The visit was conducted with you, Dana Lunsford, administrator. You provided me with applicable program, staff, and children’s records for review. Kim Ward, Technical Assistant with the Partnership for Children of Lincoln and Gaston Counties, was present upon my arrival reviewing QRIS pathway options with you. The North Carolina Secretary of State website was viewed before the visit and Mini Academy, Inc. was current/active as of October 15, 2025. The facilities contact information was reviewed and confirmed with you. This program currently operates with a Three star rated license effective July 12, 2019. The program’s compliance history was 98 percent prior to today’s visit. Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. A checklist was used to note the requirements I monitored today. Children and staff were observed during free play activities indoors, gross motor activities outdoors, lunch, and nap time. All staff were listed on the ABCMS Provider Portal roster for this facility. The most recent sanitation inspection for your facility was conducted on March 10, 2025. A superior sanitation classification was issued with 4 demerits noted on the grade card. The most recent lead water testing results were completed on October 27, 2023. Lead testing must be completed every three years. You may review your results by visiting https://www.cleanwaterforuskids.org/en/carolina/. The website indicated this facility was exempt from lead paint testing. The website indicated no asbestos hazards were identified for this facility. The most recent fire inspection for your facility was conducted on November 4, 2024. I suggested that you contact your local fire inspector and request a fire inspection for your facility. The most recent monthly fire drill was conducted on October 6, 2025, at 9:25am. The most recent quarterly lockdown/shelter-in-place drill was conducted on October 7, 2025, at 10:55am. The facilities EPR Plan was updated on January 22, 2025. The most recent monthly playground inspection was completed on October 7, 2025, by Dana Lunsford. The violation observed today was discussed with you and documented in the Visit Summary left with you at the conclusion of this visit. The following violation was observed: Violation Number Comment Rule 858 Plastic bags, materials that could be torn apart and toy parts small enough to be swallowed were accessible to children under three years of age. In space #1b, a foam eraser used for a chalkboard was observed on the art shelf accessible to children two years of age. .0604(q) The violation observed was corrected during the visit. Technical Assistance was provided on the following: Foam products- The Lead Teacher discarded the foam eraser during the visit. I suggested that administrative staff monitor all classrooms with children under three years of age daily to ensure plastic grocery bags, plastic packaging, small toys, small art supplies, foam, and other potential choking hazards are inaccessible to children or removed from the classroom. I suggested that you review safety requirements with all staff members individually and during an upcoming staff meeting. I suggested that you create a daily safety checklist for staff to utilize each day to ensure a safe environment. Plastic bags, toys, toy parts small enough to be swallowed, and materials that can be torn apart, such as foam rubber and styrofoam, shall not be accessible to children under three years of age. However, styrofoam plates and larger pieces of foam rubber may be used for supervised art activities and styrofoam plates may be used for food service. Consultation: -Staff who do not have current CPR and First Aid certification cannot open, close, be alone at the facility caring for children, transport children, or be listed on the Emergency Medical Care Plan to respond to medical emergencies. -I suggested that you contact your local Environmental Health Specialist and inquire about the discolored areas on the floor of your bathrooms. -We reviewed and discussed transportation permission form requirements. Before children are transported, written permission from a parent shall be obtained that shall include when and where the child is to be transported, expected time of departure and arrival, and the transportation provider. Parents may give standing permission, valid for up to 12 months, for transport of children to and from the center not including off premise activities as described in Child Care Rule .1005. -We reviewed and discussed activity plans. Ensure that staff are incorporating the activities listed on their activity plan by adding the necessary materials in each activity area, as applicable. We discussed certain activities and ideas to enhance the activity areas. QRIS Pathways to the Stars: -We reviewed Pathway #1, #2, and #3, Family and Community Engagement Foundational Practices, Continuous Quality Improvement, approved curriculum, formative assessment tools, staff to child ratio options, and staff education options. I encouraged you to review this information more thoroughly by visiting https://ncchildcare.ncdhhs.gov/Provider/Licensing/Star-Rated-License/QRIS-Modernization. -I suggested that you review Environment Rating Scale information and resources at www.ncrlap.org for ECERS-3 and SACERS-U, should you choose Pathway #1. -Reach out to the Partnership for Children of Lincoln and Gaston Counties for additional technical assistance and support at 704-922-0900. -I suggested that you complete the Staff Information and Education Worksheet, an Excel document, and submit the completed form to me for review of your current staff education. The visit was conducted with you, Dana Lunsford, Administrator. You provided me with applicable program and staff records for review. This program currently operates with a Three star rated license effective July 12, 2019. The program’s compliance history was 99 percent prior to today’s visit. Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The NC Secretary of State website was reviewed on April 16, 2025, and Mini Academy, Inc. was listed as current/active. Limited monitoring was conducted. Staff and children were observed during free play activities indoors and music and movement activities. The outdoor learning environment was remeasured during today’s visit. An updated playground diagram and outdoor space calculation worksheet will be sent to you once completed. The most recent sanitation inspection for your facility was conducted on March 10, 2025. A superior sanitation classification was issued with 4 demerits noted on the grade card. The most recent lead water test was completed on October 27, 2023. Lead water testing must be completed every three years. Technical Assistance: Choking Hazards: The lead teacher moved the plastic package of wipes and bundle of plastic grocery bags to a shelf above five feet from the floor during the visit. Plastic bags, materials that could be torn apart and toy parts small enough to be swallowed must be inaccessible to children under three years of age. I suggested that administrative staff monitor all classrooms for children under three years of age daily to ensure plastic grocery bags, plastic packaging, small toys, small art supplies, and other potential choking hazards are inaccessible to children or removed from the classroom. I suggested that you review safety requirements with all staff members individually and during an upcoming staff meeting. I suggested that you create a daily safety checklist for staff to utilize each day to ensure a safe environment. Consultation (the following items were reviewed today): -We discussed medication permission forms and specifics pertaining to when to apply certain medications, such as diaper rash creams and medicated lip balm. Refer to the manufacturer's instructions to ensure the medication is not administered more frequently than the manufacturer's instructions indicate. Also, have a conversation with parents to discuss their preference for applying diaper rash cream when a rash appears versus at each diaper change. -Per Senate Bill 425, facilities are currently held harmless from star rated license assessments. -Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. You stated you completed the training. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS on an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. The visit was conducted with you, Dana Lunsford, Administrator. You accompanied me on the walkthrough of the facility and provided me with applicable program records for review during the visit. When medical treatment is required by a health care professional, community clinic, or local health department as a result of an incident occurring while the child is in care, a copy of the incident report shall be mailed to a representative of the Division, in this case your Child Care Consultant, within seven calendar days after the incident. You stated you were unaware of this child care requirement. I suggested that you review all incident report requirements 10A NCAC 09 .0802 (e-g), with all staff. The child care provider shall complete an incident report each time a child is injured as a result of an incident occurring while the child is in care. This incident report shall include: (1) facility identifying information; (2) the child's name; (3) date and time of the incident; (4) witness to the incident; (5) time the parent is notified of the incident and by whom; (6) piece of equipment involved, if applicable; (7) cause of injury, if applicable; (8) type of injury, if applicable; (9) body part injured, if applicable; (10) where the child received medical treatment, if applicable; (11) description of how and where the incident occurred, and the First Aid received; and (12) steps taken to prevent reoccurrence. The schedule shall be current and shall include the: (A) location of the activity; (B) purpose of the activity; (C) time the activity will take place; (D) date of the activity; and (E) name of the person(s) to be contacted in the event of an emergency. (6) Each time that children are taken off the premises, staff shall take a list of the children participating in the activity with them. Staff members shall use this list to check attendance when leaving the center, periodically when the children are involved in the activity, before leaving the activity to return to the child care center, and upon return to the center. A list of all children participating in the off-premise activity shall also be available at the center. -We reviewed the following child care requirement, 10A NCAC 09 .0713(a)(4) when determined to be developmentally appropriate by the operator and parent, a child age two or older may be placed one age level above his or her chronological age without affecting the staff/child ratio for that group. This provision shall be limited to one child per group. You stated you have one child who meets the requirements of this rule in space #4. Documentation from the parent was available for review. Rated License Reassessment: To prepare for the reassessment process, DCDEE has developed a cohort system. There are three cohorts, and each includes a preparation year and a reassessment year. Your facility is in cohort two. The preparation year for cohort two will begin July 1, 2024. The reassessment year will be July 1, 2025, through June 30, 2026. Rated License Reassessment: To prepare for the reassessment process, DCDEE has developed a cohort system. There are three cohorts, and each includes a preparation year and a reassessment year. Your facility is in cohort two. The preparation year for cohort two will begin July 1, 2024. The reassessment year will be July 1, 2025, through June 30, 2026.
