was completed July 20, 2023, with an “Superior” classification. The fire inspection was conducted June 7, 2023, and the facility was approved for day and night - time care. The teaching staff stated that this medication was not administered to the child. We reviewed that prior to or once medication expires, it must be given back to the parent to avoid the medication being accidently given after it’s expiration date. This violation was corrected when the medication was removed from the classroom.
